Merakey’s Family Based Mental Health (FBMH) program is for children and adolescents who are at risk of being hospitalized or referred to an out-of-home placement because of mental health issues or family challenges. A team provides in-home therapy for both the family and the young person.

Provides a brief intensive therapy program designed for families with teens who are at risk of out-of-home placement to solve specific issues, and to work together on the strengths and challenges within the family.

Provides two-hour group training sessions to families who have children who meet HCBS waiver eligibility requirements under age 18. Topics vary and are geared toward families who have children with developmental disabilities. Call for more information.

Provides services which can help prevent families who have had or are at risk of having their children placed in foster care. Families receive support for stress related issues, such as case management, daycare services, homemaker services, parenting aides, transportation, recreational services, and more.

A Family Assessment Response helps ensure that children are safe, while at the same time setting aside the issue of fault and negative labels for parent and caretakers.

Works with Oswego County families who have youth at risk for possible placement due to issues related to PINS or JD behaviors. Offers casework services to assist families in identifying issues that place children at risk and developing goals to address those concerns. Provides both home-based and school-based counseling services, crisis intervention, advocacy, and referral to other support services in the community.

Offers a range of intensive mobile services such as individual and family counseling, psychiatric consultations, crisis intervention and prevention, recreation, respite, parent education and training, tutoring, mentoring, case management, and vocational opportunities.

Provides assistance to the low income community and to the community at large in organizing issues which affect the community such as housing, education, employment opportunities, drugs and youth violence. Offers long-term psychological support for at-risk children and their parents who have mental health and parenting issues. Provides dyadic therapy (working with the child and the parent) in cases involving child maltreatment, foster care, and issues involving the criminal justice system. Also do outreach to foster care agencies, family court and other related agencies.

The Intensive in Home Family Preservation Program is designed to provide immediate crisis intervention and stabilization to families with child (ren) at imminent risk of placement into foster care. Provides services to prevent out of home placement for children whose families are referred by Nassau County DSS.. Outreach services include assessment of family concerns, intensive counseling, clinical services including individual and family therapy, referral to other necessary agencies, advocacy, parent education/groups, social/educational activities for the whole family.

Offers Concrete services (i.e.: day care, homemaking, budgeting, advocacy, etc.), casework counseling, and referral services are provided to eligible families in order to avert placements of children outside their homes, enhance family functioning and/or expedite reunification of children in care with their biological families. Eligible families include those at some risk of foster care and pregnant and/or parenting adolescents. Referrals to preventive services can be made by calling 516-227-7789. Also offered is PINS Diversion Program - Services are provided through contract agencies, to families in crisis whose children might be experiencing truancy, gang involvement, running away from home, and substance abuse issues or other unmanageable behaviors in order to prevent/avert the filing of a Pins Petition and placement out of the home. Provides in-home instructional services to families who children are at risk for foster care placement or as part of family reunification planning. Focus is on parenting skills, home and money management, conflict resolution, behavior management and negotiating the social service system.

Provides collaborative support services to families of children with disabilities using a strength-based, family-centered approach to problem solving, and working in coordination with other community providers.

Provides home based parent education and supportive counseling to families who are referred by the Ulster County Department of Social Services. The primary purpose of the program is to prevent child abuse and neglect, and to prevent an out-of-home placement for the children.

Offers inter-agency approach to provide a variety of support services to children at risk of out-of-home placement and their families. Works with families to determine what services are needed in each individualized case.

Aims to prevent and/or reduce the length of stay of youth in psychiatric and residential care. Assists families in identifying and strengthening the protective factors known to be associated with resiliency in their children. Helps to identify and decrease those factors that impede resiliency. Increases a family's skills in order to reduce acute service utilization. Increase a family's ability to access appropriate community resources.

Offers a number of programs designed to meet the needs of children with developmental disabilities and their caregivers. Services include: - Respite Services: Offers planned short-term and time-limited opportunities for families and care providers to have some personal time. - Employment Services: Helps adults and youth to obtain and retain competitive employment through vocational counseling, employment training programs, work experience acquisition, soft skills development, rehabilitation services, and support services to help youth obtain, maintain, or advance in competitive employment or self-employment. - Children's Health Homes/Care Management: Includes care coordination, comprehensive transitional care, individual and family support, and referral to community and social support services as needed. - Community Habilitation: Assists youth living at home with a loved one or independently in the community to maintain or develop the highest level of independence possible for them. - Community Based Day Habilitation: Promotes community inclusion and integration. - Family Reimbursement Program: Covers expenses for goods and services needed to care for a loved one with a developmental disability who lives at home. - Self-Directed Services: Enables individuals to make choices about their staff, supports, and services. - Preventive Services: Provides support to help families learn the necessary skills to ensure their children remain at home and out of foster care placement. - Behavioral Health Services: Includes community habilitation, caregiver/family supports and services, pre-vocational services, and supported employment. - Project C.O.A.C.H.: Provides telephone counseling, referral, outreach, and intervention/prevention for individuals and families.

Responsible for providing assistance to families affected by child abuse/neglect or adoption of a Seneca child. Offers parenting classes for foster parents and biological parents. Program strives to prevent out of home placement of Native children; goal of the case plan is to reunify the family. Program provides case management services and referrals to other agencies that will help the family serve the best interests of the child.

Offers school-based and family-based approaches to preventing out-of-home placement of at-risk youth in all school districts throughout Cattaraugus County. (Families & Schools Together, Truancy Prevention, Family Group Conferencing, etc.) CPS Contract Cases: A program designed to prevent the need for future CPS involvement by engaging families who are experiencing challenges early, providing support to build family resiliency, and helping families develop neighborhood and community-based supports. Referrals are accepted from CPS county-wide. Home Visiting Program: An in-home parenting program that provides parenting skills and structure in a one-on-one setting. Program addresses safety and risk in the home and works on identifying goals within the family. Parenting Workshop: A seven-week parenting class that uses the Active Parenting model. Parenting skills are taught in classroom/group settings. Supervised Visitation: Arranged and set up such that non-custodial parents have access to their children in a safe environment. Supervised visits are utilized as a safety measure to ensure that children are safe, while also allowing parents to build relationships with their children. Parenting skills and support are provided to both the parents and the children during the visits. New Pathways to Responsible Fatherhood Initiative: The focus of the group is to promote fatherhood and the importance of being a positive male role model for children. The group is open to any male who plays a role in a child's life (Father, Step-Father, Grandfather, Uncle, Friend).
