Enhanced care is offered in a fourth home for persons living with advanced dementia and needing assistance with all aspects of daily life. Each residence is secure and Elders are able to enjoy time outdoors in the secure courtyard.

Offers supported housing in the form of community residences or group homes for adults and children with mental illness tp help them maintain independence.

Eligible individuals can receive information and referrals for Expanded in-home services for the Elderly Program and Medicaid services such as:- Referrals for home care services (personal care aides and consumer directed personal assistants) for low to moderate income levels for Tompkins County residents, including senior citizens and residents of any age with disabilities who need long-term care.- In-home assessment for an evaluation to determine specific Long Term care needs along with case management for ongoing assistance and monitoring of these services- Assistance with finding appropriate residential care.- Guidance through the process of nursing home placement by completion of Patient Review Instrument (PRI), a nursing assessment required by New York State for nursing home placement.- Referrals to local adult care homes.Traumatic Brain Injury- Assistance to referral to Traumatic Brain Injury waiver service, including completion of ongoing assessments required for recipients. Nursing Home Transition Diversion- Assistance with referral to waiver service, and completion of ongoing assessments required for recipients.Private Duty Nursing- provide information for application process and services, for individuals who require continuous skilled nursing care.Referrals for Personal Emergency Response System, FoodNet, HEAP and WRAP.Family type home- provide information for requirements of starting a family type home.

The Residential Services Program of The Arc of Madison Cortland operates different types of residential homes in order to meet the needs of adults with developmental disabilities. In order to be eligible for placement in the Residential Services Program, an individual must have a documented diagnosis of a developmental disability and be at least 18 years of age.

Serves adults with short-term rehabilitation or long-term care needs in skilled nursing facility.Two individualized residential alternatives and services for adults with developmental disabilities. Referrals made through the Office of Mental Retardation, Broome Developmental Services.

Provides one bedroom apartments for consumers with HIV-related disabilities.

Provides supervised living for people with developmental disabilities.

Offers a supervised community residence program and intensive supportive apartment program which provide structured and supportive environments for adults with psychiatric disabilities. Designed to ensure that those in need will receive the support and services necessary to maintain the most appropriate level of independent living possible.

Provides housing support for people with developmental disabilities. Residential services include houses and supportive environments with 24-hour staff support as well as environments in which individuals live on a largely independent basis but receive support as needed.

Offers a variety of living arrangements such as community residences, apartment programs, supported housing programs for individuals with mental health disorders, depending on needs. Community residencies include individualized goals, medication management, life skills training, and supportive staff visits vary between each type of residential unit.

Offers residential habilitation, day habilitation, service coordination, and respite services to people with developmental disabilities.

Operates a apartment program for individuals with a mental illness diagnosis. Provides support, monitoring, and advocacy to assist with independent living skills.

Full continuum of care- from independent living in an apartment, to assisted living offering both memory care and assisted living options to accommodate varying needs, skilled nursing care. Also offers a wellness and fitness facility with certified trainers ready to help residents of all abilities,and a short-term rehabilitation unit.
