Financial assistance to offset expenses associated with medical care such as co-pays and co-insurance, insurance premiums, and prescription drug costs as well other expenses such as rent, utilities, and vehicle gas; and, information and referral to other resources.

Provides help with navigating the 511 NY Southern Tier Rideshare services. Helps eliminate transportation barriers to and from medical appointments. Offers bus tokens and gas money to individuals in need of transportation assistance for non-emergency medical appointments, finding employment, groceries, and applying or re-certifying for services at a human service agency.

Serves consumers whose transportation needs cannot be met by Medicaid transportation and to ensure they have access to medical appointments and HIV related services. Individuals are provided cab rides, bus tokens, or gas cards to meet with their medical providers.

Provides outreach to Veterans identified as at risk of suicide, provides a baseline mental health assessment to potential participants in the program, and provides the following services for eligible participants: - Coordination of emergency treatment for participants, including coverage of any initial monetary charges for treatment, as long as the participant receives ongoing clinical services through the Department of Veterans Affairs (VA) - Case management with a holistic, individualized approach - Peer support - Assistance in obtaining VA benefits, including but not limited to vocational and rehabilitation services, employment and training services, educational assistance, housing assistance, and health care (including mental health care) services - Assistance in obtaining and coordinating with other public benefits for crisis-related needs, including health care services, daily living services, legal services, personal financial planning, credit counseling, and employment and vocational services - Eligible temporary income support assistance - Transportation assistance in the form of public transportation tokens - Childcare payment assistance for children under the age of 13 - General suicide prevention stability assistance, which can assist participants with funds to cover anything that may be contributing to a suicidal factor specific to a participant - Other suicide prevention services that are necessary for improving mental health such as adaptive sports, equine therapy, indoor or outdoor recreational therapy, substance use reduction programming, and individual, group, or family counseling All financial assistance is provided directly to third parties for services to veterans, and funds do not go directly to veterans themselves. ##Veteran##

Flex funds available to help with expenses like internet, cell phones, cleaning and hygiene products, and transportation. During the holiday season, provides assistance with coats, food vouchers to purchase ingredients for holiday meals, and toys/gifts for families who are not receiving gifts from Toys for Tots or Salvation Army.

Provides emergency assistance/referral services for needs that if not taken care of will affect the immediate health and safety of someone. Cases are reviewed on an individual basis, no monies for rental security deposits provided. Monetary assistance is limited with all other resources having been exhausted. Proof of any monetary need must be provided.

Due to the COVID-19 crisis, assistance is available with food and hygiene products, typically in the form of food vouchers/gift cards. Provides financial assistance, budgeting, advocacy, and referral. Gas cards available to assist with traveling to medical appointments. Counseling offered for depression, stress and anxiety, grief and loss, communication issues, problem solving, family transitions, and parent/child conflicts. Offers grief support groups and Domestic Violence Victims weekly support groups. Confidential location of the Domestic Violence Victims group is shared with women when they call for information. Other support groups may be formed based on need or upon request.

Provides gas cards, bus tokens, and monthly bus passes to help people get to their appointments, including for care coordination / case management, dental care, housing services, legal services, primary or specialty health care, pharmacy, substance use services, mental health services, support groups, and more.

Provides counseling services including adoption counseling, advocacy, death/grief counseling, family counseling, and individual counseling, including for teens. Also provides emergency assistance with food (food vouchers). Offers domestic violence sessions for men who are mandated by the criminal justice system or an agent of the courts for 26/52 sessions. Day and evening sessions available. Community educational presentations, training, technical assistance, and consultation provided. Flex funds available to help with expenses like cell phone bills, internet, cleaning and hygiene, and transportation expenses. During the holiday season, provides assistance with coats, food vouchers to purchase ingredients for holiday meals, and toys/gifts for families who are not receiving gifts from Toys for Tots or Salvation Army.
