This program is designed to permit persons with disabilities to maintain residence or achieve residence either in their own homes or in rental units where other assistance is not available. Repairs will be limited to accessibility issues only.

Purchases and installs modular aluminum ramp systems for use by older persons.Agency retains ownership of ramps: when a household no longer needs the ramp, it is removed, reconfigured and installed for use by another household.

ATI provides two major services relating to making agencies, businesses and public facilities accessible to people with disabilities. * Provides peer support for people with disabilities.* Offers an employment program to help people with disabilities increase their employability.* Offers benefits advisement by providing information and advocacy while navigating government health and human service systems.* Offers healthcare advocacy by providing information and assistance with regard to Medicaid, Medicare, Medicaid Managed Care, and Medicaid Funded Services.* Works in conjunction with NY Connects in Monroe, Ontario, Yates, and Wayne County, to inform, assist, and advocate for individuals, caregivers, and family members, seeking community-based long-term services and supports.* Provides housing advocacy by offering information and advocacy on affordable and/or subsidized housing, rental listings, accessibility, home renovations, and home ownership.* Offers peer support groups and services for persons with disabilities.* Provides referrals for equipment loan.* Provides education, assistance to and advocacy for disabled persons wishing to vote.* Provides independent living skills to assist with daily living activities, such as; money and medication management, nutrition, legal rights, socialization, and mobility.* Provides community outreach and education through workshops, classes and other activities to promote awareness of needs of persons with disabilities, and understanding of independent living.

Screens for potential funding sources and for those who are not eligible for funding, arrange for affordable construction at cost of the materials.

Available items include wheelchairs, walkers, Rollators, canes, crutches, ramps, grab bars, Hoyer lifts, adaptive seating, shower chairs, commodes, wander alarms, weighted items, adaptive utensils, laptops, iPads and deaf and blind/low vision technology. We also have learning aids, toys and games for early intervention, as well as pediatric equipment. Free delivery may be available.

Provides independent living skills, information and referral, advocacy, peer counseling, transitional services, benefits and entitlements advisement, educational advocacy, nursing home transition into the community, individual support services, Olmsted housing subsidies, and support groups for people with disabilities. Transportation available. Loan closet provides free, gently used medical equipment including wheelchairs, walkers, canes, crutches, shower benches, raised toilet seats, portable ramps, and more, based on availability. Delivery available during the COVID-19 pandemic to assist people with disabilities to remain in their homes.

Provides behavior management, counseling, crisis intervention, family reimbursement, guardianship assistance, benefits and entitlements assistance, home modification, independent living skills, information and referral, parenting skills for parents of developmentally disabled children (in or out of home), service coordination/case management, support groups, and transitional planning for persons with developmental disabilities of all ages. Developmentally disabled parent training is provided to first time parents with a developmental disability. Applied Behavioral Analysis (ABA) Program available for parents of children with autism.

Grant bringing accessibility modifications to income eligible homes. These modifications allow disabled individuals to return to or remain in their homes. They also assist in obtaining wheelchair ramps.

Assists with home weatherization and adaptations such as ramps, grab bars, barrier removal, and modifications.

Provides financial assistance for low to middle income seniors and individuals with disabilities who need to make accessibility modifications to their apartment or home.

Offers information, education and referral about adaptive equipment, accessibility, disability awareness, ramps and home modifications, medical equipment for loan, and civil rights for people with disabilities.

Provides free home repairs and accessibility modifications for low-income families, seniors, individuals with a disability, and Veterans.

Meets the needs of home-bound individuals while providing youth the opportunity for hands-on learning and community service through the construction of wheelchair ramps.

Offers information, education and referral about adaptive equipment, accessibility, disability awareness, ramps and home modifications, medical equipment for loan, and civil rights for people with disabilities.

* Offers the Home-Safe-Home program. Provides home safety review geared toward fall prevention. Minor home modifications such as installation of grab bars, smoke detectors, door grips, accessible toilet seats and hand rails.
