EARLY INTERVENTION PROGRAM | SUFFOLK COUNTY DEPARTMENT OF HEALTH SERVICES - DIVISION OF SERVICES FOR CHILDREN WITH SPECIAL NEEDS

Provides services to children from birth to three years of age with developmental delays or have a diagnosed disability, who are determined eligible for service based upon evaluations. Services include home visits for speech, physical and other therapies, child development groups, and family training. Call for detailed information.

CHILD FIND PROGRAM | MONTGOMERY COUNTY PUBLIC HEALTH DEPARTMENT

Child Find is a statewide program to assure that children will receive the help needed for the best growth and development in the early years. Assistance is available to families of children under 3 years of age who are at high risk for health problems, delays or disabilities. Child Find works as a partnership with parents, focusing on the best possible outcome for the child.

School | Keuka Lake School (KLS)

Offers a center-based and home-based pre-school for children with and without disabilities which emphasizes social, emotional, and academic growth in a supportive, safe environment. Also provides early intervention services for children with developmental disabilities and their families including evaluation and services in all areas of development including special education, psychological services, speech, language, occupational and physical therapy, nursing services, and family support and advocacy services. Early Head Start and Head Start is provided in collaboration with Pro Action of Steuben and Yates.

Children With Special Needs Preschool Program - Cattaraugus County Youth Bureau

Provides referrals to the appropriate school district's Committee on Preschool Special Education (CSPE) for evaluation to determine the needs of preschoolers with special needs. Offers information regarding services that may be available to children with disabilities ages 3-5. All children referred to this program are mandated by the New York State Education Department (NYSED) to have three evaluations before service funding is approved. Required evaluations are a physical examination, a psychological evaluation, and a social history. Additional evaluations may be done in the area of concern (e.g. speech, gross motor, audiological, etc.).

Oakfield Alabama Committee on Preschool Special Education

A decision-making committee to determine eligibility and the appropriate level of services for preschool children aged 3-5. The CPSE is a multidisciplinary team established to conduct meetings to develop, review, or revise the Individual Education Program (IEP) of a student with a disability. The parent or legal guardian is included as a member of the committee.

THE CHILDREN'S LEARNING CENTER | CEREBRAL PALSY ASSOCIATION OF NASSAU COUNTY

Provides educational services to assist children with disabilities including: infants, preschoolers, adolescents and young adults needing early intervention, preschool and schoolage educational and therapeutic services, so that students are physically and academically prepared for maximum independence, and when appropriate, participate in their own community schools. Programs include: The Early Intervention Program (for children birth to age 3), The Pre-School Division (ages3-5), Ungraded Kindergarten through Grade 2 (ages 5-7), and The Schoolage Program (ages 5-21). Full diagnostic evaluation including educational, therapy and psychological testing available.

Provides information, referral, assessments and evaluations for children with suspected delays in physical, cognitive, language or emotional development. Services also provided in case management, parent/family training, support groups, early intervention programs, preschool services, school age services and evaluations, clinical services for children 0-21 years old. PRESCHOOL EVALUATIONS AND THERAPY | KIDS FIRST EVALUATION AND ADVOCACY CENTER - NASSAU

Conducts evaluations of preschool children suspected of having developmental delays. Evaluations are done at the child's home and in their language. Services such as physical, occupational and speech therapy are provided to qualifying children.

Provides early intervention classroom- based programs for children birth-age 3 with a wide variety of disablilities. Parent training is available to show parents how to dress, feed, play with and stimulate their child's growth and development. Teachers give instruction in the classroom. Physical, speech and/or occupational therapy is also available.
