Provides residential respite services for developmentally disabled individuals living at home.

Provides respite care, information and referral, crisis intervention, case management, day program placement, residential placement, aging out services, and advocacy to the developmentally disabled ages 3 and up and their families.

Provides respite and support for families with children who are severely emotionally disturbed. It is designed to reduce family stress, reduce the incidence of hospitalization, and decrease the risk of child abuse. Services include evaluation, respite. family support groups, parenting skills group, tutoring, socialization, recreation, case management and a Parent Advocate.

Provides family with relief from the obligation of ongoing day-to-day care. This service is offered either in the home of the person with a developmental disability or in another environment, depending on the needs of the family.

Offers Nassau and Suffolk County residents with intellectual and/or developmental disabilities and their families the ability to select services and providers that are most suited to their needs. Offers self-directed community habilitation, respite and SEMPT (supported employment). Assisted by a Fiscal Intermediary and Support Broker, individuals and family members learn how to implement HCBS (Home & Community Based) waiver services establish a habilitation plan, assess needs, and budget allocated Medicaid funds.

Provides site based respite services and Supplemental Day Habilitation which includes social and recreational opportunities both in-house and in the community to developmentally disabled adults over the age of 21 who reside in Nassau and Western Suffolk Counties.

Provides Medicaid Service Coordination (MSC), non Medicaid Service Coordination, respite services, Community Habilitation services, Day Habilitation services as well as Individual Support Services (ISS) for individuals who have Developmental Disabilities and/or mental disabilities, and Fiscal Intermediary Services.

Coordinated team of health professionals and volunteers include registered nurses, home health aides, medical social workers and physical, clinical dietician, occupational and speech therapists. Offers a complimentary Caregivers Center that is here to support you. The Center is here to give you support, easy access to information, and a place to go rest, recharge, and regain your energy. At no cost to you. You can take a break and visit us; stay for a minute, or an hour; Have a chat; Share your experiences with other caregivers; meet with a Licensed Social Worker or just sit and have a quiet moment for yourself. In addition you can join our complimentary Caregivers Support Groups. All groups facilitated by a Licensed Social Worker and all groups will meet at Peconic Bay Medical Center. They are free to attend and all our welcome. Sample support groups include: Family Caregiver support groups, In-person support group for caregivers of a loved one with dementias and memory impairments; In-person general caregiver support group and bereavement support groups for family caregivers forming. Provides periodic respite breaks for primary caregivers while disabled relative is cared for at Rising Treetops at Oakhurst. Disabled family member lives among peers and and has access to seasonal recreational activities including crafts, athletics, swimming, games and TV. Also offers overnight respite where registered families can reserve time in advance. Day respite is available to residents of Monmouth County.

Provides respite and educational services to developmentally disabled adolescents who exhibit skills deficits and/or severe behavioral problems and are in need of services beyond the normal school day. Also provides individual programming in the areas of behavioral intervention, self-care skills, communication skills, socialization, leisure skills and functional academics.

Provides a therapeutic Recreation/Respite Program four times yearly-summer, winter, spring and fall to head injured survivors of Long Island.

Offers a weekend respite for campers with physical and developmental disabilities between ages 6-21 years old. Check in is Friday evening at 8pm and check out is Sunday at 3pm. Campers enjoy arts and crafts, music, games, movies, and walks in the woods. The program benefits parents by giving them a break from very demanding responsibilities as their child's caregiver. Provides respite for family members or friends who are caring for someone 55 years or older and a resident of Suffolk County.

This program is about seniors helping seniors. Low-income seniors receive training and a small stipend to provide chore services, friendly visiting, respite to caregivers and technology assistance. Volunteers are background checked, can volunteer 5-40 hours per week, and are matched with seniors in the community who need help.

The goal of family support is to empower parents and caregivers as informed decision-makers information, goal-planning and emotional support for parents/caregivers, parent/caregiver advocacy, service coordination, and family recreation events.in navigating complex child-serving systems. Programs include: Adult Day Health Care (ADHC) Centers: safe, supervised environment for veterans to participate in activities and socialize; professionals help with veterans' rehabilitation. Home-Based Primary Care: at-home primary care, nursing, medication management, nutrition, physical rehabilitation, mental health, social work and referrals to VA community services for veterans with medical issues making travel to and from a medical center for routine medical appointments difficult. Skilled Home Care: VA-purchased care, such as nursing services and physical, occupational or speech therapies, in the veteran's home, when travel and disability make traveling to and from appointments difficult. Homemaker and Home Health Aide Services: regularly- schedule help with personal care needs, such as eating and bathing. Home Telehealth: in-home technology to give access to care coordinator if the veteran lives far from a VA Medical Center. Respite Care: periodic relief from daily routines and care responsibilities for caregivers. Veterans requiring caregiving can receive up to 30 days per year of respite care in a variety of settings. Home Hospice Care: provides in-home comfort and support services for veterans and caregiver during advanced stages of a terminal disease. Bereavement care for caregivers and other immediate family members also available.Eligibilty -You are eligible to apply for VA health care if you are a veteran of active military service and were discharged or released from service under honorable conditions, provided that you meet minimum duty requirements.To apply for health care benefits, you will need a copy of your discharge papers and will need to fill out a form called 1010EZ. Family Support and Home and Community Based Services for individuals with developmental disabilities; Family Support Coordinators help families determine the services most appropriate for their needs.Home and Community Based Services Waiver provides funding for community habilitation, day habilitation and hourly respite. Each participant has an assigned provider to help them become more independent and active in their community.Also, offers sliding fee scale reimbursement for respite services to provide a break for care-givers. Families may choose their own worker or receive a list of qualified individual service providers.Self direction services are also available in Tioga, Tompkins, Cortland, and parts of Broome, Cayuga, Chemung, Onondaga, Schuyler and Seneca Counties
