Offers a number of programs designed to meet the needs of children with developmental disabilities and their caregivers. Services include: - Respite Services: Offers planned short-term and time-limited opportunities for families and care providers to have some personal time. - Employment Services: Helps adults and youth to obtain and retain competitive employment through vocational counseling, employment training programs, work experience acquisition, soft skills development, rehabilitation services, and support services to help youth obtain, maintain, or advance in competitive employment or self-employment. - Children's Health Homes/Care Management: Includes care coordination, comprehensive transitional care, individual and family support, and referral to community and social support services as needed. - Community Habilitation: Assists youth living at home with a loved one or independently in the community to maintain or develop the highest level of independence possible for them. - Community Based Day Habilitation: Promotes community inclusion and integration. - Family Reimbursement Program: Covers expenses for goods and services needed to care for a loved one with a developmental disability who lives at home. - Self-Directed Services: Enables individuals to make choices about their staff, supports, and services. - Preventive Services: Provides support to help families learn the necessary skills to ensure their children remain at home and out of foster care placement. - Behavioral Health Services: Includes community habilitation, caregiver/family supports and services, pre-vocational services, and supported employment. - Project C.O.A.C.H.: Provides telephone counseling, referral, outreach, and intervention/prevention for individuals and families.

Provides the following services, with some transportation provided: advocacy, benefits/entitlements, developmentally disabled parent training, family reimbursement, guardianship, home care, home modification, independent living skills training, information and referral, parenting skills for parents of developmentally disabled children, residential habilitation, in-home and out-of-home respite care (day, evening, and overnight), service coordination/case management, support groups, and day camps.

Offers seniors with intellectual disabilities, developmental disabilities, and other secondary diagnoses such as dementia, Parkinson's disease, and traumatic brain injury an opportunity to spend time with others in a creative and social atmosphere. Activities are person-centered and paced to meet the needs of seniors. The staff is trained in identifying and addressing the changing needs of this population and assists participants in maintaining daily living skills and exercise, along with memory maintenance through trivia, reminiscing, community outreach, music, and art activities. The program's goal is to provide a retirement atmosphere while encouraging participants to stay fit and active while achieving their goals and aspirations.

Provides the following clinical services: Social Work, Occupational Therapy, Physical Therapy, Speech Language Pathology, Psychiatry, Rehabilitation Counseling, and Psychology/Evaluation/Diagnosis. Mental Health Community Residence is a 12-bed residence providing 24-hour care to people who are overcoming a serious mental health diagnosis. Supportive Apartments are available for those needing minimal supervision, up to 21 hours per week. Individual Residential Alternatives (IRA) offers 24 hour supervised living in various sites throughout Orleans and Genesee County.

Social model adult day respite program that gives caregivers a break while giving frail elders a safe, enjoyable daytime experience. Also provides a co-located day program for adults with developmental disabilities.

Services may be combined with part-time supported employment, volunteer work, "without walls" options and/or pre-vocational services.Without Walls (WOW) Westchester-small group of to 4 individuals with I/DD, engaged in planned activities in the community all day such as volunteer work, attending classes at various colleges and some engage in part-time supported employment and/or internships in the Rockland communityWestchester "After the Day" Respite Services: "After the Day" Respite services provide a strong focus on learning community life, wellness activities, and developing independence in the community. Offered two afternoons per week and transportation home is provided.

Provides day habilitation services which include enhancement of daily living activities, money management, socialization, work ethics, language and communication development, and behavioral therapy.

Provides support to individuals, 18 years of age and older with developmental disabilities transitioning from school, in developing skills for independent living and employment. A diverse variety of day programs enable people to learn new skills and gain confidence in order to pursue personal goals and reach their highest level of independence in the community. Programs are designed to assist people with intellectual and developmental disabilities through person-centered planning to become participating, contributing members of their communities. At the Brighton Park site, participants enjoy a backyard patio and gardening at an in-program greenhouse. Many also take part in making ceramics and volunteering throughout the local community.

Provides residential habilitation for individuals in their own homes. Residential habilitation may include independent living skills, meal planning, money management, personal care/hygiene, and community integration.

Day program for older adults and adults with developmental disabilities. Provides socialization, community involvement, nutritional program, transportation, recreation, and case management. Helps older adults and adults with developmental disabilities remain at home and gives caregivers time away from caregiving to help them do whatever they need to do, whether that be running errands, attending appointments, or going to work. Respite services are also offered at this location; clients can select which days and times to attend.

Customized day habilitation for individuals who have developmental disabilities ages 18 years and older.
