Whitney Young's HIV/AIDS Medical Case Management team provides support to primary care patients by working to reduce the barriers that make access to care problematic, such as transportation, language, etc. Medical Case Managers can explain, in more detail, aspects of HIV Care such as Viral Load, CD4 Counts, resistance, adherence, and the need to manage co-existing conditions such as substance use, diabetes, high blood pressure, and asthma. Medical case managers can assist with arranging appointments, managing HIV/AIDS entitlements, working with external medical providers, working with external social service agencies such as housing, food programs, and transportation. Medical Case Managers can also help explain HIV/AIDS to significant others, family and friends to help build natural systems of support.

Provides primary care and emergency services for veterans. Inpatient and outpatient treatment includes evaluation, treatment, medical, surgical, psychiatry, neurology and rehabilitation medicine. A full range of health screenings, health education resources, and prevention programs (weight, smoking cessation) are available.
