Individuals with both Medicare and Medicaid, who are 21 years of age or older, seeking information on various Medicaid home care programs should call New York Medicaid Choice at 1 (888) 401-MLTC (6582). Other individuals not enrolled in Medicaid Managed Care seeking information regarding non-institutional care such as personal care aide services, should call the Home Care Intake Unit at (631) 854-9585. The Independent Consumer Advocacy Network (ICAN) is an independent, conflict-free entity providing Individuals free assistance in accessing care, filing grievances, appealing adverse decisions, and understanding and exercising rights and responsibilities. There are 60 physical locations throughout NYS for in-person assistance, a call center (1-844-614-8800) for the entire state, and a website icannys.org. ICAN currently serves MLTC plan enrollees and mainstream Medicaid managed care enrollees who receive long term care. Call for detailed information.

Provides an organized marketplace developed by New York State to help people shop for and enroll in health insurance coverage. Individuals, families and small businesses can use the Marketplace to help them compare insurance options, calculate costs and select coverage online, in-person or over the phone. The Marketplace helps people to check their eligibility for health care programs like the Essential Plan, Medicaid and Child Health Plus and sign up for these programs if they are eligible. The Marketplace also tells what type of financial assistance is available to applicants to help them afford health insurance purchased through the Marketplace.

Our EASE (Enrollment Assistance Services and Education) program assists individuals, families and small business owners enroll in health insurance coverage through the NY State of Health Marketplace.

Offers at-risk Long Island residents in targeted Nassau and Suffolk County communities a variety of outreach services including FREE rapid HIV/HCV testing and counseling, fast tracking into substance abuse treatment, Comprehensive Risk Counseling Services (CRCS), distribution of condoms and referrals to primary health care, mental health, case management and other supportive services. The program also offers on the spot education about reducing the risk of HIV/AIDS, Hepatitis and other STD transmissions and the link between substance abuse and HIV. Senior Benefits Information Centers (SBICs) help adults 60 and older, and those caring for the elderly, achieve a better understanding of Medicare health plans and prescription drug coverage (Part D prescription drug coverage, Medicare Advantage, Medic-Gap). Information and counseling is provided about different types of government programs, including federal and state financial assistance programs, and county-wide benefits. You can meet one-on-one with a trained counselor at one of our library-based locations. Spanish interpreter can be made available by appointment.

The Dutchess County Office for the Aging offers a wide variety of services to meet the nutritional needs of persons 60 years and older. Services include congregate meals at OFA Friendship Centers, Home Delivered Meals for homebound older adults, nutrition education and counseling, picnics during the summer, and year-round health promotion events.

Provides trained insurance staff to assist with Medicare explanations, claims and appeals, and offers information on Medicare plans, supplemental policies and drug plans, long term care insurance and other entitlements and benefits for seniors.

Provides food and clothing pantry services. Individuals can receive food once monthly. Will deliver food in an emergency. Serves as a surplus food distribution site. Lunch served on weekdays by Maudie Mae's Community Kitchen. Free Thanksgiving and Christmas meals provided, along with a Breakfast With Santa. Holiday baskets also available for residents of Eastern Orleans County, including the Village of Holley, Town of Clarendon, and Town of Murray. Provides family development/case management services. Offers public access computers with internet and senior activities including bingo and crafts. When funding is available, provides rent and utility assistance in emergency situations under specific circumstances; clients should reach out and share their circumstances to learn whether they'd be eligible. Also offers a health clinic which provides the following: - Telehealth services - Health coach services, including health referrals and guidance on health insurance - Visits from Oak Orchard Health's mobile health unit, which provides lab services and a nurse practitioner

Administers Medicaid program to assist eligible individuals in our community in obtaining necessary and appropriate medical care through programs such as Medicaid, Child Health Plus, Medicaid Social Security Income (SSI), Personal Care Supports including nursing and personal care aids, managed care insurance, and long-term care supports, among others. Child/Teen Health Program is a preventive health program which provides continuing medical care to your children. Also provides for burial when a Temporary Assistance recipient or other indigent person dies leaving no funds or insurance sufficient to pay the cost and there are no relatives, friends or other persons liable or willing to take responsibility for the burial expense. Office serves as a Cooling Center during times of excessive heat. Always call ahead to make sure a Cooling Center is open before going there.

Provides consultation and representation to older adults on elder abuse, Medicaid, Medicare, Social Security, consumer scams, public benefits programs, powers of attorney, health care proxies, landlord/tenant problems, and other areas of civil law. Hours available by appointment.

Provides services to adults and children with developmental disabilities who waive the right to live in an institution and choose to live at home with their families. Case management services are provided to clients who are on Medicaid and also to those who are not Medicaid eligible.

Provides the information to educate older adults and their families about Medicare, Medicaid, Managed Care, EPIC, and other health insurance options and issues. Also help older adults apply/enroll for prescription drug discounts, Medicare plans, etc. and assists Medicare recipients to access needed health care services.

The Health Insurance Information Counseling and Assistance program (HIICAP) is your source for free, current, and impartial information, counseling and advocacy services concerning health care coverage for seniors. Through our hotline and our volunteer counselors, we can provide you or family members with Medicare information on all aspects of health insurance including Medicare Parts A, B, C and D and EPIC.

All Temporary Assistance, Medicaid-only and SSI-Medicaid clients, may choose from five managed care plans to receive the complete menu of Medicaid Benefits. There are no costs, no caps, and no co-payments. Each member has his/her own physician or pediatrician-OB/GYN, who will coordinate all their health needs. Provides one-on-one assistance for individuals interested in shopping for and enrolling in health insurance through The New York State of Health – the Official Health Plan Marketplace. Navigators are now available to speak with individuals and families to provide education about the different types of health insurance plans offered through the New York State of Health (NYSoH) Marketplace and to assist with enrollment into public insurance programs and qualified health plans.
