Provides services and supports to families to prevent out of home placement for their child. Enrollment in the program is voluntary and no fees are charged to the family. Staff meet with the family/youth in their home and other locations chosen by the family.

Transportation may be available depending upon placement; not always available. Vocational Assessment Services: Services include vocational evaluations, career counseling, pre-employment testing, and linkage. Driver's Rehabilitation: Services include training, clinical evaluations, and vehicle modification evaluations, available for individuals with severe limitations.

Provides evaluation and training of individuals with disabilities to help them obtain their NYS Driver's License, along with permit tutoring, adaptive equipment prescriptions as needed, vehicle/equipment assessments, and behind-the-wheel assessment and training.
