SAIL provides information and assistance concerning long term care planning for individuals with disabilities, family members, and other interested parties. Information and assistance is provided for establishing Health Care Proxies, Living Wills, Supplemental Needs Trusts, and accessing long term care services. SAIL provides information and assistance to persons with disabilities, family members, and other interested parties concerning Medicare, Medicaid, and long-term care insurance. The center also provides insurance counseling and advisement for choosing the most appropriate coverage, as well as information concerning co-insurance and gap policies.

Assists individuals diagnosed with AIDS/HIV who need help in planning for their families future.

Assists individuals who are diagnosed with serious mental illness, HIV/AIDS, and/or two chronic conditions manage their behavioral and medical health. Provides referrals and linkages to community resources, assists in applying for benefits and entitlements, and works to develop a care plan to address and accomplish set goals. Referral and appointment required.

Offers advance care planning and geriatric care management for all ages groups including elders, the disabled, LGBT (Lesbian, Gay, Bisexual, Transgender) community, young families and individuals recently widowed or divorced. Reviews family and community supports, performs thorough on-site assessment of physical and safety needs making referrals to appropriate network and local agencies. Provides assistance with organization/preparation of documents including Advanced Directive, Health Care Proxies and negotiates with Medicare, Medicaid,insures and health providers. Coordinates home based health services, procurement of assistive devices and technological services. Offers client/family education and advocacy for the elderly and LGBT communities.

Provides information and referrals, life planning assistance and transitional planning awareness to people and their families with Developmental Disabilities.

Provides lifetime advocacy, support and oversight when a family member is no longer available.

Provides legal aid assistance to low-income individuals in civil cases ONLY, regarding housing, eviction, utility shut-offs, foreclosure defense, public benefits including public assistance, SNAP/food stamps, Medicaid, Social Security and SSI, unemployment insurance benefit, wills, power or attorney/health care proxies.

Provides free advice and information about small estates, 17-A Guardianship, Power of Attorney, wills, and other trust and estate issues. May be able to provide on-the-spot assistance with filling out petitions for Surrogate's Court.

Offers mediation for older adults and their families, regarding issues which can be relevant to aging, such as life support, inheritance, last will in testament, health care, hospice care, etc.

Offers a variety of information for individuals wanting to know more or needing help filling out forms about living wills or health care proxy issues. Forms are available in English and Spanish.

Assists families of individuals with disabilities to plan for the future by creating an individualized life plan and recording important personal (unique, vital and irreplaceable) information. Provides support services such as direct care, guidance and advocacy.

Lawyers provide free civil (non-criminal) advice, counseling, advocacy, and representation to low-income individual clients/groups.
