Eddy DayBreak is an innovative adult day service that helps improve quality of life for seniors who still live independently in the community, or who are cared for by family members. DayBreak programs offer a full range of health and social services to help older adults with physical, emotional, or mental impairments, or who require supervision and/or medical care. Services enable older adults to remain independent for as long as possible, while allowing caregivers relief from the responsibilities of at-home caregiving so they can fulfill work or other obligations.

Encourages self-esteem, social interactions, and independence to keep the participants with their caregivers in the community. Social work. Activity programs. Assistance with daily medication. Personal care and hygiene. Nursing (on-site lab work, wound care, acute medical needs). Arrangement and transportation to and from medical appointments. Physical, speech, and occupational therapies. The Wilkinson Adult Day Health Center is a community-based long term care program that provides comprehensive health care services in a congregate day setting. Clients are provided with the medical and social services they need to remain home and part of the community. St. Mary's Adult Day Care is offered at The Wilkinson Residential Healthcare Facility on the Memorial campus. Adult Day Health Care is designed to meet the health care needs of chronically ill, frail elderly, and disabled adults who require certain primary, preventive, diagnostic, therapeutic, rehabilitative or palliative services.

Provides a 7-day a week adult day program which includes nursing care, physical, occupational and speech therapy, social work, recreational activities, outings, meals, nutrition education, and wheelchair accessible transportation.

* Offers a Program of All-Inclusive Care for the Elderly (PACE), which works with seniors and their caregivers to arrange for and monitor all of their medical, social and daily living support services. Designed for the frail elderly who need assistance in the activities of daily living.

Provides the following services: - Information and referral on all services - Long-term care needs of the elderly - In-home assessment of elderly clients and development of specialized care plan - Case management to link clients to needed services - Advice for clients and family Also provides home health services and adult day care to eligible clients as funding permits; may be able to share cost for home health and adult day care services. Caregiver Resource Center has been developed to offer support, information, and services to those who care for seniors and to seniors who care for family members who are under the age of 18.

A unique social day program that meets the needs of those who have developed dementia or memory loss caused by Alzheimer's or Parkinson's disease, stroke, heart disease, or any other illness.

Provides social and recreational activities, entertainment, trips and speakers. Meals on Wheels senior activity centers are located in Nanuet, Nyack, New City. Transportation and a hot nutritious lunch are provided.

Offers an adult day health care program for individuals with a diagnosed health condition and a functional vision problem, to help maintain as much independence as possible. Services include nursing supervision, social work services, health education, occupational and physical therapy, vision rehabilitation, diabetes self-management education, nutritional counseling and therapeutic activities, including breakfast, lunch and transportation.

Offers a full service Medical Model Day Care Program for Alzheimer's, frail, disabled, or impaired registrants. Provides nursing, recreational, physical, occupational, and speech therapies, psychiatric counseling and family support meetings. Offers ophthalmology, podiatry, social, nutritional, and pharmacy services. Transportation and hot lunches are provided.

Provides health care supervision and socialization for any adult, frail or disabled, primarily ages 50-90+.

Offers an adult day program, six days a week, for older adults who need supervision during the day, which includes medical treatment, nursing, therapeutic recreational activities, transportation, meals and other support services.

Provides medical services to adults in a community day setting. Services include medication administration/treatments by LPN/RNs, case management, physical therapy, occupational therapy, and speech therapy, ongoing daily activities, and lunch and nutritional services overseen by a registered dietitian.

Provides assistance to help older adults ages 60 and older who are not Medicaid eligible and their families delay or avert costly and premature placement in nursing facilities. Services include case management services and non-medical supportive services which can include aide service for light housekeeping and personal care as well as respite. A consumer directly model, where the care receiver hires someone they know, is also offered.

Provides supervision, socialization and stimulation to individuals 60 years and older who are physically frail or cognitively impaired, and/or in need of some assistance with activities of daily living along respite for their overwhelmed caregiver. Assists individuals who would like to continue to live in the community even if they are experiencing some physical and/or cognitive decline, for those who want to spend time with others, and for those who might otherwise be isolated or unsafe.

* Provides day programs (structured, day habilitation, adult day social) for individuals with brain injury; social skill development, peer support, compensatory strategies and cognitive training.
