Provides a skilled nursing facility to care for older adults needing round-the-clock care, including a special care unit for those diagnosed with Alzheimer's or dementia. Respite care individuals requiring continuous care for short periods of time. Also offers hospice care for those who are dying or at end-stage of illness.

Offers a variety of social and recreational activities geared for those who have suffered a stroke or have physical disabilities. Activities include games, discussions, creative activities, holiday activities, picnics, group speech therapy or individual computer programs to improve language skills, seated physical exercise, guest speakers, field trips, dance and musical events. NOTE: Participants need to bring their own lunch, but snacks and beverages are provided.

Provides an adult day program with social activities and day respite services geared for care givers of those diagnosed with Alzheimer's or Dementia.

Provides health care and social interaction for frail or functionally impaired adults in a friendly and safe environment, as well as offering rest, support and counseling to family and caregivers. Respite care provides temporary, 24 hours skilled nursing care for caregivers that need temporary relief from care giving.

Marjorie Doyle Rockwell Center and Eddy Alzheimer's Center at Hawthorne Ridge are the only medical model adult day services in the Capital Region designed specifically for older adults with memory loss who are still living in their own homes or in the community with caregivers. Adult Day Programs offer a full range of health and social services to help older adults with physical, emotional, or mental impairments, or who require supervision and/or medical care. Services enable older adults to remain independent for as long as possible, while allowing caregivers relief from the responsibilities of at-home care giving so they can fulfill work or other obligations. Transportation is provided in geographic area. Lunch and snacks are also provided for participants.

Provides care to people who need long term care services and help to live independently in their own homes and communities.A wide range of services are available to suit individuals and their circumstances. Services include nursing, home health aides, therapy, medical supplies, adult day social care, non-emergency transportation, nutrition care with home delivered meals and much more.

Provides assistance to help older adults ages 60 and older who are not Medicaid eligible and their families delay or avert costly and premature placement in nursing facilities. Services include case management services and non-medical supportive services which can include aide service for light housekeeping and personal care as well as respite. A consumer directly model, where the care receiver hires someone they know, is also offered.

Starting with the individual's strengths and interests and emphasizing personal choice, The Arc Otsego offers comprehensive network of community-based programs and services that encourage individuals with intellectual and other developmental disabilities. The Wilkinson Adult Day Health Center is a community-based long term care program that provides comprehensive health care services in a congregate day setting. Clients are provided with the medical and social services they need to remain home and part of the community. St. Mary's Adult Day Care is offered at The Wilkinson Residential Healthcare Facility on the Memorial campus. Adult Day Health Care is designed to meet the health care needs of chronically ill, frail elderly, and disabled adults who require certain primary, preventive, diagnostic, therapeutic, rehabilitative or palliative services.

Offers assistance to those who would like to continue living at home. Provides high quality social, nutritional, and daily living services in a safe, supportive and stimulating environment. Optional services include transportation, showering and a barber/beauty shop.

Adult social day program provides a safe and stimulating environment for individuals age 55 and older. Individuals may attend on a planned basis, ranging from one to five days per week according to need.

Offers seniors with intellectual disabilities, developmental disabilities, and other secondary diagnoses such as dementia, Parkinson's disease, and traumatic brain injury an opportunity to spend time with others in a creative and social atmosphere. Activities are person-centered and paced to meet the needs of seniors. The staff is trained in identifying and addressing the changing needs of this population and assists participants in maintaining daily living skills and exercise, along with memory maintenance through trivia, reminiscing, community outreach, music, and art activities. The program's goal is to provide a retirement atmosphere while encouraging participants to stay fit and active while achieving their goals and aspirations.

Medical Adult Day Health Care Program that services people who are visually impaired and medically compromised. Meals provided. Transportation is arranged. Provides case management, both medical and social for participants. Clinical services available to participants may include: audiology (referral basis), referral to dentist and podiatry, RNs on staff, and registered dietician. Occupational therapy, physical therapy, and speech language pathology are provided. Social worker on staff. Vision rehabilitation and training provided. Teaches skills for daily living for people who are visually impaired, such as adaptive handling of money, Braille, food preparation (peeling, slicing), mobility training, and safe use of stairs. Recreation therapy provided; includes crafting, outings, discussion groups, exercise, special events and entertainment, religious activities, and cooking groups.
