Provides individualized services and support to individuals with acquired brain injuries (ABI) and their families. Allows ABI survivors to explore their capacity for learning within a member-driven environment that offers a variety of activities and supports, such as creative writing, memory-enhancement activities, and music and art programs.

* Offers day habilitation services. Social and recreational activities, while receiving assistance with personal care and other areas of need. Individuals work toward goals and objectives that are derived from their valued outcomes.* Provides at-home assistance to individuals to help them become increasingly independent and able to live in the least restrictive environment possible.* Offers weekday and weekend respite. Weekend Respite provides an opportunity to socialize and enjoy a variety of recreational activities. Assistance with personal care, meal times, medication and other areas of need.* Cayuga Centers is granted funding to assist families with costs of goods or services directly related to the care of a family member with a developmental disability.

* Provides Veteran patients with general health care and social needs to allow them to continue living at home or with family. Services include: Therapeutic exercise. Recreational activities. Health education & medication management. Meals. Care for patients with dementia. Support and information for family caregivers.

Adult Medical Day Care Program provides adult day care and respite services. St. Mary's Adult Day Care is offered at the Wilkinson Residential Healthcare Facility on the Memorial campus.

Provides personal care services including health and medical care, case management, health assessment, physical rehabilitation therapy, occupational therapy, speech therapy, and nutritional education as well as activity programs throughout the day.

The Wilkinson Adult Day Health Center is a community-based long term care program that provides comprehensive health care services in a congregate day setting. Clients are provided with the medical and social services they need to remain home and part of the community. St. Mary's Adult Day Care is offered at The Wilkinson Residential Healthcare Facility on the Memorial campus. Adult Day Health Care is designed to meet the health care needs of chronically ill, frail elderly, and disabled adults who require certain primary, preventive, diagnostic, therapeutic, rehabilitative or palliative services.

Provides a social day care program for the frail elderly, including transportation within our catchment area, hot meal, blood pressure screening, recreation, arts and crafts, music therapy, exercise therapy, trips, outings, and parties. Serves frail, elderly, at-risk or isolated persons over age of 60 with one or more physical or mental deficits who are continent, able to feed and toilet themselves and work in a group.

Provides an adult day program for those who need some supervision during the day due to age, illness or disability. Services include nursing care, rehabilitation therapy, social recreation, transportation and two meals.

An adult day program for seniors who need some supervision and care, with services of social and recreational activities, and training that is enhances daily living activities.

Provides a skilled nursing facility to care for older adults needing round-the-clock care, including a special care unit for those diagnosed with Alzheimer's or dementia. Respite care individuals requiring continuous care for short periods of time. Also offers hospice care for those who are dying or at end-stage of illness.

Day program for older adults and adults with developmental disabilities. Provides socialization, community involvement, nutritional program, transportation, recreation, and case management. Helps older adults and adults with developmental disabilities remain at home and gives caregivers time away from caregiving to help them do whatever they need to do, whether that be running errands, attending appointments, or going to work. Respite services are also offered at this location; services can be provided for as little as one hour to a full day.

Provides a specialized day program for those with Alzheimer's Disease and other dementia related memory loss. Programming includes socialization, case management, nutrition, transportation, recreation, and community involvement. Helps older adults to remain at home and gives caregivers time away from caregiving to help them do whatever they need to do, whether that be running errands, attending appointments, or going to work. Respite services, caregiver support groups, and information available.

Offers seniors with intellectual disabilities, developmental disabilities, and other secondary diagnoses such as dementia, Parkinson's disease, and traumatic brain injury an opportunity to spend time with others in a creative and social atmosphere. Activities are person-centered and paced to meet the needs of seniors. The staff is trained in identifying and addressing the changing needs of this population and assists participants in maintaining daily living skills and exercise, along with memory maintenance through trivia, reminiscing, community outreach, music, and art activities. The program's goal is to provide a retirement atmosphere while encouraging participants to stay fit and active while achieving their goals and aspirations.

Social model adult day services. Provides assistance with personal care, nutrition services, transportation and escort services, case management, and structured activity program.

Social adult day care program for physically and/or mentally impaired adults that offers support, supervision, stimulation, and socialization for clients, along with respite and support for caregivers. Offers recreational and inter-generational activities with support staff from Kaleida Health System. Also offers personal care (including shower and beauty salon services), lunch and snacks, case management, and family support groups.

Provides individual and group therapeutic recreation which enhance and individual's intellectual, physical, spiritual, social and emotional well being through therapeutic and stimulating activities to foster social interaction and enjoyment. Activities include trips, monthly parties, table games and painting. We offer medical assistance, physical therapy, occupational therapy, and a social worker who can help with all concierge services. Transportation is provided to and from program.

Offers socialization, recreation and a nutritious meal.

Provides an intergenerational program with compassionate, person-centered care ans supervision for frail older adults living with both physical and/or cognitive impairments during the day. Services include social and recreational activities, training for sustaining activities of daily living and meals/snacks, along with caregiver support and case management services. Also offers an evening program, twice a week for adults who are newly diagnosed with Alzheimer's or dementia.
