Program assists in locating registered people who are memory impaired and reuniting them with their families and caregivers. All pertinent information is stored in a central registry. In addition, registered members receive an identification bracelet, including an identification code and a 24-hour toll free telephone number, iron-on clothing labels, and two identification cards one for the caregiver and one for the member.
Matches volunteers with homebound seniors for weekly supportive phone calls. Telephone calls on a regular basis to ensure seniors their good health and safety, and to reassure them that help is available if and when they need it.

Provides home checks for local residents on vacation or when homes will be left unattended, assist with doors and/or windows which may have been left open or unlocked. Residents must provide all home information (who is allowed to enter the house, pick-up mail, newspaper, etc, and contact numbers). Also will check the home if reported that some one is has not answered phone calls, mail or newspapers are piling up, etc.
Provides information and assistance on community resources and services to older town residents and their families regarding issues of aging, such as flu shots, HEAP, housing, home care, etc. Printed materials are available for free, along with counseling and help to navigate the various programs and applications that are offered.

A computerized telephone system calls each enrolled subscriber every day, when the phone is answered the computer will ask if they are okay. If the subscriber fails to answer, there is a constant busy signal, or no pick-up after several tries, a patrol car will be sent to the residence to check on the person's welfare.

Provides daily phone calls by senior volunteers to homebound seniors who would benefit from the socialization and the monitoring of their well being.

Partners with community agencies to identify and help vulnerable seniors. Gatekeepers are trained individuals who come in contact with older adults in the course of their day through daily tasks or activities or their job, and they help to recognize, identify and refer older adults who may be or at risk of depression, anxiety and other related symptoms.
The First Responders Alert Registry is a unique plan to assist Town of Yorktown residents with special needs in emergency situations. The goal of this program is to reduce emotional trauma, fear and conflict when first responders have contact with special needs residents by providing responders with essential information before arriving. The information collected here is confidential and will NOT be available to the public. The information will be held securely by the Yorktown Police Department and only used for emergency response and planning. Residents who want to sign-up for the First Responders Resident Alert Registry can fill out the form linked to https://www.yorktownpd.com/first-responders-resident-alert-registry.html. Residents can also download the department's registration form and submit in person at the Yorktown Police Department Main Desk or by mail.

Provides a variety of fun, recreational, and support services to the older adults in the community. Some services include transportation equipped with a wheelchair lift for shopping, errands and medical appointments, exercise, arts and crafts, and more. Cell phones that have been donated by the community are programmed for 911 only service. These phones are then distributed to any Ulster County resident age 60 and over that requests one.EFFECTIVE 3/31/2008 ANALOG CELLPHONES NO LONGER HAVE SERVICE. PLEASE CONTACT THE OFFICE FOR THE AGING TO REPLACE THE ANALOG CELLPHONE FROM THE OFA WITH A DIGITAL CELLPHONE.
