Provides free primary care and osteopathic manipulative medicine (OMM) services to individuals 18 and older, with or without insurance, who might not be able to afford care. These services are provided regardless of the patient’s immigration or health insurance status. Services include: General health check; Sick visits; Blood pressure tests; Electrocardiogram (EKG); In-house lab tests: Urine, throat culture, blood glucose check (fingerstick) and Discounted blood tests (outside lab fees apply). Provides a model of service delivery that ensures comprehensive care to adolescents and improve the overall physical and emotional health of students. The availability of on-site healthcare reduces absenteeism and encourages adolescents to take responsibility for their own well-being. Our School-Based Health Center (SBHC) is licensed health care providers, who collaborate with schools to address the broad range of concerns and adverse experiences that affect students’ healthy development. They work in conjunction with school nurses to provide expanded medical and behavioral health services to students during the school day. At the School-Based Health Center, your child can receive the services at no cost to you, regardless of insurance status. The SBHC is allowed to bill insurance, however there are no co-pays for you, and you do not receive a bill. There is no requirement to transfer care from your primary care provider (PCP). School-Based Health Center Services includes: Complete physical examinations; Medications and prescriptions; Medical laboratory tests; Vaccines and Routine Immunizations; Medical care, including treatment for urgent and chronic conditions; Screening for vision, hearing, asthma, obesity, and other medical conditions; Health Education and Counseling and Mental Health Counseling and Services. Our care team works with other providers, including specialists and hospitals, to effectively coordinate your care. We work with you to obtain a complete medical history, and we ask that you let us know about care you receive outside the practice. We may ask you about medications you are taking, your family medical history, as well as any recent hospital visits. We strive to communicate with you effectively and provide you with an appointment as quickly as possible. We use the latest medical research and evidence to guide our decisions as together we discuss options for your care. We provide you with tools such as log books or reminder calendars to help you manage your care at home. We are committed to establishing a relationship with you and providing you with the services and tools you need to maintain your health. Medicaid, Medicare and most commercial insurances are accepted. For those without insurance, enrollment counselors are available to assist you. A sliding-fee scale is available for those who are ineligible for insurance. Co-payments and deductibles are due at the time of service. Services include: Management of chronic conditions; Referrals to specialty services; Coordination of care; Routine annual checkups; Pre-employment physicals; Labwork; CDL, DSS physicals and Pre-op evaluations.
