Offers a number of programs designed to meet the needs of children with developmental disabilities and their caregivers. Services include: - Respite Services: Offers planned short-term and time-limited opportunities for families and care providers to have some personal time. - Employment Services: Helps adults and youth to obtain and retain competitive employment through vocational counseling, employment training programs, work experience acquisition, soft skills development, rehabilitation services, and support services to help youth obtain, maintain, or advance in competitive employment or self-employment. - Children's Health Homes/Care Management: Includes care coordination, comprehensive transitional care, individual and family support, and referral to community and social support services as needed. - Community Habilitation: Assists youth living at home with a loved one or independently in the community to maintain or develop the highest level of independence possible for them. - Community Based Day Habilitation: Promotes community inclusion and integration. - Family Reimbursement Program: Covers expenses for goods and services needed to care for a loved one with a developmental disability who lives at home. - Self-Directed Services: Enables individuals to make choices about their staff, supports, and services. - Preventive Services: Provides support to help families learn the necessary skills to ensure their children remain at home and out of foster care placement. - Behavioral Health Services: Includes community habilitation, caregiver/family supports and services, pre-vocational services, and supported employment. - Project C.O.A.C.H.: Provides telephone counseling, referral, outreach, and intervention/prevention for individuals and families.

Case / Care Management
Collaborates with community agencies and service providers to support families based on their cultural and individual strengths and needs to provide preventive and foster care services. Provides child welfare program, adoption services, preventive services, and foster care resulting from voluntary or court placement. Preventive services are supports and assistance to help a family learn skills (such as coping skills, parenting skills, budgeting skills, anger management, etc.) that will prevent a child from being abused and therefore removed from the home.

Responsible for providing assistance to families affected by child abuse/neglect or adoption of a Seneca child. Offers parenting classes for foster parents and biological parents. Program strives to prevent out of home placement of Native children; goal of the case plan is to reunify the family. Program provides case management services and referrals to other agencies that will help the family serve the best interests of the child.

Offers Concrete services (i.e.: day care, homemaking, budgeting, advocacy, etc.), casework counseling, and referral services are provided to eligible families in order to avert placements of children outside their homes, enhance family functioning and/or expedite reunification of children in care with their biological families. Eligible families include those at some risk of foster care and pregnant and/or parenting adolescents. Referrals to preventive services can be made by calling 516-227-7789. Also offered is PINS Diversion Program - Services are provided through contract agencies, to families in crisis whose children might be experiencing truancy, gang involvement, running away from home, and substance abuse issues or other unmanageable behaviors in order to prevent/avert the filing of a Pins Petition and placement out of the home. Provides services to avoid placement of children and youth outside their homes. Family- focused case management services. Helps to keep families intact and to avoid problems that may cause children to be at risk. PERSONS IN NEED OF SUPERVISION (PINS) in cooperation with the Dept. of Probation, provides intensive case management services to youth placed into the Persons in Need of Supervision (PINS) Diversion Program.
