Provides individualized services and support to individuals with acquired brain injuries (ABI) and their families. Allows ABI survivors to explore their capacity for learning within a member-driven environment that offers a variety of activities and supports, such as creative writing, memory-enhancement activities, and music and art programs.

The Wilkinson Adult Day Health Center is a community-based long term care program that provides comprehensive health care services in a congregate day setting. Clients are provided with the medical and social services they need to remain home and part of the community. St. Mary's Adult Day Care is offered at The Wilkinson Residential Healthcare Facility on the Memorial campus. Adult Day Health Care is designed to meet the health care needs of chronically ill, frail elderly, and disabled adults who require certain primary, preventive, diagnostic, therapeutic, rehabilitative or palliative services.

Provides day care services for elderly and disabled adults.

Adult Medical Day Care Program provides adult day care and respite services. St. Mary's Adult Day Care is offered at the Wilkinson Residential Healthcare Facility on the Memorial campus.

Provides personal care services including health and medical care, case management, health assessment, physical rehabilitation therapy, occupational therapy, speech therapy, and nutritional education as well as activity programs throughout the day.

Provides an intergenerational program with compassionate, person-centered care ans supervision for frail older adults living with both physical and/or cognitive impairments during the day. Services include social and recreational activities, training for sustaining activities of daily living and meals/snacks, along with caregiver support and case management services. Also offers an evening program, twice a week for adults who are newly diagnosed with Alzheimer's or dementia.

Provides supervised, meaningful day activities for frail elderly people who may be suffering from Alzheimer's disease and related disorders. In-depth treatment plans are devised for each client and much one-on-one attention is given. Two meals a day plus afternoon snack are provided. Call for further information.

Offers adult day care programs which include dietary evaluation, continental breakfast, lunch, occupational and physical therapy, social work, pharmaceutical, dental, podiatry services, audiology, psychological services, ophathalmology, and door to door transport in handicapped accessible vehicles.

Offer day habilitation, pre-vocational training, and supported employment. Specializes in men and women's day habilitation programs which are custom-made according to the needs and desired outcomes of each particular individual. Consists of sessions in life skills, independent living skills, social skills, community integration, and recreational activities. Pre-vocational and supported employment programs are available to help train and sustain individuals to be productive and employable members of the community.

Provides a day program for adults who need supervision for a few hours a day. Services include two meals, snacks, planned activities, social interaction, and medication management.

155-unit congregate housing for active adults 55 years and up. Housing for seniors, provides additional services such as housekeeping, leisure time activities, maintenance, and restaurant style dining, residents enjoy an independent lifestyle. Also available, skilled nursing facilities and adult day care. Residents may access home healthcare services from an onsite, licensed home care agency or by contacting a home healthcare agency of their choice.

Offers a social model program for frail or isolated seniors, or those with memory loss, Alzheimer's or dementia that need monitoring and structured programming. The program includes wide range of therapeutic and recreational activities, meals, outings, and assistance with daily living skills.

An adult day program for seniors who need some supervision and care, with services of social and recreational activities, and training that is enhances daily living activities.

Offers a variety of activities for seniors in the community, such as aerobics, refreshments, arts and crafts, guest speakers, workshops, cards, bingo, games, and more.

A program designed for those who are well enough to live at home but are in need of daily supervision and medical monitoring. Offers health care services including, nursing care, physical, occupational and speech therapies, therapeutic recreation programs, nutritious noontime meals and snacks, social work counseling and therapeutic exercise, wellness programs. Door-to-door transportation is included..

Offers several clubhouse-like day centers for legally blind persons age 55 and older. Seniors receive special counseling and training, including updates on Social Security, Medicaid/Medicare, and nutrition. Members make friends and join in arts and crafts, games, exercise classes, luncheons, and trips to shows and museums.

Provides a social day care program for the frail elderly, including transportation within our catchment area, hot meal, blood pressure screening, recreation, arts and crafts, music therapy, exercise therapy, trips, outings, and parties. Serves frail, elderly, at-risk or isolated persons over age of 60 with one or more physical or mental deficits who are continent, able to feed and toilet themselves and work in a group.

Provides individual and group therapeutic recreation which enhance and individual's intellectual, physical, spiritual, social and emotional well being through therapeutic and stimulating activities to foster social interaction and enjoyment. Activities include trips, monthly parties, table games and painting. We offer medical assistance, physical therapy, occupational therapy, and a social worker who can help with all concierge services. Transportation is provided to and from program.

Offers assistance to those who would like to continue living at home. Provides high quality social, nutritional, and daily living services in a safe, supportive and stimulating environment. Optional services include transportation, showering and a barber/beauty shop.

This rehabilitation and day health care program offers short-term rehabilitation, inpatient, and outpatient therapy.
