Offers respite care for individuals with disabilities to allow their families and care takers time to address personal matters:

• Basic medication administration

* Provides in-home respite care for the following: Individuals with a developmental disability and who have been approved for eligibility through OPWDD. Medically frail individuals who have aged out of school services or are close to aging out.

Provides families of individuals with developmentally disabilities with temporary relief from caregiving responsibilities. Services are provided in the home setting or through community-based recreational outings by a respite worker.

Provides respite services for families of children with developmental disabilities, that can be in the home or out of the home, during the day, evenings or overnight.

Provides in-home and out-of-home respite care for families and caregivers of children diagnosed with severe emotional disabilities.

ADAPT provides respite for parents who have a child with developmental disabilities and are enrolled in a skill building program.

Provides periodic respite breaks for primary caregivers while disabled relative is cared for at Rising Treetops at Oakhurst. Disabled family member lives among peers and and has access to seasonal recreational activities including crafts, athletics, swimming, games and TV. Also offers overnight respite where registered families can reserve time in advance. Day respite is available to residents of Monmouth County.

Offers a skilled nursing care unit for veterans requiring extended or long-term care due to age, illness or disabilities. Also provides hospice and palliative care, including out-of-home respite care.

ARC provides various programs to children with serious emotional disturbances residing with their family or guardian. The programs are as follows: Overnight Children's Respite Care: Provides overnight respite care to children in a community based setting, In-Home Respite: Provides individual oversight and supervision to children, in their homes or in the community at times parents/guardians need assistance.

Provides individualized plan of care for terminally ill patients and their families. Coordinated team of health professionals and volunteers include registered nurses, home health aides, medical social workers and physical, clinical dietician, occupational and speech therapists. Offers symptom control and pain management, acupuncture, pastoral care, respite care and bereavement counseling.

This program is about seniors helping seniors. Low-income seniors receive training and a small stipend to provide chore services, friendly visiting, respite to caregivers and technology assistance. Volunteers are background checked, can volunteer 5-40 hours per week, and are matched with seniors in the community who need help.

Helps improve the quality of life for individuals living with Alzheimer's Disease and related memory disorders and their caregivers. Also offers a unique social model day program, in home respite, support groups, Brainfitness Workshops, Whimsical Wednesdays, and educational workshops.

Provides weekly respite care services designed to help families care for their children who have a serious emotional disturbance (SED) diagnosis and to foster an increase in the child's socialization skills.

100-unit supervised home for older adults. This facility is licensed as an adult home. Adult homes provide long-term residential care with 24-hour supervision and a full range of services such as meals, housekeeping, some personal care and social activities. Adult homes are licensed and regulated by the New York State Health Department.

Offers time off for caregivers living with a loved one with mental illness. Provides assistance and education to the family members while assisting their loved one to become more independent. Services are provided in the home as well as in the community and are geared toward skill development, socialization and becoming involved in the community.

Offers Nassau and Suffolk County residents with intellectual and/or developmental disabilities and their families the ability to select services and providers that are most suited to their needs. Offers self-directed community habilitation, respite and SEMPT (supported employment). Assisted by a Fiscal Intermediary and Support Broker, individuals and family members learn how to implement HCBS (Home & Community Based) waiver services establish a habilitation plan, assess needs, and budget allocated Medicaid funds.

Provides in-home respite care for families caring for a child or adult with developmental disabilities.

Provides in-home relief of primary care giving responsibilities for the caregiver. The program services developmentally disabled, adults and children with OPWDD eligibility in Nassau and Suffolk counties living in their family home.

Provides in-home visits, counseling, and resources for caregivers and individuals with Alzheimer's and Dementia, including specific assistance and training, monthly support groups throughout the county, articles and information specific to caregivers, and respite care (in home or facility based). Provides supportive counseling for family caregivers through Family Services of Jamestown. Support group meets at Christ First United Methodist Church, 663 Lakeview Avenue, Jamestown. Hearts and Hands is a volunteer based non-profit organization that provides free escorted transportation, companionship, and in-home supports for older adults in communities throughout Erie and Eastern Niagara Counties. Our care recipients are often individuals who stopped driving due to age-related medical conditions or for financial reasons. Recipients may also need assistance at home with light yard work and home maintenance, or companionship to renew and refresh their caregiver. Our elderly neighbors are often lonely, isolated, and in need of friendly human interaction. In response to a recent community assessment and a national assessment from AARP, Hearts and Hands has designed our program to address social isolation through all three avenues of service: transportation, household supports, and companionship. Hearts and Hands volunteers may be the only people our care receivers see during a week, and their company is, in and of itself, a bright spot in daily life. Hearts and Hands is a best-efforts program. Services are provided by volunteers who give their time as they have it available. Provides adult respite care for developmentally disabled individuals, age 21 and over, who reside in Suffolk/Nassau counties with their families. Limited geographic area may be eligible for transportation periodically. Universal medical application and site visit appointment required.

Provides temporary care to people with developmental disabilities to relieve the burden of the primary caregiver. Caregivers fill out voucher forms and submit them to the agency for payment for temporary help.

Volunteers may provide transportation to essential services and appointments, run errands, assist with grocery and personal shopping, or pick up prescriptions, plan a regular phone visit or home visit to provide friendly and supportive listening, prepare/deliver a weekly meal or treat, perform general yard work or light housekeeping, and/or provide respite for caregiver.

Provides coordination of services through linkage and referral to supports and services for children 3+ yrs through adults with a diagnosis confirmed by DDRO (Developmental Disabilities Regional Office) to allow them to live as independently and productively as possible.
