Programs include: Adult Day Health Care (ADHC) Centers: safe, supervised environment for veterans to participate in activities and socialize; professionals help with veterans' rehabilitation. Home-Based Primary Care: at-home primary care, nursing, medication management, nutrition, physical rehabilitation, mental health, social work and referrals to VA community services for veterans with medical issues making travel to and from a medical center for routine medical appointments difficult. Skilled Home Care: VA-purchased care, such as nursing services and physical, occupational or speech therapies, in the veteran's home, when travel and disability make traveling to and from appointments difficult. Homemaker and Home Health Aide Services: regularly- schedule help with personal care needs, such as eating and bathing. Home Telehealth: in-home technology to give access to care coordinator if the veteran lives far from a VA Medical Center. Respite Care: periodic relief from daily routines and care responsibilities for caregivers. Veterans requiring caregiving can receive up to 30 days per year of respite care in a variety of settings. Home Hospice Care: provides in-home comfort and support services for veterans and caregiver during advanced stages of a terminal disease. Bereavement care for caregivers and other immediate family members also available.Eligibilty -You are eligible to apply for VA health care if you are a veteran of active military service and were discharged or released from service under honorable conditions, provided that you meet minimum duty requirements.To apply for health care benefits, you will need a copy of your discharge papers and will need to fill out a form called 1010EZ. The form can be downloaded from https://www.1010ez.med.va.gov/sec/vha/1010ez/ or you can call 1-877-222-VETS and ask to have one mailed to you.
