Offers Nassau and Suffolk County residents with intellectual and/or developmental disabilities and their families the ability to select services and providers that are most suited to their needs. Offers self-directed community habilitation, respite and SEMPT (supported employment). Assisted by a Fiscal Intermediary and Support Broker, individuals and family members learn how to implement HCBS (Home & Community Based) waiver services establish a habilitation plan, assess needs, and budget allocated Medicaid funds.

Support Brokerage services are offered to individuals with developmental disabilities who self-direct some or all of their services. A Support Broker assists the individual initially with developing a self-directed budget and then with the day-to-day management of self-directed services. A Support Broker provides support and training to individuals and their families regarding the ongoing decisions and tasks associated with self-direction. Fiscal Intermediary services include tasks which support a participant who self directs an individualized budget. This includes tasks such as billing and payment of approved goods and services, fiscal accounting and reporting, Medicaid and corporate compliance, and general administrative supports. The Fiscal Intermediary is the employer of record for staff hired by the participant know as "Self-Hired".
