Services include advocacy and assistance with Social Security applications and appeals, and applications and appeals concerning State and local resources (including Personal Care services, home health aide services, in-home nursing, homemaker services, public assistance, food stamps, early Intervention services, Family Health Plus, Medicaid, transportation, Medicaid waivers, etc.). Where necessary, assistance and advocacy is provided for informal appeals, fair hearings, and administrative law reviews. In addition, advocacy and assistance is provided for children of school age concerning the CSE process and meetings, as well as for issues involving IDEA, FAPE, transition planning, Work-Study, and other school-related services. Assistance and advocacy are also provided for individuals who choose to access services through VESID, CBVH, OMRDD, OMH, SOFA, DOT, Hospice, and other state and local agencies.

Provides civil legal services in the following areas: - Family Law: Assists with domestic violence cases, as well as with child support and divorce cases on a limited basis. - Housing: * Until further notice, the Housing Unit can only provide assistance with ERAP application issues and illegal lockout emergencies (ex: landlord changed locks, stopped a utility, etc. without going to court); cannot assist with any other housing-related issues at this time.* Typically assists with evictions, housing conditions, Housing Choice Voucher (Section 8) issues, public and subsidized housing applications, terminations, illegal lockouts, landlord/tenant matters, and other issues. - Disability: Assists with SSI Appeals, SSI/SSD applications, advocacy for disabled individuals, benefits counseling, and funding of medical equipment. - Public Benefits: Assists with public assistance, food stamps/SNAP, Medicaid, denial of emergency rental and/or utility assistance, and homeless benefits. - Consumer Law: Assists with unfair debt collection practices and wage garnishment. Does not handle criminal cases, traffic tickets, money damage actions, estate work and real estate closings, torts, small claims court cases, worker's compensation cases, or unemployment.

Provides time limited care coordination services in the home and community to keep older adults with mental health diagnoses independent in their homes through linkage, referral, advocacy, and intervention with formal and informal supports. Care Coordinators and Nurses provide an in-depth assessment leading to the creation of an individualized action plan that empowers the older adult to remain successful in the environment they deem most comfortable. Potential services include but are not limited to: engagement, linkage, referral and advocacy; medication insight and management; navigation and assistance in obtaining entitlements; support and guidance for family and caregivers; workshops and programming to increase skills and empower the individual and their support network.
