The 2-1-1 Transportation Specialist is responsible for providing solutions-focused assistance to a caseload of clients, particularly individuals underserved by transportation services, individuals in rural communities, and inter/cross-county commuters lacking consistent access or affordable transportation options.Services includetrip planningassistance with Medical transportation options including Medicaid Transportation Employment related transportation assistanceproviding access to emergency ride home program, free bus passes for TCAT, Gadabout and cross county transit providersassistance with long distance trip planning, help with booking city bus rides (Flixbus, Ourbus)

Offers individuals and families struggling to meet their basic needs assistance, which includes a listening ear, referrals to existing programs and services, and, when needed, material goods and financial assistance.Cooling Centers

Provides transportation for seniors and to and from senior congregate meals sites and socialization activities as well as to medical appointments, shopping, banking, and other personal needs.

Provides emergency assistance to individuals and families in need, including low-cost prom attire for teens.

*Offers line run services, shuttles and public bussing.

* Offers transportation assistance for individuals living with HIV when they cannot be met by Medicaid Transportation.* Ensures access to medical appointments, social security, and HIV related services. Individuals are provided cab rides, bus tokens or gas cards to meet with their medical providers.

* Schedules non-emergency transportation or reimbursement for transportation to and from medical appointments.

Provides those who have a mental health diagnosis with transportation services. Services can include mental health appointments, primary doctor, support groups, pharmacy, food pantry, and DSS for benefit management.

Provides supplementary financial assistance for treatment for leukemia, lymphomas, multiple myeloma, and specific myelodysplastic syndromes. Assistance covers costs for certain prescription drugs, certain x-rays, transportation, and blood services. Formal application required.

Operates a coach-type bus which serves most of Delaware County. Different fixed routes, operating three (3) days a week, have been established to help riders meet their shopping and social needs. Coordinators take reservations for those persons wishing to ride the bus. For a list of the coordinators and a copy of the bus schedule, please call. Also offers limited transportation service to meet non-emergency health care appointments and pharmaceutical needs.Transportation arrangements should be made 5 business days in advance. 24 hour notice is requested for cancellations. Four requests per program year are allowed per client.

Provides transportation for elderly and disabled residents to medical and non-medical appointments as well as local shopping trips.

* Provides members of the center rides from their homes to the center and return home after program.

Transportation is available for eligible older adults and people with disabilities to reach their doctors, run errands, and more. A Medicaid transportation provider.

* Provides non-medical help, including: Transportation and escort for errands and appointments. Medication reminder. Overnight care. Companionship and conversation. Safety and supervision. Stand-by assistance. Housekeeping assistance. Meal preparation. Respite care.

Provides transportation and escort services for seniors to health related appointments at facilities in Herkimer, Oneida, and Otsego counties.

* Provides safe shelter for victims of domestic violence and their children. * Offers crisis intervention services for survivors of sexual assault, domestic violence, stalking, child abuse, and/or human trafficking, and their families. * Provides personal, community, criminal justice, educational, medical, and legal advocacy. * Provides short-term and long-term counseling. * Provides case management, housing support and access to legal services.

* Engages senior citizens into service, including: Volunteer Transportation Corps: provides medical transport for other seniors. Bone Builders: Lead low-impact exercise classes for other seniors. Tax Counseling for the Elderly: Provide IRS sanctioned tax preparation services for other seniors.* Provides training for all opportunities.

Volunteers drive sick and disabled veterans to and from VA medical facilities for treatment. Syracuse VA Medical Center- currently not offered due to driver shortage appointments must end by 2:30 pm, the latest the van leaves for Ithaca/Cortland. Riders should consider this a full-day trip. Provides financial assistance to Columbia and Greene County residents experiencing hardship due to illness or injury. Financial assistance to offset expenses associated with medical care such as co-pays and co-insurance, insurance premiums, and prescription drug costs as well other expenses such as rent, utilities, and vehicle gas; and, information and referral to other resources.
