Eddy DayBreak is an innovative adult day service that helps improve quality of life for seniors who still live independently in the community, or who are cared for by family members. DayBreak programs offer a full range of health and social services to help older adults with physical, emotional, or mental impairments, or who require supervision and/or medical care. Services enable older adults to remain independent for as long as possible, while allowing caregivers relief from the responsibilities of at-home caregiving so they can fulfill work or other obligations.

Marjorie Doyle Rockwell Center and Eddy Alzheimer's Center at Hawthorne Ridge are the only medical model adult day services in the Capital Region designed specifically for older adults with memory loss who are still living in their own homes or in the community with caregivers. Adult Day Programs offer a full range of health and social services to help older adults with physical, emotional, or mental impairments, or who require supervision and/or medical care. Services enable older adults to remain independent for as long as possible, while allowing caregivers relief from the responsibilities of at-home care giving so they can fulfill work or other obligations. Transportation is provided in geographic area. Lunch and snacks are also provided for participants.

A center specializing in Alzheimer's and related dementia day care. Provides social model adult day care, socialization, hot lunch, breakfast and afternoon snack. Services include small group crafts, exercises, field trips, personal care in activities of daily life, ongoing care plans, information and referral, transportation within a 15 mile range, and wheelchair transportation. Inter-generational program with on-site child care offered. Alzheimer's support group held the 3rd Thursday of each month, 7pm-9pm.

Provides the following services: - Information and referral on all services - Long-term care needs of the elderly - In-home assessment of elderly clients and development of specialized care plan - Case management to link clients to needed services - Advice for clients and family Also provides home health services and adult day care to eligible clients as funding permits; may be able to share cost for home health and adult day care services. Caregiver Resource Center has been developed to offer support, information, and services to those who care for seniors and to seniors who care for family members who are under the age of 18.

Provides adult day care for functionally impaired adults 40 years and over. Offers a full time nurse and nursing services, specialized medical services, information and referral, social work, recreational activities, counseling, rehabilitation, nutritional evaluation, hot meals, and personal care. Publishes brochure, calendar of events, educational material.

Offers a medical model day program for individuals with Alzheimer's, dementia or seniors with memory loss, which includes therapeutic and recreational activities, nursing support, physical and occupational therapy, social work services, community trips, strolling art garden and therapy pool.

Designed for the frail elderly who need assistance in the activities of daily living.

Offers a full service adult day program for cognitively impaired and physically frail with a variety of recreational and therapeutic activities, including special attention to nutrition. Also offers short-term respite care for caregivers, and an Alzheimer's caregiver support group.

Provides assistance to help older adults ages 60 and older who are not Medicaid eligible and their families delay or avert costly and premature placement in nursing facilities. Services include case management services and non-medical supportive services which can include aide service for light housekeeping and personal care as well as respite. A consumer directly model, where the care receiver hires someone they know, is also offered.

The Wilkinson Adult Day Health Center is a community-based long term care program that provides comprehensive health care services in a congregate day setting. Clients are provided with the medical and social services they need to remain home and part of the community. St. Mary's Adult Day Care is offered at The Wilkinson Residential Healthcare Facility on the Memorial campus. Adult Day Health Care is designed to meet the health care needs of chronically ill, frail elderly, and disabled adults who require certain primary, preventive, diagnostic, therapeutic, rehabilitative or palliative services.

Offering supervision and/or assistance with personal and/or medical care, Eddy DayBreak programs provide caregivers relief from at-home caregiving, allowing them to fulfill work or other obligations. Round-trip transportation is available and participants may attend up to five days a week. Eddy DayBreak programs include medical, social and dementia-specific models.

* Provides a medical and social adult day program serving seniors in a comfortable home-like environment. * Provides health care services: Case management and social work counseling. Health care monitoring and skilled nursing care. Physical, occupational and speech therapy. Hygiene, bathing and personal care. Nutrition and dietary counseling. Podiatry visits. Dental hygiene. Hearing evaluations . Lab and diagnostic studies. Mental health referrals. Transportation. Socialization.* Provides recreational services: Recreation therapy: music, arts & crafts, gardening and more. Field trips and outings to the mall, museums, theater, etc. Visits from area school children. Group exercise and individualized weight training/walking programs. Religious and educational activities. Trips to the hair salon.

Offers a full service Medical Model Day Care Program for Alzheimer's, frail, disabled, or impaired registrants. Provides nursing, recreational, physical, occupational, and speech therapies, psychiatric counseling and family support meetings. Offers ophthalmology, podiatry, social, nutritional, and pharmacy services. Transportation and hot lunches are provided.

Provides medical, rehabilitation, nursing and socialization services for adults who live at home. Community physicians, with the help of the staff, provide chronic disease management, wound care, therapy, recreation therapy, and diet/weight management programs one to five days each week. Light breakfast, hot lunch and snacks are provided. Transportation is available.
