Provides case management, both medical and social for participants. Clinical services available to participants may include: audiology (referral basis), referral to dentist and podiatry, RNs on staff, and registered dietician. Occupational therapy, physical therapy, and speech language pathology are provided. Social worker on staff. Vision rehabilitation and training provided. Teaches skills for daily living for people who are visually impaired, such as adaptive handling of money, Braille, food preparation (peeling, slicing), mobility training, and safe use of stairs. Recreation therapy provided; includes crafting, outings, discussion groups, exercise, special events and entertainment, religious activities, and cooking groups.

Provides comprehensive outpatient health and rehabilitation care, as well as a supportive group environment with recreational activities. Health services provided include nursing care, personal care services, audiology, speech therapy, health monitoring, nutritional services, occupational therapy, physical therapy, case management, and health care monitoring of blood pressure, blood sugar levels, medication, and other health regimes. Services enable older adults to remain independent for as long as possible, while allowing caregivers relief from the responsibilities of at-home care giving so they can fulfill work or other obligations.
