The Senior Outreach Program helps to meet the diverse needs of the aging population, sixty and over. Senior Outreach Workers provide information about programs, benefits, and services that may be available to seniors in our community. Our Senior Outreach Workers are also available to help fill out paperwork or applications for programs. They serve as advocates when seniors are having problems with social security, food stamps, Medicaid, health insurance, and more.

Provides supportive services for people returning from jail or prison who have chronic health problems, such as AIDS/HIV, addiction or other infectious diseases. Offers health education, case management, on-site medical testing, and offering assistance in finding other benefits, housing and health care.

Assists families by providing one-on-one education and support, assessing patient and caregiver needs and helping secure services at home.

Advocates on behalf of all veterans, advises Supervisor and Town Council on veterans' affairs and issues, promotes military observances and holidays to the general public, and educates the community regarding significant contributions by veterans to America's freedom.

Senior Benefits Information Centers (SBICs) help adults 60 and older, and those caring for the elderly, achieve a better understanding of Medicare health plans and prescription drug coverage (Part D prescription drug coverage, Medicare Advantage, Medic-Gap). Information and counseling is provided about different types of government programs, including federal and state financial assistance programs, and county-wide benefits. You can meet one-on-one with a trained counselor at one of our library-based locations. Spanish interpreter can be made available by appointment.

Offers seniors help with filling out forms, advocacy services, information and referral for a multitude of issues.

Provides information and referrals for eligible veterans and their families about benefit programs, education, employment, financial assistance, disability-related issues and other resources. Also helps with insurance, disability, and pension benefits.

Informs and assist members of the armed forces, veterans, their dependents and survivors in relation to (1) matters pertaining to education, training and retraining services and facilities, (2) health, medical and rehabilitation services and facilities, (3) provisions of federal, state and local laws and regulations affording special rights and privileges to members of the armed forces, veterans and their families, (4) employment and re-employment services, and other matters of similar related or appropriate nature. Provides a comprehensive array of support services for individuals and families in need. Services include emergency and other financial assistance, counseling, benefits assistance, addiction support, case management, employment referrals, holiday programs and more.

Provides a service, in collaboration with 2 other agencies (My Sisters' Place and Westchester Hispanic Coalition) to offer free legal services for poverty level residents or those who are victims of domestic violence for consumer disputes, housing, education, benefits, domestic violence, orders of protection, custody, child support, family law, including citizenship applications, work authorizations or other immigration issues.

Recognized by Board of Immigration Appeals (BIA) to administer accurate and current immigration information and assistance.ACA provides help with the following: Petition for alien relatives Fiance petitions Permanent resident (Green Card) issues and re-entry permits Foreign pension assistance Temporary Protected Status (TPS) Asylum cases DACA Free citizenship classes Naturatlization assistance VAWA - Violence Against Women Act

* Ensures all involved in an individual's care are working together and sharing the necessary information to support recovery: medical, mental health, substance abuse services, and social service needs.

Provides information and assistance to veterans, family members, and active military personnel in obtaining records, benefits, and entitlements from the Department of Veteran's Affairs.

Assists local residents with information on town government issues and services, including government-based benefits, with necessary information, filing, and guidance.

Offers peer support, benefits advisement and linkages to community resources for individuals with mental illness.

Works with individuals, community organization and government networks to promote inclusion of people with disabilities, heighten awareness, and understanding of disability-related issues and improve the implementation of existing laws that affect people with disabilities, such as advocating for parents with school-related issues, or interpreters for those with hearing impairments.

Assists veterans seeking eligible benefits from the US Veterans Administration (VA). An attorney will help determine eligibility and assists with and submits necessary applications and pursues necessary appeals.

Angels for Warriors connects veterans with the resources they have earned from their service to our country. We assist Veteran's transition to permanent housing, register them for veteran benefits and advocate on the veteran's behalf for medical care.

Provides outreach, crisis intervention, housing counseling, benefits advocacy, tenant/landlord mediation, representation in administration proceedings with government agencies, home ownership counseling and translation.

Helps seniors fill out forms such as H.E.A.P. (Heating Energy Assistance Program), and Senior Tax Exemptions.

Provides benefits assistance for seniors by answering questions, filling out applications, giving information, explaining tax or other government-related services, etc. or offers referrals for other resources for other services they may need.
