Eddy DayBreak is an innovative adult day service that helps improve quality of life for seniors who still live independently in the community, or who are cared for by family members. DayBreak programs offer a full range of health and social services to help older adults with physical, emotional, or mental impairments, or who require supervision and/or medical care. Services enable older adults to remain independent for as long as possible, while allowing caregivers relief from the responsibilities of at-home caregiving so they can fulfill work or other obligations.

Provides personal care services including health and medical care, case management, health assessment, physical rehabilitation therapy, occupational therapy, speech therapy, and nutritional education as well as activity programs throughout the day.

Provides supervision, socialization and stimulation to individuals 60 years and older who are physically frail or cognitively impaired, and/or in need of some assistance with activities of daily living along respite for their overwhelmed caregiver. Assists individuals who would like to continue to live in the community even if they are experiencing some physical and/or cognitive decline, for those who want to spend time with others, and for those who might otherwise be isolated or unsafe.

* Offers a Program of All-Inclusive Care for the Elderly (PACE), which works with seniors and their caregivers to arrange for and monitor all of their medical, social and daily living support services. Offers a skilled nursing facility for individuals who need continuous care due to age, illness, recovery or disability. Also has an adult day program for individuals who need supervision for a period of time during the day.

Medical model day programs for older adults. Medical management, physical, occupational and speech therapy inpatient and outpatient. Also offers hot meals, and kosher, therapeutic recreation, respite care, personal care, and care management.

Offers an adult day program, six days a week, for older adults who need supervision during the day, which includes medical treatment, nursing, therapeutic recreational activities, transportation, meals and other support services.

Provides medical, rehabilitation, nursing and socialization services for adults who live at home. Community physicians, with the help of the staff, provide chronic disease management, wound care, therapy, recreation therapy, and diet/weight management programs one to five days each week. Light breakfast, hot lunch and snacks are provided. Transportation is available.

Adult daycare-medical model services.

Case management, evaluation of needs and service coordination for elderly people designed to assist caregivers in keeping frail elderly people living in their homes. Home visits are also available.

Provides adult day care for functionally impaired adults 40 years and over. Offers a full time nurse and nursing services, specialized medical services, information and referral, social work, recreational activities, counseling, rehabilitation, nutritional evaluation, hot meals, and personal care. Publishes brochure, calendar of events, educational material.

Provides both a medical model adult day program with speech, occupational and physical therapies, recreation and counseling, and a social model day program with socialization, nutrition and supportive services, including nurse supervision and transportation for both programs.

Offers a comprehensive adult social day program for individuals who may require supervision during the day; assistance with self-sufficiency is given. Assist caregivers with respite care.

Provides a 7-day a week adult day program which includes nursing care, physical, occupational and speech therapy, social work, recreational activities, outings, meals, nutrition education, and wheelchair accessible transportation.

Provides an alternative to institutionalization for the frail elderly, and adults with chronic illness or disabilities. Services include nursing care, occupational and physical therapies, recreation, hot lunches and door-to-door transportation.

A center specializing in Alzheimer's and related dementia day care. Provides social model adult day care, socialization, hot lunch, breakfast and afternoon snack. Services include small group crafts, exercises, field trips, personal care in activities of daily life, ongoing care plans, information and referral, transportation within a 15 mile range, and wheelchair transportation. Inter-generational program with on-site child care offered. Alzheimer's support group held the 3rd Thursday of each month, 7pm-9pm.

A unique social day program that meets the needs of those who have developed dementia or memory loss caused by Alzheimer's or Parkinson's disease, stroke, heart disease, or any other illness.
