Day program for older adults and adults with developmental disabilities. Provides socialization, community involvement, nutritional program, transportation, recreation, and case management. Helps older adults and adults with developmental disabilities remain at home and gives caregivers time away from caregiving to help them do whatever they need to do, whether that be running errands, attending appointments, or going to work. Respite services are also offered at this location; clients can select which days and times to attend.

Provides Family Care and Individual Residential Alternatives (IRA) for developmentally disabled persons ages 18 and older who are HCBW eligible. Intermediate Care Facility is available for those who are Medicaid eligible. Transportation is available for all. Supportive apartments are also available, with limited transportation. Provides respite care allowing families of developmentally disabled persons to take a break from the daily routine of care-giving duties. Currently, two locations in a certified home can offer respite for as little as one day or for a few weeks. Reservations are encouraged; however, emergency accommodations are available. Provides At-Home Residential Habilitation service that assists individuals in improving the skills necessary to live in the community while living at home. Qualified staff work one-on-one with individuals to help them learn socialization, recreation, daily living, and mobility skills.

Weekend respite program offering a place for children and adults with developmental disabilities to spend time away from home, including overnight accommodations with trained Direct Support Professionals providing oversight of safety, recreational activities, medical needs, and medication administration. Some transportation is provided during a stay at the Respite Center. A limited amount of respite is offered to each family. Weekend respite available Thursday afternoon thru Monday morning.

Offers respite care up to 42 days in a calendar year, for the relief of primary caregivers. Individuals in the respite program will be provided with a room, some assistance with personal care, appropriate meals, activities, and 24 hour supervision.
Offers short term respite care for families or caregivers who need help caring for someone who need some form of continuous care when they are away.
Provides periodic respite breaks for primary caregivers while disabled relative is cared for at Rising Treetops at Oakhurst. Disabled family member lives among peers and and has access to seasonal recreational activities including crafts, athletics, swimming, games and TV. Also offers overnight respite where registered families can reserve time in advance. Day respite is available to residents of Monmouth County.

Offers once-a-month respite program for caregivers to drop of individuals living with memory loss. Individuals participate in crafts, music, games, meals and more. Lunch is provided.
Housing Supports are designed to help people live as independently as possible in a community setting through person-centered supports and services. Residences range in size and with varying levels of support for people with intellectual and developmental disabilities of all ages. Homes and apartments are located throughout Chemung and Schuyler counties. All residential options are designed to be family-like in all-male, all-female, or co-ed settings. Direct Support Professionals are on-site 24-hours-a-day to assist with cooking, housekeeping, and daily life skills. The supportive apartment program supplements the needs of people who are living on their own, but who may need periodic financial and staff supports to retain their independent living. Also offers a respite program to families who are responsible for the daily care of a loved one with a developmental disability and need relief.

Provides opportunities for patients and caregivers to maintain social contacts.
Designed to meet the need of children diagnosed with autism, ages 3-7. Classroom size is 6 children, grouped by age and developmental level. This program runs 3 Saturdays per month and include creative, social activities that help children with ASDs better develop coping and interactive skills with others. Attendees will be rotated through program on a schedule of 2 consecutive Saturdays a month. The program also runs simultaneously with vacation respite.
The goal of this after school, out-of-home program is to teach the youthful participants socialization and recreational skills while giving their parents/guardians respite.
