Provides Family Care and Individual Residential Alternatives (IRA) for developmentally disabled persons ages 18 and older who are HCBW eligible. Intermediate Care Facility is available for those who are Medicaid eligible. Transportation is available for all. Supportive apartments are also available, with limited transportation. Provides respite care allowing families of developmentally disabled persons to take a break from the daily routine of care-giving duties. Currently, two locations in a certified home can offer respite for as little as one day or for a few weeks. Reservations are encouraged; however, emergency accommodations are available. Provides At-Home Residential Habilitation service that assists individuals in improving the skills necessary to live in the community while living at home. Qualified staff work one-on-one with individuals to help them learn socialization, recreation, daily living, and mobility skills.

OMH licensed Service Enriched Single Residence Occupancy (SRO). Single apartments provided in a supportive community. Case management and activities available. Smaller staff to resident ratio than a CR or a TAP.

96-bed Assisted Living Program serving the frail elderly 65 years or older. Provides room, board, and assistance with medications and activities of daily living. Services are provided by nurses and home health aides.

Assisted living facility that provides 24 hour supervision, case management, meals, personal care, housekeeping, laundry, activities, transportation service, and assistance with medication requirements. STEL also has an adult home in Fredonia under the auspices of the Department of Health.

Assisted Living Facility and retirement home (57 residents). Residents are provided three meals per day, along with housekeeping, maintenance, activities, transportation, and physician services one day per week. Medication supervision, personal care, supervision, and assistance provided. Weekly religious service and pastoral care (in-house) offered. Beauty/barber shop (in-house), shopping services, and podiatry services on site. Additional charges for suites.

Residential and home services to adults and children with developmental disabilities and/or mental illness, including respite services, supervised congregate care, and family care. Transportation is available for Family Care and IRA. Supportive Apartments are available to those individuals with developmental disabilities ages 18+.

Offers a supervised community residence program and intensive supportive apartment program which provide structured and supportive environments for adults with psychiatric disabilities. Designed to ensure that those in need will receive the support and services necessary to maintain the most appropriate level of independent living possible.

Wildwood's residential programs serve individuals with all levels of disability and are designed to provide individuals with the highest quality services while providing the feeling that where they live is truly home. * Provides assisted living memory care in 3 homes each housing 14 elders. Enhanced care is offered in a fourth home for persons living with advanced dementia and needing assistance with all aspects of daily life. Each residence is secure and elders are able to enjoy time outdoors in the secure courtyard.

Mental Health Community Residence is a 12-bed residence providing 24-hour care to people who are overcoming a serious mental health diagnosis. Supportive Apartments are available for those needing minimal supervision, up to 21 hours per week. Individual Residential Alternatives (IRA) offers 24 hour supervised living in various sites throughout Orleans and Genesee County.

Group Homes: room and board, supportive counseling, individual service planning, medication management, behavior intervention, case management, socialization, transportation, crisis management, training in activities of daily living, discharge planning, and follow-up. Independent living in apartment of choice with minimal staff support. Programs funded by HUD are for homeless mentally ill; housing stipend. Other programs are for non-homeless, low income (SSI) mentally ill adults. Priority access for those who qualify for Erie County Care Coordination Program. Limited number of beds available for those graduating from a licensed community residence program.

Supervised group home. Provides skills training and support in all areas of daily living.

Residents 18-21 years old live in supervised group home and supported apartments. Skills training and support provided in all areas of daily living.
