Offers two (day and evening) five hour a day programs for individualized medical and nursing care. Provides social work services, therapeutic recreation, rehabilitation, nutrition counseling, dental services, podiatry services, medication counseling, audiology screening, socialization, hot lunch and transportation. Serves frail elderly and disabled who are unable to function independently in the community, physically or mentally impaired, socially isolated or those who need assistance with personal care.

Marjorie Doyle Rockwell Center and Eddy Alzheimer's Center at Hawthorne Ridge are the only medical model adult day services in the Capital Region designed specifically for older adults with memory loss who are still living in their own homes or in the community with caregivers. Adult Day Programs offer a full range of health and social services to help older adults with physical, emotional, or mental impairments, or who require supervision and/or medical care. Services enable older adults to remain independent for as long as possible, while allowing caregivers relief from the responsibilities of at-home care giving so they can fulfill work or other obligations. Transportation is provided in geographic area. Lunch and snacks are also provided for participants.

Provides medical, rehabilitation, nursing and socialization services for adults who live at home. Community physicians, with the help of the staff, provide chronic disease management, wound care, therapy, recreation therapy, and diet/weight management programs one to five days each week. Light breakfast, hot lunch and snacks are provided. Transportation is available.

Adult daycare-medical model services.

Provides both a medical model adult day program with speech, occupational and physical therapies, recreation and counseling, and a social model day program with socialization, nutrition and supportive services, including nurse supervision and transportation for both programs.

Eddy DayBreak is an innovative adult day service that helps improve quality of life for adults who still live independently, or are being cared for by family members. Offering supervision and/or assistance with personal and/or medical care, Eddy DayBreak programs provide caregivers relief from at-home caregiving, allowing them to fulfill work or other obligations. Round-trip transportation is available and participants may attend up to five days a week. Eddy DayBreak programs include medical, social and dementia-specific models.

* Offers a Program of All-Inclusive Care for the Elderly (PACE), which works with seniors and their caregivers to arrange for and monitor all of their medical, social and daily living support services. Day services appropriate for people who require comprehensive medical supports including nursing, medication administration, and behavioral assistance, and who are not pursuing employment.

Provides primary care and emergency services for veterans. Inpatient and outpatient treatment includes evaluation, treatment, medical, surgical, psychiatry, neurology and rehabilitation medicine. A full range of health screenings, health education resources, and prevention programs (weight, smoking cessation) are available. Services for women veterans include GYN services, PAP smears, mammograms, and programs for domestic violence and sexual trauma. The following services may be arranged as needed: laboratory services, radiology, dentistry, ophthalmology, podiatry, and beauty/barber shop visits. Provides medical services to adults in a community day setting. Services include medication administration/treatments by LPN/RNs, case management, physical therapy, occupational therapy, and speech therapy, ongoing daily activities, and lunch and nutritional services overseen by a registered dietitian.

Provides supervision, socialization and stimulation to individuals 60 years and older who are physically frail or cognitively impaired, and/or in need of some assistance with activities of daily living along respite for their overwhelmed caregiver. Assists individuals who would like to continue to live in the community even if they are experiencing some physical and/or cognitive decline, for those who want to spend time with others, and for those who might otherwise be isolated or unsafe.

Provides assistance to help older adults ages 60 and older who are not Medicaid eligible and their families delay or avert costly and premature placement in nursing facilities. Services include case management services and non-medical supportive services which can include aide service for light housekeeping and personal care as well as respite. A consumer directly model, where the care receiver hires someone they know, is also offered.
