This program reaches out to individuals who are homeless, and or engaging in using substances. * Provides a harm reduction approach to care including providing Narcan, Fentanyl test strips, and provides resources for safe usage. The intent is for the program to wrap individuals with support and assist in getting the individual connected with services.
Provides supportive services to very low income Veterans and their families who are homeless or are in danger of losing their homes. Services include outreach, case management, and assistance obtaining VA and other benefits. Provides time-limited payments to third parties, such as landlords, utility companies, moving and storage companies, and child care providers, if those payments can help Veteran families stay in or obtain permanent, sustainable housing.

The CAPTAIN Street Outreach Program carries out services related to helping homeless and troubled youth leave the streets and secure appropriate, safe living arrangements, and to prevent the occurrence or reoccurrence of homelessness. Street Outreach is also in the community to help any youth who are in, or run the risk of becoming involved in a crisis. Street Outreach engage youth in their environment offering basic survival needs such as food, clothing, hygiene products, and access to emergency shelter as well as referrals, intervention, and exposure to positive adult role models. The purpose of Street Outreach is to aid youth in making positive decisions in their own lives to remain happy, healthy, and contributing members of society.

The Rural Outreach Workers can assist with the same needs as the downtown outreach workers, except that they will be mobile, traveling across the entire Tompkins County with a focus on "high needs" areas and rural communities where there tend to be service deserts. A rural Outreach Worker can assist if someone is in need of food and pantry items, if someone is in distress but is not elevated to an emergency level (needing someone to talk with, emotional support), to refer to services, and link to resources.

Runs a mobile unit that reaches out to individuals experiencing homelessness, offering to bring them to shelter, and providing food, water, clothing, blankets, and other support.
Assist the unsheltered in Yonkers with community resources, entry point in pathway to services,. Help by transporting them to Department of Social Services, where they can apply for food stamps and medical or mental health services. They accept donations such as socks, hats, gloves, scarves. No acceptance of clothing (i.e., pants, shirts, undergarments) at this time.

Provides "on the street" services to Long Island's unsheltered persons in need. Trained members offer crisis intervention and referrals for essential services, including emergency housing. Team members also distribute kits with clothes, non-perishable foods, blankets, and toiletries to individuals living on the street. * Assembles a group of volunteers in the community to perform wellness checks, distribue basic needs items, clean up needles in "hot zones', and outreach. Clean ups by request.* Offers onsite health resources, food, wound care kits, Nalaxone, blankets, seasonal clothing, Narcan training and doses, and hotline information.
Offers programs, support, and referrals to homeless individuals, including legal and medical clinics. Individuals are offered acute medical assistance, blankets, water, food, and transitional housing.
* Provides community outreach, education, and support for those affected by addiction: 24-hour help line. Referrals to recovery services. Trainings and demonstrations for schools, churches, and other groups. * Organizes a weekly community cleanup of sharps and other hazardous materials. * Provides a weekly popup outreach station for community members in need, which offers free lunches, toiletry items, harm reduction supplies, and clothing.

Offers an outpatient treatment-referral program designed to serve individuals between the ages of 12 and 35 who are at risk or who have been sexually exploited/human trafficked, at-risk of sexual exploitation/human trafficking, runaway or are homeless. Services provided include individual and family counseling, street outreach, GED classes, DSS advocacy, life skills education, housing assistance, and education and job placement and are tailored to meet all the needs of each individual utilizing services.
