Free program offering a safe space and engaging activities for loved ones to enjoy, giving their family members an opportunity to take a few hours off. Registration is required

Programs include: Adult Day Health Care (ADHC) Centers: safe, supervised environment for veterans to participate in activities and socialize; professionals help with veterans' rehabilitation. Home-Based Primary Care: at-home primary care, nursing, medication management, nutrition, physical rehabilitation, mental health, social work and referrals to VA community services for veterans with medical issues making travel to and from a medical center for routine medical appointments difficult. Skilled Home Care: VA-purchased care, such as nursing services and physical, occupational or speech therapies, in the veteran's home, when travel and disability make traveling to and from appointments difficult. Homemaker and Home Health Aide Services: regularly- schedule help with personal care needs, such as eating and bathing. Home Telehealth: in-home technology to give access to care coordinator if the veteran lives far from a VA Medical Center. Respite Care: periodic relief from daily routines and care responsibilities for caregivers. Veterans requiring caregiving can receive up to 30 days per year of respite care in a variety of settings. Home Hospice Care: provides in-home comfort and support services for veterans and caregiver during advanced stages of a terminal disease. Bereavement care for caregivers and other immediate family members also available.Eligibilty -You are eligible to apply for VA health care if you are a veteran of active military service and were discharged or released from service under honorable conditions, provided that you meet minimum duty requirements.To apply for health care benefits, you will need a copy of your discharge papers and will need to fill out a form called 1010EZ. Offers a complimentary Caregivers Center that is here to support you. The Center is here to give you support, easy access to information, and a place to go rest, recharge, and regain your energy. At no cost to you. You can take a break and visit us; stay for a minute, or an hour; Have a chat; Share your experiences with other caregivers; meet with a Licensed Social Worker or just sit and have a quiet moment for yourself. In addition you can join our complimentary Caregivers Support Groups. All groups facilitated by a Licensed Social Worker and all groups will meet at Peconic Bay Medical Center. They are free to attend and all our welcome. Sample support groups include: Family Caregiver support groups, In-person support group for caregivers of a loved one with dementias and memory impairments; In-person general caregiver support group and bereavement support groups for family caregivers forming. Provides periodic respite breaks for primary caregivers while disabled relative is cared for at Rising Treetops at Oakhurst. Disabled family member lives among peers and and has access to seasonal recreational activities including crafts, athletics, swimming, games and TV. Also offers overnight respite where registered families can reserve time in advance. Day respite is available to residents of Monmouth County.

Provides respite and educational services to developmentally disabled adolescents who exhibit skills deficits and/or severe behavioral problems and are in need of services beyond the normal school day. Also provides individual programming in the areas of behavioral intervention, self-care skills, communication skills, socialization, leisure skills and functional academics.

Offers respite for caregivers who need a temporary break form taking care of a family member with mild to moderate cognitive impairment, dementia, or Alzheimer's. Provides subsidy for child care tuition for eligible families with children that have special needs.

Provides in-home relief of primary care giving responsibilities for the caregiver. The program services developmentally disabled, adults and children with OPWDD eligibility in Nassau and Suffolk counties living in their family home.

Provides adult respite care for developmentally disabled individuals, age 21 and over, who reside in Suffolk/Nassau counties with their families. Limited geographic area may be eligible for transportation periodically. Universal medical application and site visit appointment required.

Provides 12 co-ed beds to youth in need of either a diagnostic evaluation, emergency placement, or a crisis/preventive respite stay. The youth are supervised 24 hours a day in a professionally staffed setting and are engaged in an academic program during the school year. Evaluations provided include psychological, psychiatric, psychosocial, and substance abuse risk assessments as needed.

Offers peer services to Nassau County residents with mental health illnesses. Provides opportunities for participants to discuss recovery, relationships, school and work related issues and to problem solve with certified peers through Phone Link, Mobile Support and peer-run support groups. Provides peer respite temporary housing and support for individuals in crisis.

Offers a weekend respite for campers with physical and developmental disabilities between ages 6-21 years old. Check in is Friday evening at 8pm and check out is Sunday at 3pm. Campers enjoy arts and crafts, music, games, movies, and walks in the woods. The program benefits parents by giving them a break from very demanding responsibilities as their child's caregiver.
