Provides day habilitation services to individuals ages 21 and older with moderate, severe or profound levels of intellectual developmental disabilities. Day Habilitation Program operates under the OMRDD's Home and Community Based Services (HCBS). The programs serve people with a wide range of intellectual and other developmental disabilities. Day habilitation services help individuals achieve satisfying and rewarding connections and relationships within their communities. Services and supports revolve around the assumption that each person is a member of the community. The day habilitation program uses consumer skill acquisition and staff support to help people build relationships, to participate in the community, and to experience work and volunteerism. Volunteer work assignments allow individuals to contribute to their communities as well as to enhance and strengthen skills necessary for supported employment. Social and recreational opportunities and clinical needs are also addressed. Part-time attendance is offered to individuals who are engaged in supported 28 small community based locations throughout Nassau County.

Offers a comprehensive day habilitation program for individuals with autism and other developmental disabilities to engage in recreational and volunteer activities which offers exercise, recreation, library time, volunteer work, developing and enhancing socialization, safety skills, nutrition awareness and community integration. Also provides community habilitation where trained staff go to an individuals home to work on daily activity and other necessary skills to build as much independence as possible.

Offers at home support to children and adults with developmental disabilities and autism. Provides one-on-one trained staff to assist individuals with skill development in activities of daily living, community integration and travel training.

Provides residential services e.g. community residences, intermediate care facilities, individualized residential alternatives and in-home services, day habilitation, case management, supported apartments for adults and children with developmental disabilities.

Programming is done either in the home or in the community or both, again based upon the need of the individual and the desires of the family. The individual and/or the family is involved in the hiring of staff members who then work directly with that person, whether child or adult, working on those goals that have been identified as valuable in the ISP. The amount of hours this programming entails is agreed upon during initial meetings with the MSC and the family as approved by OMRDD.

Offers day habilitation services which include enhancement of daily living activities, money management, socialization, work ethics, language and communication development, and behavioral therapy

Provides a program which combines both recreational and pre-vocational activities for adults with developmental disabilities. Participants learn how to interact within their community through volunteer work at local preschools, food pantries, and other community organizations, engage in educational activities with trips to the zoo, museums, shopping centers, etc, and physically challenging activities with trips to a gym, a park to exercise, hike and interact with others.

Provides programs for children 5 years old to young adults with special needs including learning disabilities, Aspergers Syndrome, speech and language delays and Attention Deficit Hyperactive Disorder (ADHD). This program occurs within a therapeutic setting and focuses on independent living skills, self-care, pre-vocational training and nutrition for individuals with disabilities. This program is offered in partial or full day sessions. Also offered is occupational, physical, and speech therapy.

Offers programs to middle school students ages 11-14 in grades 6th to 8th. The Prevocational Program services students with various developmental disabilities. The primary goal of the program is to maximize each student's potential and to teach skills that will enable them to become as independent as possible within the confines of his/her disability. Program components include academics with an emphasis on functional skill development, as well as simulated work activities to apply newly acquired knowledge. Behavior management, independent living, and social skills are also addressed. Where needed, discrete trial teaching and applied behavioral analysis is utilized in conjunction with grade level curriculum. Functional assessments of behavior intervention plans are implemented for support when needed. The Secondary Developmental Program services students with various developmental disabilities. The primary goal of the program is to maximize each student's potential and to teach skills that will enable them to become as independent as possible within the confines of his/her disability. Program components include improving cognition with an emphasis on sensory motor skills development. Functional assessments of behavior and behavior intervention plans are put in place for support when needed. The 12:1:4 Program emphasizes a multi-sensory approach to learning. Emphasis is placed on improving cognition, sensory motor skills development and use of adaptive equipment to aid movement and independence. Students placed in the 12:1:4 class component are dependent upon staff for meeting their needs throughout the school day. Classroom instruction is provided with hand over hand assistance. Related services of speech, occupational therapy and physical therapy are provided on a group push-in basis. All students are assessed using the New York State Alternate Assessment. The Academic Center Middle School Program services students with learning and behavioral disabilities. The primary goal of the program is to maximize each student's potential. Program components include instruction that is comprised of the general education curriculum and which supports the Common Core State Standards. Behavior management, self-advocacy and social skills are addressed. A building-wide behavior management plan is an integral part of the program. Functional assessments and behavior intervention plans are implemented for support when needed. Programs are offered at various locations.

Provides educational, social, and cultural activities on Saturdays for intellectually and/or developmentally disabled individuals. Call for further information.

Provides day programs that assist individuals with developmental disabilities, acquire, retain or improve their self-help, socialization and adaptive skills including communication, travel and other areas in adult education. Activities and environments are designed to foster the development of skills and appropriate behavior, greater independence, community inclusion, relationship building, self-advocacy and informed choice.

Offers a center-based full day program to 5 - 21 year old people with autism/PDD. The goal of the program is to help the person with autism achieve his or her full potential. Programming includes goals in communication academics, social skills, self-help skills, and the promotion of adaptive behavior.

Hudson Valley Health Specialties, Inc. and Ulster Rehabilitation Clinic: Services include primary medical care, psychiatric, dental, vision, and psychology as well as speech, occupational and physical therapies among others. Also offers specialized autism training and services.

This program focuses on personal success, developing each individual's interests, social and life skills, education, and emphasizing community inclusion. Activities are provided on-site and in a variety of outside settings that enriches the participants with new experiences and interests.

RAYIM serves the developmentally disabled with in home respite for guardians of children, in home teaching of basic living skills, day trips within the community for adults, supported group home for single adults.

Provides Independent Living Services by culturally sensitive, bi-lingual staff members who focus on issues directly related to and which impact the disabled Hispanic Community. Provides services such as peer counseling, advocacy, housing referrals, Consumer Directed Personal Assistance Program (CDPAP), limited transportation, equipment loan, etc. to the Hispanic community in Nassau County. Provides in-home assistance to persons with developmental disabilities and their families. The goal is to achieve a better match between the expectations of family members and the needs of the person with the disability. Staff work directly with the disabled and provide training to the family members to accomplish these goals.
