Medical Adult Day Health Care Program that services people who are visually impaired and medically compromised. Meals provided. Transportation is arranged. Provides case management, both medical and social for participants. Clinical services available to participants may include: audiology (referral basis), referral to dentist and podiatry, RNs on staff, and registered dietician. Occupational therapy, physical therapy, and speech language pathology are provided. Social worker on staff. Vision rehabilitation and training provided. Teaches skills for daily living for people who are visually impaired, such as adaptive handling of money, Braille, food preparation (peeling, slicing), mobility training, and safe use of stairs. Recreation therapy provided; includes crafting, outings, discussion groups, exercise, special events and entertainment, religious activities, and cooking groups.

A center specializing in Alzheimer's and related dementia day care. Provides social model adult day care, socialization, hot lunch, breakfast and afternoon snack. Services include small group crafts, exercises, field trips, personal care in activities of daily life, ongoing care plans, information and referral, transportation within a 15 mile range, and wheelchair transportation. Inter-generational program with on-site child care offered. Alzheimer's support group held the 3rd Thursday of each month, 7pm-9pm.

Eddy DayBreak is an innovative adult day service that helps improve quality of life for adults who still live independently, or are being cared for by family members. Offering supervision and/or assistance with personal and/or medical care, Eddy DayBreak programs provide caregivers relief from at-home caregiving, allowing them to fulfill work or other obligations. Round-trip transportation is available and participants may attend up to five days a week. Eddy DayBreak programs include medical, social and dementia-specific models.

Offers day program providing same services as a nursing home. Serves frail/ill/handicapped persons living at home. Offers physical/occupational/speech therapies,dietary/podiatry/dental/optometry services, and medication management. Conducts therapeutic recreational activities to provide stimulation and encourage socialization. Serves two meals, assists with activities of daily living, offers wellness programs, hair care services. Provides transportation. Offers family support/counseling.

Skilled nursing facility and Adult Day Health Care providing short and long term care. Offers Alzheimer's Disease/Dementia Care, dialysis services, hospice care, intensive rehabilitation services including physical, occupational, and speech therapy, IV Therapy including hydration and medication administration, long term care, orthopedic rehabilitation, pain management, respiratory therapy, respite care, short term rehabilitation, mental health, and wound care. The Wilkinson Adult Day Health Center is a community-based long term care program that provides comprehensive health care services in a congregate day setting. Clients are provided with the medical and social services they need to remain home and part of the community. St. Mary's Adult Day Care is offered at The Wilkinson Residential Healthcare Facility on the Memorial campus. Adult Day Health Care is designed to meet the health care needs of chronically ill, frail elderly, and disabled adults who require certain primary, preventive, diagnostic, therapeutic, rehabilitative or palliative services.

Provides a 7-day a week adult day program which includes nursing care, physical, occupational and speech therapy, social work, recreational activities, outings, meals, nutrition education, and wheelchair accessible transportation.

Marjorie Doyle Rockwell Center and Eddy Alzheimer's Center at Hawthorne Ridge are the only medical model adult day services in the Capital Region designed specifically for older adults with memory loss who are still living in their own homes or in the community with caregivers. Adult Day Programs offer a full range of health and social services to help older adults with physical, emotional, or mental impairments, or who require supervision and/or medical care. Services enable older adults to remain independent for as long as possible, while allowing caregivers relief from the responsibilities of at-home care giving so they can fulfill work or other obligations.
