Medical Adult Day Health Care Program that services people who are visually impaired and medically compromised. Meals provided. Transportation is arranged. Provides case management, both medical and social for participants. Clinical services available to participants may include: audiology (referral basis), referral to dentist and podiatry, RNs on staff, and registered dietician. Occupational therapy, physical therapy, and speech language pathology are provided. Social worker on staff. Vision rehabilitation and training provided. Teaches skills for daily living for people who are visually impaired, such as adaptive handling of money, Braille, food preparation (peeling, slicing), mobility training, and safe use of stairs. Recreation therapy provided; includes crafting, outings, discussion groups, exercise, special events and entertainment, religious activities, and cooking groups.

Day services appropriate for people who require comprehensive medical supports including nursing, medication administration, and behavioral assistance, and who are not pursuing employment.

Provides primary care and emergency services for veterans. Inpatient and outpatient treatment includes evaluation, treatment, medical, surgical, psychiatry, neurology and rehabilitation medicine. A full range of health screenings, health education resources, and prevention programs (weight, smoking cessation) are available. Services for women veterans include GYN services, PAP smears, mammograms, and programs for domestic violence and sexual trauma. Provides comprehensive outpatient health and rehabilitation care, as well as a supportive group environment with recreational activities. Health services provided include nursing care, personal care services, audiology, speech therapy, health monitoring, nutritional services, occupational therapy, physical therapy, case management, and health care monitoring of blood pressure, blood sugar levels, medication, and other health regimes. The following services may be arranged as needed: laboratory services, radiology, dentistry, ophthalmology, podiatry, and beauty/barber shop visits. Offers services to individuals with Traumatic Brain Injury, Multiple Sclerosis, Strokes, Parkinson's Disease, Diabetes, Hypertension, Intellectual Disabilities, and Behavioral Health issues.

Provides both a medical model adult day program with speech, occupational and physical therapies, recreation and counseling, and a social model day program with socialization, nutrition and supportive services, including nurse supervision and transportation for both programs.

Offers an adult day health care program for individuals with a diagnosed health condition and a functional vision problem, to help maintain as much independence as possible. Eddy DayBreak is an innovative adult day service that helps improve quality of life for adults who still live independently, or are being cared for by family members. Offering supervision and/or assistance with personal and/or medical care, Eddy DayBreak programs provide caregivers relief from at-home caregiving, allowing them to fulfill work or other obligations. Round-trip transportation is available and participants may attend up to five days a week. Eddy DayBreak programs include medical, social and dementia-specific models.

Provides assistance to help older adults ages 60 and older who are not Medicaid eligible and their families delay or avert costly and premature placement in nursing facilities. Services include case management services and non-medical supportive services which can include aide service for light housekeeping and personal care as well as respite. A consumer directly model, where the care receiver hires someone they know, is also offered.
