Helps individuals at high risk for developing type 2 diabetes adopt and maintain healthy lifestyles by eating healthier, increasing physical activities, and losing a modest amount of weight in order to reduce their chances of developing the disease.

Offers a lifestyle modification programs for adults diagnosed as pre-diabetic and/or those considered at high risk for pre-diabetes.

Offers classes with an evidence based curriculum (PreventT2) proven to prevent or delay the progression of prediabetes into the onset of type 2 diabetes and other serious health problems by introducing realistic lifestyle changes and goals. Prediabetes is a serious health condition where blood sugar levels are higher than normal, but not high enough yet to be diagnosed as type 2 diabetes. Approximately 84 million American adults have prediabetes. Of those with prediabetes, 90% don't know they have it. Diabetes Management Programs: Help people with diabetes to take control of their diabetes and live healthier lives. An expert diabetes team (Certified Diabetes Educator/RN, Registered Dietician, Health Educator, Pharmacist, etc.) teaches people what they need to know to feel better and help avoid complications of diabetes. Classes, support groups, and one-one education available.

Classes focus on the fundamentals of diabetes self-management, covering topics such as understanding diabetes, blood sugar monitoring, nutrition and exercise, acute and chronic complications, and diabetes medications. - Diabetes Support Group: For any individual with diabetes and those supporting them. Meets once every other month to share struggles, celebrate successes, and discuss topics related to diabetes management.
