Provides information for those who qualify for Medical Transportation. Medicaid can pay for transportation to and from emergency medical care and services. Medicaid can also pay for transportation to and from Medicaid billable non-emergency medical care or services for a Medicaid eligible individual or reimburse that individual for the expense related to public transportation or private vehicle use. If your medical condition requires you to travel by taxi, ambulette, or ambulance, effective July 1, 2015 you must contact LogistiCare Solutions, the New York State Department of Health's transportation coordinator, at 1-844-678-1103. Individuals with both Medicare and Medicaid, who are 21 years of age or older, seeking information on various Medicaid home care programs should call New York Medicaid Choice at 1 (888) 401-MLTC (6582). Other individuals not enrolled in Medicaid Managed Care seeking information regarding non-institutional care such as personal care aide services, should call the Home Care Intake Unit at (631) 854-9585. The Independent Consumer Advocacy Network (ICAN) is an independent, conflict-free entity providing Individuals free assistance in accessing care, filing grievances, appealing adverse decisions, and understanding and exercising rights and responsibilities. There are 60 physical locations throughout NYS for in-person assistance, a call center (1-844-614-8800) for the entire state, and a website icannys.org. ICAN currently serves MLTC plan enrollees and mainstream Medicaid managed care enrollees who receive long term care. Call for detailed information.

Provides Welfare Fraud Hotline and places great emphasis on identifying fraudulent applications both prior to and during case opening. Provides case management, coordinated home care services including nursing care and management, physical therapy, occupational therapy, respiratory therapy, social work services, speech therapy, nutritional counseling, personal care, and homemaker/housekeeper services. The program can also coordinate a personal emergency response system, Meals on Wheels, respite care, adult day health care, and social day care. Visits by specialists such as podiatrists, dentists, and ophthalmologists are arranged as needed. Specialized services include: HIV/AIDS Home Care Program and Traumatic Brain Injury Waiver Services.

Covered medical services includes care provided by a primary care physician, specialists visits, emergency care, diagnostic testing and treatment, women's health services, prescription and dental care coverage and long term care services including home health services as referred by your primary care physician.

Provides services and supports to families to prevent out of home placement for their child. Enrollment in the program is voluntary and no fees are charged to the family. Staff meet with the family/youth in their home and other locations chosen by the family.
