Marjorie Doyle Rockwell Center and Eddy Alzheimer's Center at Hawthorne Ridge are the only medical model adult day services in the Capital Region designed specifically for older adults with memory loss who are still living in their own homes or in the community with caregivers. The Wilkinson Adult Day Health Center is a community-based long term care program that provides comprehensive health care services in a congregate day setting. Clients are provided with the medical and social services they need to remain home and part of the community. St. Mary's Adult Day Care is offered at The Wilkinson Residential Healthcare Facility on the Memorial campus. Adult Day Health Care is designed to meet the health care needs of chronically ill, frail elderly, and disabled adults who require certain primary, preventive, diagnostic, therapeutic, rehabilitative or palliative services.

Social day program for people with mild memory impairment. Lunch and transportation are available.

Offers a variety of social and recreational activities geared for those who have suffered a stroke or have physical disabilities. Activities include games, discussions, creative activities, holiday activities, picnics, group speech therapy or individual computer programs to improve language skills, seated physical exercise, guest speakers, field trips, dance and musical events. NOTE: Participants need to bring their own lunch, but snacks and beverages are provided.

Medical model day programs for older adults. Medical management, physical, occupational and speech therapy inpatient and outpatient. Also offers hot meals, and kosher, therapeutic recreation, respite care, personal care, and care management.

Provides a social day care program for the frail elderly, including transportation within our catchment area, hot meal, blood pressure screening, recreation, arts and crafts, music therapy, exercise therapy, trips, outings, and parties. Serves frail, elderly, at-risk or isolated persons over age of 60 with one or more physical or mental deficits who are continent, able to feed and toilet themselves and work in a group.

Designed for the frail elderly who need assistance in the activities of daily living.

FCA's SMART Van is equipped with 4 computer stations and visits senior programs and health fairs within the Town of Hempstead with the goal of bringing technology education and access to resources to older adults within the Town of Hempstead. In partnership with local Universities, students and volunteers help seniors improve their skills and feel more comfortable with using technology. The computer stations can also be used to help seniors connect with resources and entitlement programs.

Offers respite or short-term stays for families or caregivers of Alzheimer's patients who need a break or take time for a vacation or do other things.
