Marjorie Doyle Rockwell Center and Eddy Alzheimer's Center at Hawthorne Ridge are the only medical model adult day services in the Capital Region designed specifically for older adults with memory loss who are still living in their own homes or in the community with caregivers. Adult Day Programs offer a full range of health and social services to help older adults with physical, emotional, or mental impairments, or who require supervision and/or medical care. Services enable older adults to remain independent for as long as possible, while allowing caregivers relief from the responsibilities of at-home care giving so they can fulfill work or other obligations. Transportation is provided in geographic area. Lunch and snacks are also provided for participants.

Provides an adult day program for those who need some supervision during the day due to age, illness or disability. Services include nursing care, rehabilitation therapy, social recreation, transportation and two meals.

Eddy DayBreak is an innovative adult day service that helps improve quality of life for adults who still live independently, or are being cared for by family members. Offering supervision and/or assistance with personal and/or medical care, Eddy DayBreak programs provide caregivers relief from at-home caregiving, allowing them to fulfill work or other obligations. Round-trip transportation is available and participants may attend up to five days a week. Eddy DayBreak programs include medical, social and dementia-specific models.

Offers an adult day program, six days a week, for older adults who need supervision during the day, which includes medical treatment, nursing, therapeutic recreational activities, transportation, meals and other support services.

Caregiver Resource Center has been developed to offer support, information, and services to those who care for seniors and to seniors who care for family members who are under the age of 18.

Provides primary care and emergency services for veterans. Inpatient and outpatient treatment includes evaluation, treatment, medical, surgical, psychiatry, neurology and rehabilitation medicine. A full range of health screenings, health education resources, and prevention programs (weight, smoking cessation) are available. Services for women veterans include GYN services, PAP smears, mammograms, and programs for domestic violence and sexual trauma. Provides health care supervision and socialization for any adult, frail or disabled, primarily ages 50-90+.

Offers an adult day health care program for individuals with a diagnosed health condition and a functional vision problem, to help maintain as much independence as possible. Services include nursing supervision, social work services, health education, occupational and physical therapy, vision rehabilitation, diabetes self-management education, nutritional counseling and therapeutic activities, including breakfast, lunch and transportation.

Offers a skilled nursing facility for individuals who need continuous care due to age, illness, recovery or disability. Also has an adult day program for individuals who need supervision for a period of time during the day.
