* Coordinates the match between active seniors and seniors who need a little help in their home. The senior workforce can relate with their fellow seniors. Services include respite care, companion care, light housekeeping, meal prep, medication reminders, rides to doctors appointments, shopping, errands, laundry.

Responsible for authorizing personal care services to Medicaid-eligible individuals who require in-home assistance with housekeeping and/or personal care. Services provided include housekeeping chores, errands, laundry, meal preparation, assistance with personal hygiene, dressing, and grooming. Unit is also responsible for administering the Long Term Home Health Care Program, designed to meet the needs of the chronically ill and disabled person in their home.

The Personal Care Assistance program is a home care program for individuals who are Medicaid eligible, medically stable and who require assistance to remain safely at home.

Provides eligible older persons with a variety of non-medical, in-home services that may help them to remain at home. Services include case management, housekeeping/chore assistance, personal care, and respite for caregivers.

* Provides homecare services that include: Medical management, personal care, nursing care, companionship, errands, light housekeeping, safety services, nutrition, body mechanics. * Offers nursing services. Private duty nurses, home health aides. * Offers dementia management. Nursing consultative services for home assessment of needs and safety.

An alternative to "traditional home care/personal care", CDPA allows eligible individuals to hire and direct their own personal care workers (including most family members, a neighbor or friend), to assist with bathing, dressing, eating, and other daily tasks. Eligibility is determined by the appropriate county entity. CDPA believes there's no place like home.

Provides a non-medical home care program for functionally impaired seniors in Rensselaer County. Services include light housekeeping, shopping, bathing, medication management, meal preparation, and other non-medical assistance. The fee is a sliding scale based on the client's monthly income.

Provides personal care, housekeeping/chore services, and case management for the frail elderly.

* Provides individuals the connection to assistance necessary to stay in the home including:

• Non medical personal care
• Housekeeper/chore services
• Case management
• Information and referral services

*Allows individuals the opportunity to choose what services they receive through Consumer Directed EISEP.

*Provides linkage to outside agencies who will perform these services.

Provides personal care services to individuals who have medical needs in the home. Coordinates with the Schoharie County Public Health Department and home care agencies for client assessment and provision of aide services.

Provides assistance to help older adults to remain in their home instead of entering a nursing home or other community residence.

Visiting Nurses Home Care is a fiscal intermediary for the Consumer Directer Program. This Medicaid-funded program empowers Consumers to recruit, hire, train and supervise their own home care worker.

Provides a comprehensive program of in-home services for functionally impaired individuals 60 years of age and older. Services include case management, in-home services (housekeeping and personal care), in-home respite, and ancillary services.

* Provides non-medical help, including: Transportation and escort for errands and appointments. Medication reminder. Overnight care. Companionship and conversation. Safety and supervision. Stand-by assistance. Housekeeping assistance. Meal preparation. Respite care.

* Offers a senior companion program, which includes paid opportunities for low-income older adults to volunteer with frail older adults. Services include: Companionship Transportation to medical appointments Grocery shopping Daily errands Light meal preparation Respite Socialization* Offers a foster grandparents program, which includes paid opportunities for low-income older adults to volunteer with children in area schools, child care centers, and Head Start Programs.

To help individuals remain independent and in the comfort of their own homes, Eddy Visiting Nurse Association (VNA) offers comprehensive in-home healthcare services: home health aides; visiting nurses; specialty nurses, including IV therapy and HIV/AIDS care; physical, occupational and speech therapists; medical social workers; nutritionists; home infusion; home medical equipment and supplies; telehomecare; personal emergency response systems; monitored medication dispensers; palliative care; and wound-ostomy-continence care. Eddy VNA also offers volunteer care to provide non-medical assistance to patients. Our Resident Service Advisor Program helps individuals living in senior apartment complexes find and access the services they need to remain independent in their own homes.
