SOCIAL SERVICES DEPARTMENT, CORTLAND COUNTY - CORTLAND CO COORDINATED CHILDREN'S SERVICES INITIATIVE | SOCIAL SERVICES DEPARTMENT, CORTLAND COUNTY

Helps families stay together and avoid out-of-home placement of children with emotional and behavioral challenges. Family, child, family support representative and representatives from other agencies meet to review family situation, develop and implement service plan and track family's progress during a three- to six-month period.

ACT | ICAN

* Provides the Regular Intensive KO/ACT Program which offers wraparound services to keep families together in situations where children with a mental health diagnosis are at imminent risk of out of home placement. Services are team based and include:
  • Comprehensive strength based treatment plan
  • Behavior Management
  • Mentoring
  • Psychiatric consultations
  • Intensive supervision
  • Special therapy
* Offers the Step Down Program for families that may not be in such imminent risk of out of home placement yet require assistance and support in achieving their goals. Services include:
  • Team based approach
  • Individualized treatment
  • Collaboration with community partners

OSWEGO PREVENTIVE SERVICES | LIBERTY RESOURCES

Works with Oswego County families who have youth at risk for possible placement due to issues related to PINS or JD behaviors. Offers casework services to assist families in identifying issues that place children at risk and developing goals to address those concerns. Provides both home-based and school-based counseling services, crisis intervention, advocacy, and referral to other support services in the community.

* Investigates allegations of child abuse and neglect received by NYS Central Registry of Abuse and Maltreatment. Services may include: Foster care placement for abused or neglected children. Preventive Services for families in crisis to avoid foster care placement. Casework/referral services. Family preservation & support programs. JD & PINS programs. Foster parent recruitment. Foster parent training and certification. Certification of adoptive families for children in foster care. Adult Protective services works with individuals 18 years and older, with physical and/or mental impairments, and no support system to help meet basic needs. On call caseworker for after hours emergencies.

FAMILY CONNECTIONS | BERKSHIRE FARM CENTER AND SERVICES FOR YOUTH

Provides intensive, short-term services for families in which children are at risk of being removed from the home. Workers teach practical skills for successful family life and provide counseling and other support to keep families together.

HOME SAFE - BAY SHORE | FAMILY SERVICE LEAGUE - BAY SHORE FAMILY CENTER

Purpose is to prevent children from being placed in either congregate residential care or foster care. Home Safe works with families that need strengthening and other assistance. Children must be referred through Suffolk County Department of Social Services. Program provides in-home parenting skills and case management services to families who have DSS/CPS involvement. Services are offered weekly for a period of six months.
