Community Supports and Day Services are designed to provide enriching opportunities for people with developmental disabilities to enhance their social skills and their independence. Programs are responsive to new and changing interests of people receiving supports and their families. The menu of available supports focus on social skills, daily living skills, wellness, technology, music, and the arts. Participants are fully included in their communities through recreation and experiential learning.

Coordinates care for Medicaid recipients through effective communication with other providers, advocate on your behalf so you receive the care deserved. Helps navigate the complex healthcare system, link to resources for finding affordable housing, food, clothing, childcare and transportation. Office for the Aging Continuing Care provides access to anyone, regardless of age and payment source, to long term care services. Case Managers and Public Health Nurses can assess the clients in their home, the nursing home or in the hospital, establish a realistic plan of care, and provide advice on available funding sources. They may also make suggestions regarding other alternatives such as Adult Day Care, Adult Homes, Assisted Living, Foster Care, Home Health Care, Respite Services, and Nursing Home placement. Office for the Aging / Continuing Care can help arrange for the provision of services, which may include in-home care such as homemaking and personal care, help with paperwork, etc.

Provides assistance to individuals with developmental disabilities in gaining access to the necessary support and aid needed to live productive lives. Uses person-centered planning focusing on the needs and desires of the individual receiving services while striving for the highest quality of life. Develops an Individualized Service Plan (ISP) which outlines what goals the individual hopes to accomplish, the strategies to achieve these goals, and what support they will receive along the way. Plans are evaluated and updated as the individual progresses and to ensure all support and services are meeting the individual's needs. Also assists individuals with services such as future planning, residential planning, recreation, residential habilitation, day habilitation, reimbursement, adaptive equipment, and environmental modifications.

Assists families who are caring for a relative with a developmental disability at home by aiding the caregiver and thereby enhancing family stability. The goals of the program are to maintain family unity, prevent premature or inappropriate out-of-home placement, enhance parenting skills, maximize the potential of the family member with a developmental disability, and provide quality of life.

The Office for the Aging offers many services for the senior residents of Steuben County including, but not limited to, information and referral, nutrition through congregate meals, advocacy, support groups, transportation services, caregiver services, case management, and the Bone Builders and Senior Meal Programs. Bone Builders and the Senior Dinner Clubs are overseen by Pro Action of Steuben-Yates.

Aids individuals with intellectual and developmental disabilities in making choices and finding individualized services. They provide help in navigating through available services including assistance with eligibility determination, service coordination, and advocacy services. Service coordination is also available to children ages birth to 3 years old who have been diagnosed with a developmental disability or delay.

Offers Greenport seniors, ages sixty and older, assistance in accessing community services. Provides case care management, health care coordination, and personal development through individual and group services. Offers opportunities for socialization, education, recreation, volunteering, networking and participation in a program advisory board. Provides assistance with transportation to and from appointments, social/recreational activities and shopping.

* Provides service planning and assistance in securing access to services to help individuals in their recovery.* Assists adults, young adults, and youth that are experiencing chronic medical, mental health and/or substance use disorders conditions with care coordination and related community-based services.
