Provides time limited care coordination services in the home and community to keep older adults with mental health diagnoses independent in their homes through linkage, referral, advocacy, and intervention with formal and informal supports. Care Coordinators and Nurses provide an in-depth assessment leading to the creation of an individualized action plan that empowers the older adult to remain successful in the environment they deem most comfortable. Potential services include but are not limited to: engagement, linkage, referral and advocacy; medication insight and management; navigation and assistance in obtaining entitlements; support and guidance for family and caregivers; workshops and programming to increase skills and empower the individual and their support network.

Provides free information, referrals, advice, and representation to persons who have received utility disconnect notices, whose utility has already been disconnected, who have been denied Emergency Utility Assistance or HEAP through ECDSS, or who have another utility related issue. If denied, applicants should get their denial in writing, then call the Utility Disconnection Hotline for possible representation. The applicant has 60 days from the date of the denial to request a Fair Hearing.

Provides assistance to New Yorkers with disabilities who are receiving or applying for vocational rehabilitation (VR) services from New York State VR agencies or from Tribal VR programs. The state agencies are the New York State Adult Career and Continuing Education Services-Vocational Rehabilitation (ACCES-VR) and the New York State Commission for the Blind (NYSCB). CAP also assists applicants and recipients of services from federally funded independent living centers (ILCs).

Provides outreach to Veterans identified as at risk of suicide, provides a baseline mental health assessment to potential participants in the program, and provides the following services for eligible participants: - Coordination of emergency treatment for participants, including coverage of any initial monetary charges for treatment, as long as the participant receives ongoing clinical services through the Department of Veterans Affairs (VA) - Case management with a holistic, individualized approach - Peer support - Assistance in obtaining VA benefits, including but not limited to vocational and rehabilitation services, employment and training services, educational assistance, housing assistance, and health care (including mental health care) services - Assistance in obtaining and coordinating with other public benefits for crisis-related needs, including health care services, daily living services, legal services, personal financial planning, credit counseling, and employment and vocational services - Eligible temporary income support assistance - Transportation assistance in the form of public transportation tokens - Childcare payment assistance for children under the age of 13 - General suicide prevention stability assistance, which can assist participants with funds to cover anything that may be contributing to a suicidal factor specific to a participant - Other suicide prevention services that are necessary for improving mental health such as adaptive sports, equine therapy, indoor or outdoor recreational therapy, substance use reduction programming, and individual, group, or family counseling All financial assistance is provided directly to third parties for services to veterans, and funds do not go directly to veterans themselves. ##Veteran##
