Update- this program is closing at the end of 2025 - Support, through building intra-family and community connections, to keep families with youth ages 7 to 18 together in a safe, healthy environment and prevent out-of-home placement.

Supports families by providing services and resources needed to strengthen the family unit, enable families to remain intact, and avoid unnecessary placement of children in foster care.

Helps families stay together and avoid out-of-home placement of children with emotional and behavioral challenges. Family, child, family support representative and representatives from other agencies meet to review family situation, develop and implement service plan and track family's progress during a three- to six-month period.

Assists families in reducing children's risk of foster care placement and provide supports necessary to reunify or develop an alternate permanency plan for children when out of home placement becomes necessary.

Children and Family Treatment and Supportive Services that support youth and their families to live their best life.The services that are offered are the following:- Psychosocial Rehabilitation (PSR)- Community Psychiatric Supports and Treatment (CPST)- Therapy Services with Licensed Practitioner (OLP)- Family Peer Support Services (FPSS)- Planned Respite Services

* Provides the Regular Intensive KO/ACT Program which offers wraparound services to keep families together in situations where children with a mental health diagnosis are at imminent risk of out of home placement. Services are team based and include:

• Comprehensive strength based treatment plan
• Behavior Management
• Mentoring
• Psychiatric consultations
• Intensive supervision
• Special therapy

* Offers the Step Down Program for families that may not be in such imminent risk of out of home placement yet require assistance and support in achieving their goals. Services include:

• Team based approach
• Individualized treatment
• Collaboration with community partners

Provides youth and their families with intensive support in the home, school, and community. Services include case management, crisis intervention services, and skill development.

Provides an intensive, home-based program designed to reduce out-of-home placement for the most at-risk youth and families. Services include counseling, respite services, mentoring, community supervision, recreation, school and community advocacy, psychiatric and psychological services, and linkage to other community support and programs.

Provides referrals to help children with serious emotional disturbances by supporting families connect, and link with community resources, to prevent out-of-home placements by providing case management support to families.

* Investigates abuse, exploitation, neglect (by self or caretakers) for adults and children. * Offers services for families in crisis to avoid the need for foster care placement. Case management. Supportive services. Home management. Parenting skills. Social adjustment.

Engages young adults to work through challenges they face living with a disability, mental health or substance abuse disorder with peers who have struggled with similar experiences.

* Partners with families to increase the safety and permanency of their children to eliminate out of home placement. Case manager provides weekly home visits for a period of up to eighteen months with focus on:

• Housing stability
• Employment support
• Emotional and physical well-being

* Works with family members to provide skill building in areas including:

• Parenting
• Budgeting
• Communication
• Decision making
• Problem solving

* Provides pregnancy counseling services and adoption services for domestic, international and older special needs children

Works with Oswego County families who have youth at risk for possible placement due to issues related to PINS or JD behaviors. Offers casework services to assist families in identifying issues that place children at risk and developing goals to address those concerns. Provides both home-based and school-based counseling services, crisis intervention, advocacy, and referral to other support services in the community.

The Human Services Division provides families and individuals with services that will enhance the ability of families to live together, enable individuals to remain in their homes, minimize the risk of abuse or neglect, and provide for specialized care in residential settings when necessary.

A Family Assessment Response helps ensure that children are safe, while at the same time setting aside the issue of fault and negative labels for parent and caretakers.

A program for low income families, whose children are in placement or at risk of placement outside the family, and offers a multi-team approach of intensive intervention services, including workshops and parent support groups.

Provides two-hour group training sessions to families who have children who meet HCBS waiver eligibility requirements under age 18. Topics vary and are geared toward families who have children with developmental disabilities. Call for more information.

Provides a brief intensive therapy program designed for families with teens who are at risk of out-of-home placement to solve specific issues, and to work together on the strengths and challenges within the family.

* Investigates allegations of child abuse and neglect received by NYS Central Registry of Abuse and Maltreatment. Services may include: Foster care placement for abused or neglected children. Preventive Services for families in crisis to avoid foster care placement. Casework/referral services. Family preservation & support programs. JD & PINS programs. Foster parent recruitment. Foster parent training and certification. Certification of adoptive families for children in foster care. Adult Protective services works with individuals 18 years and older, with physical and/or mental impairments, and no support system to help meet basic needs. On call caseworker for after hours emergencies.

Provides intensive, short-term services for families in which children are at risk of being removed from the home. Workers teach practical skills for successful family life and provide counseling and other support to keep families together.

Purpose is to prevent children from being placed in either congregate residential care or foster care. Home Safe works with families that need strengthening and other assistance. Children must be referred through Suffolk County Department of Social Services. Program provides in-home parenting skills and case management services to families who have DSS/CPS involvement. Services are offered weekly for a period of six months.
