Care Managers meet with residents to assess their needs, offer services so that they may remain independent in their own homes as long as possible and then follow up. Elderbus provides transportation 5 days each week so that seniors can complete essential errands like grocery shopping, have their hair done or get to a local doctor appointment. Transportation outside of Fairport and Perinton is also available.

Facilitates a smooth transition from the hospital to home for Nassau County children under 18 years of age with mental illness. Provides crisis assessment, individualized planning and family needs assessments. Provides patients with care following an acute care hospitalization, when the patient needs more time to recover from a surgery, illness or injury. Includes a private room, skilled nursing care, physical therapy, occupational therapy, speech therapy, care management, respiratory therapy, nutritional counseling, activities and compassionate care.

* Guides individuals through the steps involved in finding eligible services with OPWDD, identify the needs and help work on planning getting those services.

Provides mental health assessment, ongoing counseling, psychiatric evaluation and medication therapy, mental health and substance use disorder screening, crisis intervention, pre-release treatment planning, post-release referral and case management, and advocacy for incarcerated or recently released individuals presenting with mental health problems, in order to improve adjustments to and functioning within the community, increase completion of linkage with outpatient mental health services, and reduce the relapse into criminal behavior.
