Residential and home services to adults and children with developmental disabilities and/or mental illness, including respite services, supervised congregate care, and family care. Transportation is available for Family Care and IRA. Supportive Apartments are available to those individuals with developmental disabilities ages 18+.

Provides residential apartment homes in an independent living environment for seniors. Also offers priority access to all levels of care provided on campus, including assisted living at GreenField Court, enhanced assisted living and memory care at GreenField Terrace, and health/rehab services at the GreenField Health and Rehabilitation Center. One and two bedroom apartments available, each with a fully equipped kitchen, washer/dryer, individual heat/air conditioning units, and walk-in shower. Some apartments feature fireplaces, patios, and porches. Amenities include 24 hour security, flexible meal plan, courtesy transportation, scheduled activities, a pond, and a gazebo along walking paths.

Provides a safe, natural, family setting for individuals to receive the care and support they need, with the independence to explore their own interests. Perfect Fit's unique residential setting welcomes individuals with warmth, compassion, and encouragement, enabling them to gain a genuine feeling of acceptance through stable relationships and community participation.

Supervised group home. Provides skills training and support in all areas of daily living.

Subsidized apartments (150 units) for seniors 62 and older, including 15 accessible units for adults 18 and older with disabilities. Enriched Housing Program (45 units) provides services to residents 55 and older, catering to those who are low-income. Program provides meals, housecleaning, etc. Apartments are one bedroom, handicap accessible, and HUD subsidized.

Residents live in a group home with 24-hour supervision. Skills training and support provided in all areas of daily living.

OMH Certified Treatment Apartment Program. A staff person visits these apartments 3-5 times/week, relevant to provision of community rehabilitation skills. Staff are available 24 hours a day, 7 days a week.

96-bed Assisted Living Program serving the frail elderly 65 years or older. Provides room, board, and assistance with medications and activities of daily living. Services are provided by nurses and home health aides.

Offers traditional community residences, or one- and two-bedroom apartments for adults with a mental health diagnosis. Support and assistance are provided in the areas of daily living skills (budgeting, parenting, assertiveness training, medication management, and vocational and educational skills.

Provides supervised living for people with developmental disabilities.

Provides 24/7 supervised residential supports and supportive supports, or less than 24 hour supports, to people with developmental and intellectual disabilities.

Provides services for individuals with developmental disabilities including service coordination and supervised living services through Independent Residential Alternatives (IRA).

Offers a variety of living arrangements such as community residences, apartment programs, supported housing programs for individuals with mental health disorders, depending on needs. Community residencies include individualized goals, medication management, life skills training, and supportive staff visits vary between each type of residential unit.

Offers supported housing in the form of community residences or group homes for adults and children with mental illness tp help them maintain independence.

Designed to ensure that those in need will receive the support and services necessary to maintain the most appropriate level of independent living possible.

A nurse/care manager on site to develop an individual care plan and coordinate any additional required or desired services. Licensed Assisted Living Program (ALP) community, providing additional support services for those who are medically eligible and qualify.
