Eddy DayBreak is an innovative adult day service that helps improve quality of life for seniors who still live independently in the community, or who are cared for by family members. DayBreak programs offer a full range of health and social services to help older adults with physical, emotional, or mental impairments, or who require supervision and/or medical care. Services enable older adults to remain independent for as long as possible, while allowing caregivers relief from the responsibilities of at-home caregiving so they can fulfill work or other obligations.

Marjorie Doyle Rockwell Center and Eddy Alzheimer's Center at Hawthorne Ridge are the only medical model adult day services in the Capital Region designed specifically for older adults with memory loss who are still living in their own homes or in the community with caregivers. Adult Day Programs offer a full range of health and social services to help older adults with physical, emotional, or mental impairments, or who require supervision and/or medical care. Services enable older adults to remain independent for as long as possible, while allowing caregivers relief from the responsibilities of at-home care giving so they can fulfill work or other obligations. Transportation is provided in geographic area. Lunch and snacks are also provided for participants.

A program designed for those who are well enough to live at home but are in need of daily supervision and medical monitoring. Offers health care services including, nursing care, physical, occupational and speech therapies, therapeutic recreation programs, nutritious noontime meals and snacks, social work counseling and therapeutic exercise, wellness programs. Door-to-door transportation is included..

Offers a program for someone who has been diagnosed with Alzheimer’s, dementia, Parkinson’s, or another degenerative illness. "Our Space" is a social model of adult day services established in 1987 for Nassau County residents over the age of 60 who are frail and memory impaired. Caregivers are provided with respite, education and support. All programs require an assessment to ensure proper placement. Volunteer opportunities are also available. Provides a social day care program for the frail elderly, including transportation within our catchment area, hot meal, blood pressure screening, recreation, arts and crafts, music therapy, exercise therapy, trips, outings, and parties. Serves frail, elderly, at-risk or isolated persons over age of 60 with one or more physical or mental deficits who are continent, able to feed and toilet themselves and work in a group.

Adult Medical Day Care Program provides adult day care and respite services. St. Mary's Adult Day Care is offered at the Wilkinson Residential Healthcare Facility on the Memorial campus.

* Offers a Program of All-Inclusive Care for the Elderly (PACE), which works with seniors and their caregivers to arrange for and monitor all of their medical, social and daily living support services. Provides assistance to help older adults ages 60 and older who are not Medicaid eligible and their families delay or avert costly and premature placement in nursing facilities. Services include case management services and non-medical supportive services which can include aide service for light housekeeping and personal care as well as respite. A consumer directly model, where the care receiver hires someone they know, is also offered.
