Eddy DayBreak is an innovative adult day service that helps improve quality of life for adults who still live independently, or are being cared for by family members. Offering supervision and/or assistance with personal and/or medical care, Eddy DayBreak programs provide caregivers relief from at-home caregiving, allowing them to fulfill work or other obligations. Round-trip transportation is available and participants may attend up to five days a week. Eddy DayBreak programs include medical, social and dementia-specific models.

DayBreak programs offer a full range of health and social services to help older adults with physical, emotional, or mental impairments, or who require supervision and/or medical care. Provides both a medical model adult day program with speech, occupational and physical therapies, recreation and counseling, and a social model day program with socialization, nutrition and supportive services, including nurse supervision and transportation for both programs.

Offers day program providing same services as a nursing home. Serves frail/ill/handicapped persons living at home. Offers physical/occupational/speech therapies,dietary/podiatry/dental/optometry services, and medication management. Conducts therapeutic recreational activities to provide stimulation and encourage socialization. Skilled nursing facility and Adult Day Health Care providing short and long term care. Offers Alzheimer's Disease/Dementia Care, dialysis services, hospice care, intensive rehabilitation services including physical, occupational, and speech therapy, IV Therapy including hydration and medication administration, long term care, orthopedic rehabilitation, pain management, respiratory therapy, respite care, short term rehabilitation, mental health, and wound care. Offers a medical model day program for individuals with Alzheimer's, dementia or seniors with memory loss, which includes therapeutic and recreational activities, nursing support, physical and occupational therapy, social work services, community trips, strolling art garden and therapy pool.

Offers a full service Medical Model Day Care Program for Alzheimer's, frail, disabled, or impaired registrants. Provides nursing, recreational, physical, occupational, and speech therapies, psychiatric counseling and family support meetings. Offers ophthalmology, podiatry, social, nutritional, and pharmacy services. Transportation and hot lunches are provided.

Offers a comprehensive adult social day program for individuals who may require supervision during the day; assistance with self-sufficiency is given. Assist caregivers with respite care.

Medical model day programs for older adults. Medical management, physical, occupational and speech therapy inpatient and outpatient. Also offers hot meals, and kosher, therapeutic recreation, respite care, personal care, and care management.

Provides adult day care for functionally impaired adults 40 years and over. Offers a full time nurse and nursing services, specialized medical services, information and referral, social work, recreational activities, counseling, rehabilitation, nutritional evaluation, hot meals, and personal care. Publishes brochure, calendar of events, educational material.

Provided onsite to residents: activities, dental services, dietary services, housekeeping services, health services, nursing services, occupational therapy services, physical therapy, physician services, podiatry, social work services, speech/language pathology services and diagnostic x-ray services. Provided offsite to residents: clinical laboratory services, dental services, mental health services, pharmacy services, podiatry services, and diagnostic x-ray services.

Day services appropriate for people who require comprehensive medical supports including nursing, medication administration, and behavioral assistance, and who are not pursuing employment.

Provides primary care and emergency services for veterans. Inpatient and outpatient treatment includes evaluation, treatment, medical, surgical, psychiatry, neurology and rehabilitation medicine. A full range of health screenings, health education resources, and prevention programs (weight, smoking cessation) are available. Services for women veterans include GYN services, PAP smears, mammograms, and programs for domestic violence and sexual trauma.
