Medical Adult Day Health Care Program that services people who are visually impaired and medically compromised. Meals provided. Transportation is arranged. Provides case management, both medical and social for participants. Clinical services available to participants may include: audiology (referral basis), referral to dentist and podiatry, RNs on staff, and registered dietician. Occupational therapy, physical therapy, and speech language pathology are provided. Social worker on staff. Vision rehabilitation and training provided. Teaches skills for daily living for people who are visually impaired, such as adaptive handling of money, Braille, food preparation (peeling, slicing), mobility training, and safe use of stairs. Recreation therapy provided; includes crafting, outings, discussion groups, exercise, special events and entertainment, religious activities, and cooking groups.

Provides comprehensive outpatient health and rehabilitation care, as well as a supportive group environment with recreational activities. Health services provided include nursing care, personal care services, audiology, speech therapy, health monitoring, nutritional services, occupational therapy, physical therapy, case management, and health care monitoring of blood pressure, blood sugar levels, medication, and other health regimes. The following services may be arranged as needed: laboratory services, radiology, dentistry, ophthalmology, podiatry, and beauty/barber shop visits. Offers services to individuals with Traumatic Brain Injury, Multiple Sclerosis, Strokes, Parkinson's Disease, Diabetes, Hypertension, Intellectual Disabilities, and Behavioral Health issues.

Offers an adult day program, six days a week, for older adults who need supervision during the day, which includes medical treatment, nursing, therapeutic recreational activities, transportation, meals and other support services.

Provides 24 hour nursing care and a complete program of appropriate care to meet the total needs of each individual resident. Emphasis is also placed on resident's psychological, emotional and spiritual needs.

Offers a full service adult day program for cognitively impaired and physically frail with a variety of recreational and therapeutic activities, including special attention to nutrition. Also offers short-term respite care for caregivers, and an Alzheimer's caregiver support group.

A center specializing in Alzheimer's and related dementia day care. Provides social model adult day care, socialization, hot lunch, breakfast and afternoon snack. Services include small group crafts, exercises, field trips, personal care in activities of daily life, ongoing care plans, information and referral, transportation within a 15 mile range, and wheelchair transportation. Inter-generational program with on-site child care offered. Alzheimer's support group held the 3rd Thursday of each month, 7pm-9pm.

A unique social day program that meets the needs of those who have developed dementia or memory loss caused by Alzheimer's or Parkinson's disease, stroke, heart disease, or any other illness.
