This is a permanent supportive housing program that provides supportive case management, as well as rental subsidies.Tenants who are enrolled in in the program work in collaboration with case management services in order for tenants to overcome barriers in achieving their desired outcome. The goal of the program is to transition chronically homeless individuals and families into stable housing and assist them in achieving financial independence.Referrals are accepted from other agencies that work with homeless populations. We document and record past living situations and establish a plan that involves both what actions the case manager will take and the steps the clients will follow to realize their goals.

Provides services for a monthly fee, including all utilities (heat, electricity, air conditioning, telephone, cable TV, high-speed Internet access.) Services include: three restaurant-style meals daily, private studio apartments with kitchenette, assistance with bathing, dressing and medication management, housekeeping, laundry and linen services, 24-hour security and emergency call system, licensed nursing staff and resident care assistants, social activities, cultural, recreational and educational programs, scheduled transportation

CARES provides supportive scattered site residence for people with HIV and AIDS. CARES works to address unmet housing and services needs among homeless and-or disabled persons within local communities. CARES works with persons living with HIV-AIDS, other disabilities and-or other life challenges, serving as a catalyst for improving the quality of life in local communities. It does so by helping communities address unmet needs as well as create and coordinate the resources necessary to address these needs.

Private accommodations, extensive residential services, community facilities and amenities, plus comprehensive, on-site health care for life, on a 105-acre campus.Residents live independently in cottages and apartments and can move into assisted living or skilled nursing care with single rooms and private baths when this care is needed.Costs include an entrance fee and ongoing monthly fee, and residents enter into a lifecare or continuing care contract for life

Offers two levels of residential environments for individuals with developmental disabilities.
Supervised Living - allowing residents active involvement in daily programs, around-the-clock supervision and support to enhance and maintain independence to live, work, socialize and participate in community life.
Supported Living - offer oversight, guidance and intermittent support to residents who can live on their own, or with a roommate, in the community.

* Provides OMH Apartment Treatment Program, which includes 24-hour support to residents to enhance their independent living skills in a less restrictive setting and under realistic conditions of personal responsibility while still having access to program staff on a daily basis for instruction, assistance, and support.

* Provides OMH Supportive Scattered-Site Housing, which includes supportive services and financial assistance to adults age 18 or older at risk of homelessness who are diagnosed with a serious mental illness.

* Serves individuals in recovery from mental health and substance use disorders.* Offers customized services based on individual need and include: Evaluation and treatment. Family education and support. Coordination with other service providers. Case management.

Coordinates with adults, ages 21 years and older, including family members, who are willing to provide foster care to dependent children who've been removed from their homes and need alternative family arrangement.

Also, assists children or adults with developmental disabilities, sensory impairments, physical disabilities, emotional problems or multiple disabilities who are unable to live with their birth families or in an independent setting.

* Provides assisted living memory care in 3 homes each housing 14 elders. Enhanced care is offered in a fourth home for persons living with advanced dementia and needing assistance with all aspects of daily life. Each residence is secure and elders are able to enjoy time outdoors in the secure courtyard.

Offers supervised and supportive housing in the community for people with chronic mental illness and for those with developmental disabilities.

* Offers an Assisted Living community that includes the following services: Three meals daily in a family dinning setting. Light housekeeping and laundry service. A nurse/care manager on site to develop an individual care plan and coordinate any additional required or desired services. On site nursing support, with home health aides to help with routine personal care needs. Assistance with medications. Social, recreational, and spiritual activities. Beauty salon and barber shop. Provides 24/7 supervised residential supports and supportive supports, or less than 24 hour supports, to people with developmental and intellectual disabilities.

Provides a daily meal, shelter, showers and laundry, and case management services to homeless and chronically homeless individuals. Offers communication tools such as mailboxes, telephone, and copier as well as storage lockers. Offers a 10-bed permanent supportive housing program to eligible chronically homeless individuals.

Offers site-based supportive housing for chronically homeless adults with a disability. Provides support to formerly homeless families with children when the head of the household has a disability. On-site services include care management, advocacy, life skills and daily skills training, and other supportive services are offered to assist tenants achieve and maintain independence.
