Provides individualized services and support to individuals with acquired brain injuries (ABI) and their families. Allows ABI survivors to explore their capacity for learning within a member-driven environment that offers a variety of activities and supports, such as creative writing, memory-enhancement activities, and music and art programs.

Day program for older adults and adults with developmental disabilities. Provides socialization, community involvement, nutritional program, transportation, recreation, and case management. Helps older adults and adults with developmental disabilities remain at home and gives caregivers time away from caregiving to help them do whatever they need to do, whether that be running errands, attending appointments, or going to work. Respite services are also offered at this location; clients can select which days and times to attend.

Offers assistance to those who would like to continue living at home. Provides high quality social, nutritional, and daily living services in a safe, supportive and stimulating environment. Optional services include transportation, showering and a barber/beauty shop.

Provided onsite to residents: activities, dental services, dietary services, housekeeping services, health services, nursing services, occupational therapy services, physical therapy, physician services, podiatry, social work services, speech/language pathology services and diagnostic x-ray services. Offers seniors with intellectual disabilities, developmental disabilities, and other secondary diagnoses such as dementia, Parkinson's disease, and traumatic brain injury an opportunity to spend time with others in a creative and social atmosphere. Activities are person-centered and paced to meet the needs of seniors. The staff is trained in identifying and addressing the changing needs of this population and assists participants in maintaining daily living skills and exercise, along with memory maintenance through trivia, reminiscing, community outreach, music, and art activities. The program's goal is to provide a retirement atmosphere while encouraging participants to stay fit and active while achieving their goals and aspirations.

A program designed for those who are well enough to live at home but are in need of daily supervision and medical monitoring. Offers health care services including, nursing care, physical, occupational and speech therapies, therapeutic recreation programs, nutritious noontime meals and snacks, social work counseling and therapeutic exercise, wellness programs. Door-to-door transportation is included..

Provides a specialized day program for those with Alzheimer's Disease and other dementia related memory loss. Programming includes socialization, case management, nutrition, transportation, recreation, and community involvement. Helps older adults to remain at home and gives caregivers time away from caregiving to help them do whatever they need to do, whether that be running errands, attending appointments, or going to work. Respite services, caregiver support groups, and information available.

Social adult day care program for physically and/or mentally impaired adults that offers support, supervision, stimulation, and socialization for clients, along with respite and support for caregivers. Offers recreational and inter-generational activities with support staff from Kaleida Health System. Also offers personal care (including shower and beauty salon services), lunch and snacks, case management, and family support groups.

Offers elderly or severely disabled individuals residing at home a day program providing nursing and medical care, case/care management and recreation. Services include access to a variety of medical specialists (dentist, podiatrist, audiologist, therapists), and individualized nursing care for a wide range of medical conditions associated with chronic illness and aging including Alzheimer's, dementia, diabetes and Parkinson's Disease. Program attendance is based on individual needs. Transportation to and from the Center is available.

Offers a lunch meal, during the week, for seniors in community, giving them an opportunity to socialize and get a nutritious meal.

Provides social model adult day services, including lunch for frail elderly persons and early dementia. Bus transportation available. A nurse is available on Tuesdays and Fridays from 10am-12noon.

Provides daily lunch program, Nassau County sponsored classes, special events, transportation, parties, arts and crafts, volunteer opportunities, guest speakers, book clubs and more.Catchment Areas: Bethpage, Farmingdale, Massapequa, Massapequa Park, Plainview, Seaford, Wantagh.

Medical model adult daycare program for older adults in need of daily supervision and health services.

Offer day habilitation, pre-vocational training, and supported employment. Specializes in men and women's day habilitation programs which are custom-made according to the needs and desired outcomes of each particular individual. Consists of sessions in life skills, independent living skills, social skills, community integration, and recreational activities. Pre-vocational and supported employment programs are available to help train and sustain individuals to be productive and employable members of the community.
