Coverage is provided by managed care plans and includes physician services, inpatient and out-patient hospital care, prescription drugs, and smoking cessation products, lab tests and x-rays, vision, speech and hearing services, rehabilitation services, limited durable medical equipment, emergency room and emergency ambulance services, drug, alcohol, and mental health treatment (limited), diabetic supplies and equipment, radiation therapy, chemotherapy, hemodialysis, and dental services (if offered through plan). Annual recertification by mail or phone.

When funding is available, provides rent and utility assistance in emergency situations under specific circumstances; clients should reach out and share their circumstances to learn whether they'd be eligible. Also offers a health clinic which provides the following: - Telehealth services - Health coach services, including health referrals and guidance on health insurance - Visits from Oak Orchard Health's mobile health unit, which provides lab services and a nurse practitioner

The purpose of this program is to identify people in nursing homes who wish to transition back into the community, explain programs and services available to them, and make the appropriate referrals to begin the process.After their transition, participants are offered Person Centered Coaching for 6-8 skill building sessions to help identify strengths, preferences, needs, and desired outcomes. Open Doors also works closely with the Olmstead Housing Subsidy (OHS) Program, which provides housing assistance, furniture, and household goods to those who qualify.

Certified counselors provide free, accurate, and objective information, counseling, assistance and advocacy on Medicare, private health insurance, Medicaid and related health coverage plans including Part D prescription drug plans.
