SENIOR ACTIVITIES AND RECREATION CENTER | BRIARCLIFF MANOR RECREATION DEPARTMENT

Provides a variety of fun, recreational, and support services to the older adults in the community. Some services include transportation equipped with a wheelchair lift for shopping, errands and medical appointments, exercise, arts and crafts, and more. A senior advocate is available, by appointment to help with benefits or insurance information, long term care, health issues, volunteer opportunities, help with filling out and understanding application forms, etc.

SENIOR SUPPORT PROGRAMS | POUND RIDGE RECREATION DEPARTMENT

Provides support services to homebound seniors, frail elderly or those with disabilities, from the Town of Pound Ridge, to help make their lives safer. The R-U-OK program offers a daily computerized call to participants, to make sure they are doing okay. If there is not a response a police officer is sent to make sure everything is okay. The File for Life provides a magnetized container which holds medical information, placed on the refrigerator, in case of an emergency and is available to anyone responding to help that person.The Loan Closet provides donated medical equipment, such as wheelchairs, walkers, mobile toilets, canes, etc to seniors with limited finances.The Meals on Wheels program offers home delivered meals on a temporary basis.

SENIOR SUPPORT PROGRAMS | TOWN OF NEW CASTLE RECREATION AND PARKS DEPARTMENT

Provides a Citizen Assistance Registry to place important medical information that can be located easily in case of an emergency. Also provides daily computerized telephone calls to those living alone or homebound in the community, and if the phone is not answered, a police car is dispatched to check on the individual. May also register for this with the police department, a collaborative program.

ARE YOU OK? (R-U-OK) | LEWISBORO PARK AND RECREATION DEPARTMENT

This program offers a daily computerized telephone call to check on frail elderly, homebound and/or those with disabilities to ensure they are safe and well. If there is no answer, and person is known to be home, the police are dispatched to go to the house, and check on the person.

ESKATON TELEPHONE REASSURANCE | ESKATON TELEPHONE REASSURANCE

Brings together older adults and volunteers of all ages to share experiences, stories and laughter through friendly conversation. Beyond building meaningful connections, programs staff provide the following services to participants: daily cost-free calls 365 days a year, safety and wellness checks, access to resources and education, an on-call chaplain for spiritual support, and access to support groups. The program was created for seniors and disabled adults, but everyone is eligible.

The warmline is a 24/7 phone line for individuals with a mental health challenge to speak to a certified peer who can listen, provide some guidance or referrals, and offer some support to the caller.

EXPANDED IN HOME SERVICES FOR THE ELDERLY (EISEP) | SARATOGA COUNTY OFFICE FOR THE AGING

Provides assistance to help older adults ages 60 and older who are not Medicaid eligible and their families delay or avert costly and premature placement in nursing facilities. Services include case management services and non-medical supportive services which can include aide service for light housekeeping and personal care as well as respite. A consumer directly model, where the care receiver hires someone they know, is also offered.

SUPPORT LINE | RESPONSE CRISIS CENTER

Provides telephone calls to community members who are homebound or physically isolated who are suffering from psychological or emotional problems or who require a support system. Persons called can be self-referred or referred by physician, psychiatrist, mental health center, case manager, health care provider, or hotline counselor. Also provides therapist referrals and a referral service.

SENIOR WELLNESS PROGRAM | WHITE PLAINS HOSPITAL

This program provides an evaluation team to assess health and social needs of senior citizens and then recommendations are made for obtaining necessary services. Coordination of services with a physician is made if applicable, along with community service resources. Includes Mall Walkers exercise program at the Galleria, medication awareness program, help in accessing community information and help with filing insurance claims. Also offers caregivers support groups, emergency response devices and telephone reassurance.

ARE YOU OKAY? PROGRAM | PUTNAM COUNTY SHERIFF'S DEPARTMENT

A computerized telephone system calls each enrolled subscriber every day, when the phone is answered the computer will ask if they are okay. If the subscriber fails to answer, there is a constant busy signal, or no pick-up after several tries, a patrol car will be sent to the residence to check on the person's welfare.

Senior Safety - Chautauqua County Office for Aging Services

Also known as the Are You OK Program, this is a collaborative effort between the Office for the Aging and the Chautauqua County Sheriff's Department in which the Sheriff's Department maintains a computerized system to contact elderly or disabled people at prearranged times. When there is no response, the Sheriff's Department notifies a designated contact person to check on the individual. Clients can arrange to cancel the call on a particular day in the event that they won't be home or the contact person won't be available.
