Offers 58 scattered site, furnished apartments to chronically homeless individuals as part of a Housing First Model. The Housing First Model does not put conditions on housing; treatment and sobriety are not conditions, and income and ID are not required. Low to medium demand supportive case management is provided. The resident will be assisted with obtaining food, appropriate clothing, identification, benefits, medical services, and follow ups. Opportunities for work activities to develop skills and income potential are made available.

Residents live in scattered site apartments. Skills training and support provided in all areas of daily living. Staff can visit 7 days/week, depending on need.

Provides housing in a service-enriched, recovery-oriented setting for adults with psychiatric disabilities. Program is licensed by the New York State Office of Mental Health and has a residential atmosphere with individual bedrooms (Single Residence Occupancy), inviting common areas and outdoor courtyards. Other services include 24-hour staffing, daily living skills training, crisis management, medication management, linkages to medical and dental care and health education services, social/recreational assistance, and vocational/educational services among others. 80 beds available.

OMH Licensed Service Enriched Single Residence Occupancy (SRO): Efficiency apartments in a supportive community. Case management, activities, and 3 meals a day are provided. Smaller staff to resident ratio than a Community Residence or a Treatment Apartment Program.

The Homestead is a community dedicated to providing active retired adults with a comfortable and enjoyable lifestyle. Every resident is given ample opportunity to partake in activities tailored to his or her personal interests. Offers a variety of Independent Living residences (cottages and apartments) to fit any lifestyle, including the Hillview Cottages. Bergquist Memorial Assisted Living Residences offers 32 assisted-living homes with an all-inclusive fee.

A residential service for persons who have been diagnosed with serious mental illness. The program places these individuals with community members certified to deliver residential care in their own homes. BPC staff supervise the Family Care Home and provide case management services for the residents. The home provides meals, housekeeping, medication administration assistance, and support with day to day stressors and routines.

Offers supervised residences for individuals with developmental disabilities. Homes are inspected and certified by OPWDD. Services provided are based upon each individual's specific interests and needs.

Provides a safe, secure, home-like setting to encourage individuals with intellectual and developmental disabilities to live independently at every stage of life. Residences are supervised by staff trained in all aspects of care and a registered nurse oversees health care needs.

Offers full range of residential alternatives for intellectually and/or developmentally disabled adults.

Group Homes: room and board, supportive counseling, individual service planning, medication management, behavior intervention, case management, socialization, transportation, crisis management, training in activities of daily living, discharge planning, and follow-up. Residents live in supervised group home (14 beds). Skills training and support provided in all areas of daily living. Staff on premises 24 hours a day.

Offers a continuum of living opportunities designed for individuals of all ages who have mild to profound developmental delays, multiple physical disabilities, specialized medical needs, dual diagnosis, or dementia. An individualized plan of services is developed for all residents, which may include Intensive Medical Services, Nursing Supports, Therapy Services, Person-Center Planning and Supports, Behavioral Supports, Wellness and Nutrition Training, and Dementia/Memory Care.

96-bed Assisted Living Program serving the frail elderly 65 years or older. Provides room, board, and assistance with medications and activities of daily living. Services are provided by nurses and home health aides.

Provides Family Care and Individual Residential Alternatives (IRA) for developmentally disabled persons ages 18 and older who are HCBW eligible. Intermediate Care Facility is available for those who are Medicaid eligible. Transportation is available for all. Supportive apartments are also available, with limited transportation. Provides respite care allowing families of developmentally disabled persons to take a break from the daily routine of care-giving duties. Currently, two locations in a certified home can offer respite for as little as one day or for a few weeks. Reservations are encouraged; however, emergency accommodations are available. Provides At-Home Residential Habilitation service that assists individuals in improving the skills necessary to live in the community while living at home. Qualified staff work one-on-one with individuals to help them learn socialization, recreation, daily living, and mobility skills.

* Offers learning and enriching experiences through Chandler Day Habilitation Center through a variety of excursions and activities.* Oversees seven supervised residential settings.* Provides Community Habilitation Services to individuals with various services designed to assist individuals in acquiring, retaining, and improving the self-help, socialization, and adaptive skills necessary to interact in the community.* Provides employment services to individuals through SEMP, ETP, and Pre-Vocational Services.

* Provides independent living patio homes and apartments for older adults.* Provides assisted living apartments for older adults and receive person and comprehensive care.* Provides dementia management through Memory care that consists of a personalized care plan that supports continued active life.

Provides a congregate care setting serving older adults with psychiatric disabilities that works with residents on the skills needed to build independence.

Offers traditional community residences, or one- and two-bedroom apartments for adults with a mental health diagnosis. Support and assistance are provided in the areas of daily living skills (budgeting, parenting, assertiveness training, medication management, and vocational and educational skills.

* Provides residential services to adults with Intellectual and Developmental Disabilities (I/DD) in 24-hour staff supervised homes as well as a supportive apartment program.

Provides supervised living for people with developmental disabilities.

Provides 24/7 supervised residential supports and supportive supports, or less than 24 hour supports, to people with developmental and intellectual disabilities.

Assists participants receiving Medicaid; and have one or more documented physical disability, and two or more chronic conditions (e.g., asthma, diabetes). All participants must fit in the category of being unstably housed/or homeless, and high utilizers of Medicaid. Referrals are to be made by Hospitals, Managed Care Organizations, Medical Respites, Performing Provider Systems, or a Skilled Nursing Facility. Assist individuals with the eligibility process, locating and subsidizing housing units; and obtaining services necessary to support successful living in the community. Community Transition Services (CTS) are available, on a case by case basis, to assist with security deposits, moving expenses, startup utility payments, and household furnishings/items.
