A family-driven service in which caregivers partner with a Family Peer Advocate (FPA) who has lived experience helping parents navigate systems more effectively, learn from the experiences of other families, decrease isolation, and gain hope, ideas, and information. Families can enroll in one or all of the following service components to best meet their needs: - Individual Mentoring & Skills Development: Can be delivered in the home, community and/or office. A Family Peer Advocate provides mentoring, skills development, support, and information on community resources and linkage to other services. - Empowerment Groups: These services are support groups offering community resources, parent connections, and skills development. This service gives parents/caregivers an opportunity to connect and build natural community supports with other parents/caregivers with similar challenges. - Family Advocacy: This service is short term and provides instruction and support around advocacy in areas such as School and Committee of Special Education (CSE), court (preparedness and support), and systems navigation (working with providers in various systems of care). - Parent Education: Offers a variety of trainings and parenting classes throughout the year. Also offers GLOW Youth Peer Support, in which youth partner with a Youth Peer Advocate (YPA) who has lived experience navigating systems of care. The YPA supports youth in their resiliency, recovery, and wellness efforts by providing assistance in the following areas: - Skill Building - Coaching: Enhancing resiliency/recovery oriented attitudes (i.e., hope, confidence, and self-efficacy) - Engagement, Bridging, and Transition Support - Self-Advocacy, Self-Efficacy, & Empowerment - Community Connections & Natural Supports

Provides coordination of services through linkage and referral to supports and services for children 3+ yrs through adults with a diagnosis confirmed by DDRO (Developmental Disabilities Regional Office) to allow them to live as independently and productively as possible.

Program offers services, assistance, information, advocacy and referral for those needing help navigating special education services, community habilitation, pre-vocational and employment programs, independent living skills and peer-to-peer support.

Provides support to families of children who are struggling in school and may or may not have a diagnosed disability. Individualized consultation, support, and coaching are provided to help the parent/guardian navigate the school system in order to create positive outcomes for children.

Provides case management and educational advocacy support for children with disabilities and their families. Provides Pre-Employment Transition Services to school age youth between the ages of 14-21. Provides screenings, assessments, ancillary services (PT, OT, speech therapy), respite, and health and mental health clinic services.

Families Together collaborates with families, schools, policymakers and service providers to ensure families receive the services needed for their children. An annual legislative luncheon during session allows families to have the opportunity to voice their concerns and ideas with policymakers. The goal is to ensure that all children and youth have the support they need in order to succeed. The board and staff are made up primarily of family members and youth representing thousands of families from across the state whose children have been involved in many systems including mental health, substance abuse, special education, juvenile justice, and foster care.

Provides workshops to parents and care takers who are looking for guidance and legal advice involved with Special Needs.Promotes inclusion and provides anti-bullying presentations to schools. Call for further information

Provides information, referral, and advocacy for children birth to 21 years who have, or are suspected of having a serious or chronic physical, developmental, behavioral, and/or emotional condition and who require health or related services of a type or amount beyond that typically required of children. Provides case management services for families to advocate with health care providers, obtain information on programs available, advocate with school districts for related or special services, make referrals for special health care or related services, and obtain family support services.

Provides benefits advisement for disabled youth under the age of 21 years regardless of insurance coverage. Also provides advocacy to youth and their families in school during IEP and CSE meetings.

Provides advocacy training for parents of children with disabilities on any school related topic.

Provides ongoing communication with parents of students enrolled in Center's programs to monitor child's progress. Assigns specially trained staff to Committee on Special Education and to parent/guardian meetings at parents' request.

Hosts an annual conference for families and service providers, offering trainings and educational workshops. Attends various conferences and other events exhibiting and distributing information.

Offers assistance accessing programs, benefits and community resources. Provides ongoing assessment, coordination, and advocacy for all needs, including hospital discharge, and educational services.

Provides ongoing communication with parents of students enrolled in the center's program in order to monitor a child's progress. Assigns specially trained staff to committees on special education and parent/guardian meetings at the parent's request.

Provides mediation and arbitration services for community, parent/child, divorce, custody and visitation, and juvenile issues. Mediators are trained volunteers.

Provides legal assistance to individuals with disabilities with a focus on access to disability-related services and programs, and on the educational rights of children.

Legal services for families and children in federal and state courts in these areas: special education school disciplinary and suspension hearings emancipation child custody and support, adoption, guardianship divorce

Advocates are able to support families through the special education process through a varieties of venues including email, phone, face to face, letter writing, assistance at school meetings and reviewing records.
