Offers programs to students ages 13-21 in grades 8-12. The Prevocational Program services students with various developmental disabilities. The primary goal of the program is to maximize each student's potential and to teach skills that will enable them to become as independent as possible within the confines of his/her disability. Program components include academics with an emphasis on functional skill development, as well as simulated work activities to apply newly acquired knowledge. Behavior management, independent living, and social skills are also addressed. Where needed, discrete trial teaching and applied behavioral analysis is utilized in conjunction with grade level curriculum. Functional assessments of behavior intervention plans are implemented for support when needed. Students are classified as Multiple Disabilities; Autism; Intellectual Disability; Emotional Disturbance; Speech Impairment and Other Health Impairment. The Secondary Developmental Program services students with various developmental disabilities. The primary goal of the program is to maximize each student's potential and to teach skills that will enable them to become as independent as possible within the confines of his/her disability. Program components include improving cognition with an emphasis on sensory motor skills development. Functional assessments of behavior and behavior intervention plans are put in place for support when needed. The 12:1:4 Program emphasizes a multi-sensory approach to learning. Emphasis is placed on improving cognition, sensory motor skills development and use of adaptive equipment to aid movement and independence. Students placed in the 12:1:4 class component are dependent upon staff for meeting their needs throughout the school day. Classroom instruction is provided with hand over hand assistance. Related services of speech, occupational therapy and physical therapy are provided on a group push-in basis. All students are assessed using the New York State Alternate Assessment. The District Based Program (Half day at WHBLC @ Shoreham-Wading River High School / Half day at H.B. Ward Career & Tech SCE or TSP @ BLC) services students with various developmental disabilities. The primary goal is to introduce the students to work skills that may prepare them for entry level employment or help them achieve success in their transitional plan. Functional assessments of behavior and behavior intervention plans are put in place for support when needed. Programs are offered at various locations.

Assists individuals and families to achieve the life they most desire through person-centered planning and links to a network of supports.

Provides an individualized program, based on the level of severity for individuals with developmental disabilities. Services include educational, vocational, habilitative, supportive employment, socialization, and life skills training.

Offers comprehensive services and support to people with developmental disabilities. HCBS Waiver Home services include respite, community habilitation, day habilitation, and case management. Family Care provides a residential alternative for people with developmental disabilities to live in the community.

Provides the opportunity for individuals of all ages with disabilities to experience the joy and benefits that come with playing baseball. Helps players develop social skills and increase self esteem while promoting community support and sponsorship for the league.

Offers personalized recovery oriented services including group and individual therapy, medication management, symptoms monitoring, health assessments, goal and job development, psychiatric evaluation, basic living skills, benefits management, and crises intervention.

Provides Independent Living Services by culturally sensitive, bi-lingual staff members who focus on issues directly related to and which impact the disabled Hispanic Community. Provides services such as peer counseling, advocacy, housing referrals, Consumer Directed Personal Assistance Program (CDPAP), limited transportation, equipment loan, etc. to the Hispanic community in Nassau County. Provides in-home assistance to persons with developmental disabilities and their families. The goal is to achieve a better match between the expectations of family members and the needs of the person with the disability. Staff work directly with the disabled and provide training to the family members to accomplish these goals.

Provides opportunities to assist individuals in becoming more independent.

Assists men and women with intellectual and developmental disabilities live in their communities by providing residential services in 11 family-like residences with five to twelve residents. Provides long-term living arrangements that have the necessary supervision and structure including instruction in everyday tasks like doing laundry and cooking, coping with upsetting situations, conflicts with friends and family, and planning leisure time and activities. Provides daytime activities for individuals who are developmentally disabled. Individuals need not be visually impaired. Individuals each have a person-centered plan of activities designed to make them more independent, and community volunteer experiences.

Offers hourly Community Habilitation for families of children/adults with autism and other developmental disabilities. Caregivers and families can also engage with their peers and participate in skill-building activities.

Offers eligible individuals support both in their own home, as well as the community. They are offered to enhance the individual’s daily activity through structured, individualized support. Provided under the guidance of the NYS Department of Health.

Provides day habilitation services that are in an environment which recognizes and builds on each person's talents, interests, expectations and feelings including health and nutrition, speech, self-care, and social skills, physical and occupational therapy, psychological services, and independent living and pre-vocational skills in order to help the participant increase their independence.
