Provides easy, local access to information and assistance for consumers and their families who are exploring available options for long term care. An effort is made to provide all available care options so as to enable consumers to remain at home. Also conducts screenings to determine eligibility for different services. Will conduct in-home assessments as necessary. Serves as the point of contact for the Senior Farmers' Market Nutrition Program, which provides low-income seniors with coupons that can be exchanged for eligible foods (fruits, vegetables, honey, and fresh-cut herbs) at farmers' markets, roadside stands, and community supported agriculture (CSA) programs.
