Offers classes with an evidence based curriculum (PreventT2) proven to prevent or delay the progression of prediabetes into the onset of type 2 diabetes and other serious health problems by introducing realistic lifestyle changes and goals. Prediabetes is a serious health condition where blood sugar levels are higher than normal, but not high enough yet to be diagnosed as type 2 diabetes. Approximately 84 million American adults have prediabetes. Of those with prediabetes, 90% don't know they have it. Diabetes Management Programs: Help people with diabetes to take control of their diabetes and live healthier lives. An expert diabetes team (Certified Diabetes Educator/RN, Registered Dietician, Health Educator, Pharmacist, etc.) teaches people what they need to know to feel better and help avoid complications of diabetes. Provides one-on-one visits with a diabetes care and education specialist to discuss individual needs and support. - Diabetes Self-Management Classes: For any individual with diabetes. Classes focus on the fundamentals of diabetes self-management, covering topics such as understanding diabetes, blood sugar monitoring, nutrition and exercise, acute and chronic complications, and diabetes medications. - Diabetes Support Group: For any individual with diabetes and those supporting them. Meets once every other month to share struggles, celebrate successes, and discuss topics related to diabetes management.
