SAIL provides information and assistance concerning long term care planning for individuals with disabilities, family members, and other interested parties. Information and assistance is provided for establishing Health Care Proxies, Living Wills, Supplemental Needs Trusts, and accessing long term care services. SAIL provides information and assistance to persons with disabilities, family members, and other interested parties concerning Medicare, Medicaid, and long-term care insurance. The center also provides insurance counseling and advisement for choosing the most appropriate coverage, as well as information concerning co-insurance and gap policies.

Assists individuals diagnosed with AIDS/HIV who need help in planning for their families future.
Assists individuals who are diagnosed with serious mental illness, HIV/AIDS, and/or two chronic conditions manage their behavioral and medical health. Provides referrals and linkages to community resources, assists in applying for benefits and entitlements, and works to develop a care plan to address and accomplish set goals. Referral and appointment required.

Provides assistance with organization/preparation of documents including Advanced Directive, Health Care Proxies and negotiates with Medicare, Medicaid,insures and health providers. Coordinates home based health services, procurement of assistive devices and technological services. Offers client/family education and advocacy for the elderly and LGBT communities.
