Provides primary health care and comprehensive dental care for all ages. Primary care includes back to school physicals, sick calls, annual physicals and wellness visits, immunizations, and chronic disease management. Dental care includes exams, cleanings and deep cleanings, fillings (composite and amalgam), teeth whitening, crowns/bridges, emergency care, dentures and partials, complicated extractions, and wisdom teeth removal.

Federally Qualified Health Center (FQHC) offering pediatrics, internal medicine/family medicine, OB/GYN, pediatric and adult dental services, podiatry, social work, nutritional counseling, medications for opioid use disorder, and pharmacy with home delivery. Serves all populations with special focus on the medically under-insured, uninsured, and those experiencing homelessness. Also provides onsite lab, ultrasound, STD testing, COVID-19 testing and vaccines, health education/groups, blood pressure screening, and free walk-in pregnancy testing. Provides transportation assistance to help clients get to appointments and referral assistance. Participates in 340B drug discount program and has onsite enrollers for the New York State of Health insurance marketplace. Hospital affiliations: Sisters Hospital and the John R. Imaging services include 3D mammography (digital tomography), x-ray, bone density, MRI, CT, ultrasound, and nuclear medicine.

Provides primary and specialty care, care coordination, individual and group counseling, health education, self-management training and treatment adherence for adolescents ages 13-24 years of age. Team specializes in care for youth and young adults who are living with HIV or at risk for HIV, sexually transmitted infections (STIs), who are experiencing homelessness, mental health and/or substance use. Also provides comprehensive and supportive service for youth who identify LGBTQ.

A comprehensive health center offering primary care (assessment, treatment, and referral), dental clinic, health education, and diabetes education. Tuscarora Community Health Worker Program provides home-based case management services to participating women and their children and helps them overcome barriers to care. Workers help identify complicating factors that make it difficult for clients to access the care they need; assist in enrolling pregnant women into prenatal care and children into primary care; assist in enrolling for services such as WIC, Medicaid/PCAP and Child Health Plus; provide education on topics such as breastfeeding, baby care, lead poisoning, immunization, nutrition, HIV/AIDS, domestic violence, and substance abuse; provide referrals to food pantries, housing assistance, educational programs, childcare, and other services; and provide support and help to overcome the barriers that prevent families from receiving a healthy start in life.
