NICHOLAS CENTER | NICHOLAS CENTER

Offers community, home, school or site-based services to Town of Hempstead residents with autism, intellectual or developmental disabilities, ages 5 to adult. Aims to support participants in living a full life by addressing individuals' challenges while promoting inclusion and participation in the Port Washington community. Offers daily, and after-school programs aimed at addressing participants' educational, behavioral and social needs. Provides supervision, parent and staff training, and consultation for school-aged students, ages 5 and older. Offers two summer sessions featuring a variety of programs focusing on fitness, wellness and sports, independent living skills, communication and social skills, prevocational guidance, work discovery courses and habilitation services (HCBS Waiver Services/OPWDD). Offers a Speakers Bureau. Serves as a Provide agency for the Office for People with Developmental Disabilities ACCES_VR and local school districts.

AMERICAN FOUNDATION FOR THE BLIND | AMERICAN FOUNDATION FOR THE BLIND

Provides the following services to the blind and visually-impaired and to organizations/individuals working with this audience: information and referral, publications on blindness, data collection and information dissemination, and production of recorded materials for the blind through the Talking Books Program. Conducts a wide variety of programs to support independent living, literacy, employment and access to technology. Advocates for legislation and operates a web site which offers access to an on-line book store, Career Connect(employment planning tool), and Braille Bug(children's web channel for literacy).

BROOKHAVEN LEARNING CENTER | BROOKHAVEN LEARNING CENTER

Offers a secondary school that empowers students to develop functional academic, personal, and pre-vocational skills. Our mission is to encourage independence and self-advocacy, and to prepare students to successfully transition from the school environment to post-school options. Skilled staff, in partnership with families, districts and the community, provides quality instruction and services to students. Diverse educational strategies, aligned with the Common Core Learning Standards, offer curriculum accessing real life settings, in conjunction with community-based education and work experiences. Together, these provide unique opportunities to enrich each student's education. The strength of the Brookhaven Learning Center lies in the integration of these dynamics which are designed to maximize opportunities so that each student can achieve his/her fullest potential. The Brookhaven Learning Center provides a continuum of services for students with moderate to severe developmental disabilities. Students participate in a modified New York State Curriculum with an emphasis on life skills. Serves students ages 12-21 in grades 7th-12th at the Middle School and Secondary Level. Classroom options are 8:1:1+4; 8:1:1+2; 8:1:1+3; 8:1:1 and .12:1:1. Students are classified as: Autism; Intellectual Disability; Traumatic Brain Injury; Multiple Disabilities and Other Health Impairment. Also offered is The Transition Services Program which provides a half or full day applied academics program, which emphasizes life and employability skills to students with mild to moderate developmental disabilities. By utilizing a variety of simulated and real-life settings students are prepared to transition from school to post-secondary outcomes. Included in the Transition Services Program is the Work Activities Center (WAC) for students sixteen to twenty-one, who meet established criteria, to have a volunteer work experience in the community under the direct supervision of a job coach. The Brookhaven Learning Center is offered at various locations.

YOUTH SHELTER PROGRAM | HOPE FOR YOUTH

Provides a safe, supportive and secure environment for all Suffolk County youth between the ages of 12 and 20 who find themselves in crisis and need shelter. The program provides year round support services 24/7 for up to 30 days. Offers case management services, individual and family counseling, transportation, food, clothing, educational assistance and advocacy, medical care coordination and referrals, living skills development and other essential services.

Responsible Fatherhood Program - COI

Works with dads--biological fathers, adoptive fathers, father figures, and grandfathers--of children up to 24 years old. Provides life skills training, life coach services, employability skills training, parenting skills training, 24/7 DADS class, and MoneySmart program to benefit dads. Works with dads both in the community and in incarceration. Incentives are provided to complete the training.

Chautauqua Opportunities for Development, Inc. (CODI) - Dunkirk

Focuses on creating asset development opportunities for both residents and entrepreneurs. Assists individuals in growing and investing in their communities by providing lending capital, training, consulting, technical assistance, seminars, workshops, and low-cost business rental space to new and existing businesses that would otherwise lack access to these services. Assists area residents in developing asset based thinking by providing financial literacy training, basic budgeting, and home appliance loans to combat predatory lenders. Loan programs and Cottage Industry Assistance Program target low to moderate income existing and prospective individuals. Chautauqua Microenterprise Loan Fund and the Women and Minority Microenterprise Loan Fund Programs help small businesses by providing small business start up and expansion loans. The small business incubator offers small businesses an opportunity to grow and stabilize with subsidies around rent, business services, and technical assistance. Program site serves as a Warming Center during times of excessive cold and as a Cooling Center during times of excessive heat. Always call ahead to make sure a Warming/Cooling Center is open before going there.

Community Cares Center - Saving Grace Outreach

Provides a food pantry. Baby items available, including baby food, formula, and diapers. Also offers cooking classes, after school programs (which include enrichment activities such as archery and various events), and summer youth programs. Summer program options include a free drop-in day camp, which usually lasts 3 hours per day and provides lunch, and an all-day camp for which there is a daily fee. Offers a free Thanksgiving dinner, with delivery available for homebound people; see Schedule and Intake below for 2025 details.

Offers a school which is designed for students with moderate to severe developmental disabilities. The instructional program, which is aligned to the New York State Standards for Students with Severe Disabilities and the New York State Common Core Learning Standards, builds confidence and capabilities of students and focuses on developing communication skills and personal independence. Research-based methodologies, such as Applied Behavior Analysis (ABA), visual strategies, the Picture Exchange Communication System (PECS), augmentative communications systems, functional behavioral assessment and positive behavioral supports are utilized in this program. The Rosemary Kennedy School consists of the elementary, middle and high school classes as well as district-based classes which provide mainstreaming opportunities for students. Community-based instruction is an essential part of the curriculum and compliments traditional classroom learning. Students participate in recreational activities (libraries, bowling), life skills (grocery shopping, restaurants), volunteer work sites (Mercy Hospital, Hofstra University) and vocational enclaves in the community (fast food restaurants, retail stores). Each student is educated by an interdisciplinary team comprised of the classroom teacher, teacher aides, psychologist, social worker, related service personnel, curriculum teachers and administrators. Parents are encouraged to participate in their child's education through daily communication notebooks, conferences, visits, parent training sessions and parent support groups. Transition planning for students from ages 12 to 21 is facilitated by vocational rehabilitation counselors and social workers in coordination with students and their families prior to graduation. LIFE OPTIONS | CEREBRAL PALSY ASSOCIATION OF NASSAU COUNTY

Life Options is a NYS OPTS, 5 days a week program to help participants develop personal life skills for their leisure time, retirement and self-intiatied activities. Each individual has a life studies advisor who assists in the develpment of their personal program schedule.
