* Assists participants to maintain their benefits, access necessary in-home care, attend medical appointments to avoid unnecessary emergency room visits, and feel supported to work on their individual goals.

Services for: individuals 18 and older with physical or mental disabilities or at risk of abuse or neglect long term care needs

Offers free, confidential HIV antibody testing or other screening procedures. Also provides case management services to those that have tested positive for HIV.

Aids individuals with intellectual and developmental disabilities in making choices and finding individualized services. They provide help in navigating through available services including assistance with eligibility determination, service coordination, and advocacy services. Service coordination is also available to children ages birth to 3 years old who have been diagnosed with a developmental disability or delay.

* Offers nursing support, advocacy and assistance coordinating doctor appointments, and help with understanding medical care plans and documents.* Accompanies patients to medical appointments, coordinates transportation for appointments and medical treatments.* Provides assistance with filling prescriptions and ensure linkages are made with other supportive community-based services.

Consumer Directed Personal Assistance (CDPAP): a Medicaid service that allows people with chronic conditions with or without developmental disabilities who need aid or nursing care to hire and train attendants to provide care.

* Offers vision rehabilitation services for individuals with vision that includes: Low Vision Evaluations - specialized lenses, visual aids, magnifiers. InSight Shop - full service non-optical store, offering adaptive devices, visual aids, talking watches & clocks, cooking aids, large print items, frames, magnifiers, lighting, and adapted toys for sale.
