* Provides access to support programs available through the New York State Department of Health including: Children's Health Home: care management addressing chronic medical, mental health, substance abuse, behavioral health, and developmental needs. Consumer Directed Personal Assistance (CDPAP): a Medicaid service that allows people with chronic conditions with or without developmental disabilities who need aid or nursing care to hire and train attendants to provide care.

* Ensures all involved in an individual's care are working together and sharing the necessary information to support recovery: medical, mental health, substance abuse services, and social service needs.

* Provides comprehensive, long term case management and counseling services to young parents and their children. Services include: Home visiting Parenting education Employment services Education support Peer support groups

* Offers supportive services for individuals and families affected by HIV/AIDS. Case management. Housing subsidies. Short term emergency assistance. Transportation to medical appointments. Employment services. Peer Support.* Step Up to Employment - assistance to help individuals with HIV gain employment with health care benefits.

* Offers individualized services to persons who are disabled such as: Homemaker basic skills training for caregiver. Parent support group. Companionship and respite services. Family crisis program. Transportation to and from recreation events. Children's day camp, games, sports, supervised exercise. After school wellness program - for school-age children who have high medical or behavioral challenges. Financial Reimbursement for families to assist with costs of caring for a family member with a developmental disability. Behavior Management Services - providers will assess behavior, make referrals and develop a support plan to assist families to manage challenging behaviors in the home.

* Provides service coordination for youth, and their families, who have behavioral and/or emotional issues.

* Assists participants to maintain their benefits, access necessary in-home care, attend medical appointments to avoid unnecessary emergency room visits, and feel supported to work on their individual goals.

* Offers Non Medicaid Care Management services, Assertive Community Treatment Services and Mental Health Residential Services.

* Provides care coordination services to children and adults with intellectual and developmental disabilities. Care coordination combines developmental disability services and supports with health, wellness, behavioral health and family services.

* Provides support to people with disabilities and their families in Seneca and Cayuga counties. Benefit management assistance. Legal and educational advocacy. Crisis assistance. Individual habilitation services. Family reimbursement. Parent support. Guardianship information. Respite assistance. Summer camp and recreation for adults and children. Transportation to doctor's appointments and planned shopping can be provided.

* Provides long term case management to economically challenged families enrolled with transitional housing.* Offers home visits that provide: Needs assessment, goal planning, referral to other services.* Offers conflict resolution. * Provides transportation to services if no other means available.

* Provides residential services for adults with Intellectual disabilities and community residential habilitation.* Provides Medicaid service coordination.* Provides Traumatic Brain Injury service coordination.* Offers Asperger family support services.* Provides guardianship program.* Provides volunteer services to not-for-profit agencies and other area organizations by CDS clients.

* Plans, develops and administers of community based services for older persons in Cayuga County.* Provides information and assistance for senior programs and services such as: Elder In-Home Services Program (EISEP) with case management Caregiver and Alzheimer's Community Service Program Health Insurance Information Counseling and Assistance Program (HIICAP) Senior Nutrition Program for the Elderly provides home delivered meals and Congregate site meals Benefits application assistance in accessing programs and services Limited Legal services

* Aims to reduce the incidence of shootings, killings, and other acts of violence using a public health approach. Identifies and engages high-risk youth in case management Conducts youth outreach Prepares public education campaigns Mediates conflict Provide referrals to local agencies Involves faith leaders Have presence in the streets that neighborhood shootings occur

* Provides case management services to individuals, connects Medicaid HARP-enrolled individuals with employment, financial, vocational, educational, and more. Services are person-centered and are provided in a client's home.* Provides PEER Empowerment services.

* Connects individuals with substance use issues to various services in the community including;Housing assistance.Transportation assistance.Benefits Advocacy.Legal assistance.Motivational coaching.Support linking to primary health care, mental health, and vocational services.

* Assists with housing, employment and other coordinated care services for individuals with multiple health-related needs.

* Authorization of services to individuals with traumatic brain injury or stroke. Waiver allows payment and budgeting for services. Participant may be elegible for rent subsidies and husing supports and limited one-time payment for furtniture and household supplies.* Management of Medicaid insurance money.* Linkage to services.

* Provides an outpatient community mental health clinic treatment services for children and youth that includes: Intake assessment & evaluation Case management, supportive and intensive Crisis services Individual, group, and family therapies Skill-building interventions for symptom management Referrals as indicated Satellite clinics in all Wayne County school districts Child Psychiatric Services
