Provides assistance to individuals with developmental disabilities in gaining access to the necessary support and aid needed to live productive lives. Uses person-centered planning focusing on the needs and desires of the individual receiving services while striving for the highest quality of life. Develops an Individualized Service Plan (ISP) which outlines what goals the individual hopes to accomplish, the strategies to achieve these goals, and what support they will receive along the way. Plans are evaluated and updated as the individual progresses and to ensure all support and services are meeting the individual's needs. Also assists individuals with services such as future planning, residential planning, recreation, residential habilitation, day habilitation, reimbursement, adaptive equipment, and environmental modifications.

Provides information to individuals who are at risk or living with chronic illness or HIV/AIDS. Also offers care coordination nutrition education and a food pantry with healthy disease-specific foods .

Assists veterans with long-term care needs to stay in their own homes. Design to help qualifying Veterans to hire, train, and supervise care attendants. Personal Care Attendants help with the Veteran’s daily needs of care, provide caregiver respite, and perform light housekeeping.

Services include: Individual mental health counseling. Group therapy. Support therapies (Art). Case management. Specialized groups to support reintegration.

* Coordinates services related to medical, mental health, substance use, and social needs. Works closely with community providers, e.g., schools and mentor programs, to promote success and independence in children and family stability.

Also helps families with children who have developmental disabilities transition from children's services through adult services.

Community Supports and Day Services are designed to provide enriching opportunities for people with developmental disabilities to enhance their social skills and their independence. Programs are responsive to new and changing interests of people receiving supports and their families. The menu of available supports focus on social skills, daily living skills, wellness, technology, music, and the arts. Participants are fully included in their communities through recreation and experiential learning.

* Offers health home care management for adults living with chronic conditions, including: Mental illness. Substance use. Asthma. COPD. Diabetes. Heart disease. High blood pressure. Obesity.

Aids individuals with intellectual and developmental disabilities in making choices and finding individualized services. They provide help in navigating through available services including assistance with eligibility determination, service coordination, and advocacy services. Service coordination is also available to children ages birth to 3 years old who have been diagnosed with a developmental disability or delay.

Provides supports and services for people with developmental disabilities under contract with the NYS Office for People with Developmental Disabilities (OPWDD), including Service Access, Brokerage, Self-Directed Services, Community Habilitation, and Family Support Services. Family Support Services include monthly youth groups, social skills classes, and summer day camps.

Provides care management to individuals with a serious mental illness, HIV/AIDS, or two or more chronic health conditions and significant behavioral, medical, and/or social risk factors, and who are in need of assistance linking to services to stabilize and improve their overall health functioning. Program services include outreach and engagement, comprehensive care management with assessment and care planning, care coordination and health promotion, comprehensive transitional care, client and family support, referral to community and social support services, and use of health information technology to link to services.

Transportation to and from recreation events. Children's day camp, games, sports, supervised exercise. After school wellness program - for school-age children who have high medical or behavioral challenges. Financial Reimbursement for families to assist with costs of caring for a family member with a developmental disability. Behavior Management Services - providers will assess behavior, make referrals and develop a support plan to assist families to manage challenging behaviors in the home.

* Provides coordination between all types of care a participant may need: behavioral health, medical and social services.

Office for the Aging Continuing Care provides access to anyone, regardless of age and payment source, to long term care services. Case Managers and Public Health Nurses can assess the clients in their home, the nursing home or in the hospital, establish a realistic plan of care, and provide advice on available funding sources. They may also make suggestions regarding other alternatives such as Adult Day Care, Adult Homes, Assisted Living, Foster Care, Home Health Care, Respite Services, and Nursing Home placement. Office for the Aging / Continuing Care can help arrange for the provision of services, which may include in-home care such as homemaking and personal care, help with paperwork, etc.
