Provides emergency assistance/referral services for needs that if not taken care of will affect the immediate health and safety of someone. Cases are reviewed on an individual basis, no monies for rental security deposits provided. Monetary assistance is limited with all other resources having been exhausted. Proof of any monetary need must be provided.

* Provides a medical equipment loan closet that offers: wheelchairs, portable ramps, walkers, crutches, canes, bath items, quad canes, commodes, and raised toilet seats. All are available on a first come first served basis. Extendable three month limit on equipment loans.

A community-based agency run by and for individuals with disabilities that provides specialized information and referral, independent living skills training, individual and systems advocacy, peer counseling, medical equipment loan program, and transportation. Also offers assistance finding employment and housing and applying for social security benefits.

* Provides pharmacy services, medical equipment and supplies and prescription delivery service.

* Provides full service durable medical equipment products and services for a variety of medical needs. Lifts and ramps for car, home, etc. Services include delivery.* Offers home mobility/accessibility evaluations for patients needs.

* Offers volunteer ambulance and rescue services and acts as a 911 responder.* Offers trainings in various first aid programs including Cardiopulmonary Resuscitation (CPR) and Automated Defibrillation (AED).* Provides Heart Saver public courses for adults, children, infants and follows the American Heart Association curriculum.* Provides blood pressure checks.* Provides light duty rescue for motor vehicle accidents.* Offers defensive driving for emergency vehicles.* Provides medical equipment loans such as: Crutches. Canes. Wheelchairs. Walkers.

* Provides peer support for people with disabilities.* Offers an employment program to help people with disabilities increase their employability.* Offers benefits advisement by providing information and advocacy while navigating government health and human service systems.* Offers healthcare advocacy by providing information and assistance with regard to Medicaid, Medicare, Medicaid Managed Care, and Medicaid Funded Services.* Works in conjunction with NY Connects in Monroe, Ontario, Yates, and Wayne County, to inform, assist, and advocate for individuals, caregivers, and family members, seeking community-based long-term services and supports.* Provides housing advocacy by offering information and advocacy on affordable and/or subsidized housing, rental listings, accessibility, home renovations, and home ownership.* Offers peer support groups and services for persons with disabilities.* Provides referrals for equipment loan.* Provides education, assistance to and advocacy for disabled persons wishing to vote.* Provides independent living skills to assist with daily living activities, such as; money and medication management, nutrition, legal rights, socialization, and mobility.* Provides community outreach and education through workshops, classes and other activities to promote awareness of needs of persons with disabilities, and understanding of independent living.

* Provides volunteer ambulance services to the community. * Hosts trainings and community events, including First Aid, CPR, Automatic External Defibrillator (AED) and Emergency Medical Technician (EMT) training, community blood pressure clinics, first aid standby at community events. * Maintains a loan cupboard for the use of walkers, canes and crutches to those in need.
