* Provides home care services and transitional services to individuals with a variety of needs. The level of care provided depends on the need, which is outlined below: Home Health Aide (HHA) - Provides assistance with feeding, dressing and bathing. HHA's monitor temperature, pulse rate, respiration rate, and blood pressure. Personal Care Aide (PCA) - Provides health care tasks, activities of daily living including personal hygiene services, housekeeping tasks essential to the patient's health and other related supportive services. Companion/Homemaker - A program that provides assistance with house cleaning, laundry, meal preparation, reading, conversation, grocery shopping, prescription pick-ups and errands. Private Pay Care - Private Pay Care encompasses any of the above services. Contact office for private pay rates if not contracted with your insurance company. Skilled Nursing - Provides skilled nursing services such as medication management, diabetic management, wound changes, catheter care, injections. Vary depending on agency availability.
