Provides persons with disabilities and senior citizens an opportunity to live in the community by offering services such as re-teaching basic skills, environmental modifications to provide accessibility and safety to homes, community transitional services and intensive service coordination.

* Provides fitness classes, recreation programs, wellness amenities, special events for all ages. This includes: Daily nutrition program for seniors 60 years and older. Wellness seminars and social activities for older adults.* Offers senior care services including transportation coordination to medical appointments, case management, support services and arranging with Medical Motors to bring seniors to The Center for daily activities and lunch.

* Coordinates care and communication among providers and offers support during transitions of care. Services include: Health promotion. Patient and family support. Referrals to community and social support services.

* Serves as the entry point to Golisano Autism Center services.* Provides assessment of potential service needs by a family navigator.* Connects families to the appropriate resources both within the Golisano Autism Center family of providers as well as the greater Rochester area.

Provides information, referral, and short-term case management to individuals diagnosed with epilepsy or a seizure disorder.

* Assists individuals in accessing the following: Health insurance. Medical transportation. Financial assistance for medical visits. Medical copays. Medicaid services. Mental health services.

* Offers support, information and referral, and education for developmental disabilities, traumatic brain injury, and epilepsy. Case management. Residential Homes. Self-Directed Services, including Fiscal Intermediary and Brokerage services. Family reimbursement. Respite. Health Homes. Family support services, including parent support groups. Structured day and evening programs. Support groups, counseling. Community habilitation. Day Habilitation. Camp EAGR - Residential summer camp for children with epilepsy.

* Provides immediate emergency assistance to households in crisis including: Food bags. Personal Hygiene Bag. Prescription Assistance. Bus Passes (when available). Stranded Traveler Assistance. Clothing Assistance. Diaper Assistance. Vouchers to area supermarkets for food and personal items. Advocacy and referrals to outside agencies. Crisis assessment and management. Bilingual Services. Registration for Salvation Army Programs (Project Bundle Up, Long Point Camp, Christmas Assistance, etc.)

* Provides emergency services that include: Diapers-sizes vary each day. Hygiene and personal care items. Holiday program, including toys, for registered clients only.NOTE: items available monthly while supplies last. Call ahead to confirm.* Provides short term case management for families in economic crisis.* Assess and makes referrals for a variety of non-mental health crisis situations on a case by case basis.

* Provides the following services for adults and children with multiple chronic health conditions, HIV/AIDS or a persistent mental health condition. Care Management Transitional care Patient and family support Referral to community services

* Assists families of individuals with Alzheimer's disease and other forms of dementia with care management/caregiver consultation, as well as referrals and navigation to health care coordination, education & training, long-term care services, planning, respite services, support groups and health & wellness programming.

* Provides support and services for veterans:Outreach and education to providers and individuals throughout New York State.VA benefit eligibility evaluation.Legal advice and counsel/or representation regarding discharge status upgrades especially regarding discharges related to sexual orientation.Case management assistance.programs for older LGBTQ+ veterans."Know Your Rights" materials for LGBTQ+ veterans.Support group for older LGBTQ+ veterans.Restoring Equal Rights to LGBTQ+ veterans.

* Offers casework assistance including emergency financial resources, information on state and federal survivor benefits, connections to private support for survivors, and information on educational resources.

* Reduces the prevalence of high lead levels in children.* Provides community education and tracking.* Offers lead testing to clients who don't have a physician or medical insurance.* Offers case management for children identified with lead poisoning.

* Provides linkage and community support for individuals seeking inpatient, outpatient, partial hospitalization, and intensive outpatient services for eating disorders treatment. Provided by Office of Mental Health grant funded program.* Offers Care management, peer mentoring services, life coaching, and creative arts therapy.

Provides Service Coordination, Community Integration Counseling, Positive Behavioral Interventions and Supports, and Independent Living Skills Training as an alternative to institutionalization.
