Eddy DayBreak is an innovative adult day service that helps improve quality of life for seniors who still live independently in the community, or who are cared for by family members. DayBreak programs offer a full range of health and social services to help older adults with physical, emotional, or mental impairments, or who require supervision and/or medical care. Services enable older adults to remain independent for as long as possible, while allowing caregivers relief from the responsibilities of at-home caregiving so they can fulfill work or other obligations.

Offering supervision and/or assistance with personal and/or medical care, Eddy DayBreak programs provide caregivers relief from at-home caregiving, allowing them to fulfill work or other obligations. Round-trip transportation is available and participants may attend up to five days a week. Eddy DayBreak programs include medical, social and dementia-specific models.

Provides medical services to adults in a community day setting. Services include medication administration/treatments by LPN/RNs, case management, physical therapy, occupational therapy, and speech therapy, ongoing daily activities, and lunch and nutritional services overseen by a registered dietitian.

The Wilkinson Adult Day Health Center is a community-based long term care program that provides comprehensive health care services in a congregate day setting. Clients are provided with the medical and social services they need to remain home and part of the community. St. Mary's Adult Day Care is offered at The Wilkinson Residential Healthcare Facility on the Memorial campus. Adult Day Health Care is designed to meet the health care needs of chronically ill, frail elderly, and disabled adults who require certain primary, preventive, diagnostic, therapeutic, rehabilitative or palliative services.

Provides assistance to help older adults ages 60 and older who are not Medicaid eligible and their families delay or avert costly and premature placement in nursing facilities. Services include case management services and non-medical supportive services which can include aide service for light housekeeping and personal care as well as respite. A consumer directly model, where the care receiver hires someone they know, is also offered.

Marjorie Doyle Rockwell Center and Eddy Alzheimer's Center at Hawthorne Ridge are the only medical model adult day services in the Capital Region designed specifically for older adults with memory loss who are still living in their own homes or in the community with caregivers. Adult Day Programs offer a full range of health and social services to help older adults with physical, emotional, or mental impairments, or who require supervision and/or medical care. Services enable older adults to remain independent for as long as possible, while allowing caregivers relief from the responsibilities of at-home care giving so they can fulfill work or other obligations. Transportation is provided in geographic area. Lunch and snacks are also provided for participants.

Encourages self-esteem, social interactions, and independence to keep the participants with their caregivers in the community. Social work. Activity programs. Assistance with daily medication. Personal care and hygiene. Nursing (on-site lab work, wound care, acute medical needs). Arrangement and transportation to and from medical appointments. Physical, speech, and occupational therapies. Provides medical, rehabilitation, nursing and socialization services for adults who live at home. Community physicians, with the help of the staff, provide chronic disease management, wound care, therapy, recreation therapy, and diet/weight management programs one to five days each week. Light breakfast, hot lunch and snacks are provided. Transportation is available.

Medical model adult daycare program for older adults in need of daily supervision and health services.

Offers two (day and evening) five hour a day programs for individualized medical and nursing care. Provides social work services, therapeutic recreation, rehabilitation, nutrition counseling, dental services, podiatry services, medication counseling, audiology screening, socialization, hot lunch and transportation. Serves frail elderly and disabled who are unable to function independently in the community, physically or mentally impaired, socially isolated or those who need assistance with personal care.

Offers a full service adult day program for cognitively impaired and physically frail with a variety of recreational and therapeutic activities, including special attention to nutrition. Also offers short-term respite care for caregivers, and an Alzheimer's caregiver support group.

Medical model that provides supportive services for families or spouses that desire to keep their loves ones at home.

Offers day program providing same services as a nursing home. Serves frail/ill/handicapped persons living at home. Offers physical/occupational/speech therapies,dietary/podiatry/dental/optometry services, and medication management. Conducts therapeutic recreational activities to provide stimulation and encourage socialization. Serves two meals, assists with activities of daily living, offers wellness programs, hair care services. Provides transportation. Offers family support/counseling.

Skilled nursing facility and Adult Day Health Care providing short and long term care. Offers Alzheimer's Disease/Dementia Care, dialysis services, hospice care, intensive rehabilitation services including physical, occupational, and speech therapy, IV Therapy including hydration and medication administration, long term care, orthopedic rehabilitation, pain management, respiratory therapy, respite care, short term rehabilitation, mental health, and wound care. Provides an adult day program for those who need some supervision during the day due to age, illness or disability. Services include nursing care, rehabilitation therapy, social recreation, transportation and two meals.

Offers an adult day program for older adults who need some supervision during the day, with on-site medical care, rehabilitation therapy, recreation, pastoral care, meals, transportation and a hair salon.

Case management, evaluation of needs and service coordination for elderly people designed to assist caregivers in keeping frail elderly people living in their homes. Home visits are also available.
