Jurisdiction over health, safety and permanency of children of substance abusing parents involved in neglect proceedings, providing: judicial oversight comprehensive services close collaboration between Family Treatment Court team members rapid assessment and referral to services and ongoing monitoring of the family.

Helps individuals transition out of a nursing home setting and return to their home and helps prevent placement into a nursing home.

Provides patients with care following an acute care hospitalization, when the patient needs more time to recover from a surgery, illness or injury. Includes a private room, skilled nursing care, physical therapy, occupational therapy, speech therapy, care management, respiratory therapy, nutritional counseling, activities and compassionate care.

Offers an outpatient treatment facility for adults and adolescents that want to overcome substance use disorders including chemical use and dependency, as well as internet addiction and gambling addiction. Provides evaluations/assessments, group therapy, individual counseling, case management, consultations, toxicology screenings, community referrals, follow-up care/transitional services, family counseling, and relapse prevention groups. Medication Assisted Therapy (MAT) is also available for alcohol, opiate (synthetic or heroin), and/or nicotine dependence.

Provides help with bail expediting, court advocacy, and court ordered restrictions including home monitoring and supervision. Offers information and referral regarding the legal system and services that may be available to help individuals stay out of the justice system.
