Provides following services for people living within boundaries of the parish: advocacy, information and referral, home visitation, telephone reassurance, clothing, emergency food, and limited transportation to medical services. Also brings Communion to the homebound.

Works to achieve full recognition of the civil rights of lesbian, gay, bisexual, transgendered and people with HIV or AIDS through impact litigation, education and public policy work. Pursues litigation in areas such as: housing, public accommodations, military, public policy, parenting and relationship issues, equal marriage rights, equal employment and domestic partnership benefits, and immigration issues.

Provides the following services: advocacy, information and referral, food pantry, limited emergency funds, bus tokens for transportation, employment programs, and companionship. Also offers bereavement counseling.

Offers free legal advice and direct representation to veterans in civil (non-criminal)legal matters, such as housing issues, eviction, foreclosure prevention, denied benefits, consumer protection, child support assistance, etc.

An outreach program that targets communities who would like ESL classes to help improve health literacy.

Provides benefits and entitlement counseling, long term care planning, information and referral services, and application assistance and advocacy. Community outreach and education regarding various programs and services are also provided and are available to community groups upon request. Suffolk County Office for the Aging provides a Senior Advocate to assist seniors with applications for various entitlements and benefits. Will offer referrals to needed services in the community, help with claims against employers who discriminate and/or do not pay, help file police reports if they are robbed or assaulted, etc.

Offers information and referrals to resources for older adults within the community, based on the needs of the individual.

Assists claimants who are unable to work due to work-related injury or disease in obtaining medical benefits and partial income. Staff includes claims, rehabilitation and social services.

Provides a venue for older adults with social and recreational activities, transportation for shopping, and offers information and referrals for local resources and services in the community.

Offers advocacy to individuals and families with benefits applications, rent or eviction issues, find resources or services and refers people to appropriate agencies for the specific need.

Advocates for individuals with disabilities for specialized needs or other services, helps with complaints regarding ADA compliance, or discrimination, etc, and refer to agencies which specialize in specific areas of need.

Offers referral for support groups for victims of domestic violence and child support issues, conducts legal awareness seminars, and provides advocacy, information and referral, and assistance with entitlement.

Provides intensive, short term, home-based services to families in order to prevent the psychiatric hospitalization of a child or adolescent. The program is devoted to working with the entire family involved in a crisis situation.

Provides crime victim advocacy and support services for all victims of hate crimes as well as hate crime prevention programs for adult and mature teen audiences.

Provides residents of adult homes with legal and non legal advocacy services as well as training in issues which include the rights of adult home residents, rights of persons receiving services from mental health agencies and self-advocacy.

Services provided on a case-by-case basis after assessment. Emergency assistance may include information, advocacy, referrals, emergency food, personal care toiletries, diapers (through Every Bottom Covered), clothing, household items, prescription medications, budgeting assistance, and utility payment assistance (Neighbor for Neighbor Heat Fund and Care and Share; see separate listings on these programs). Care Managers help families and their children achieve wellness goals by providing support and service navigation with various health and social care systems and other resources. Also provides a care management program centered around helping seniors access benefits, transportation, health care, and social services.
