Personal Care Services Aide Program: Doctor's orders requesting service. Any concerned person, family member, friend, neighbor, law enforcement officer, health professional, clergy or financial institution employee who observes an individual having difficulty in providing for their most basic needs (such as food, clothing, shelter, medical care, or personal safety) are urged to call 585-344-2580 ext. 6484 to make referral. Individuals recognizing their own needs for help should also contact this number for assistance. All referrals accepted for an adult service assessment are evaluated for Adult Protection eligibility, regardless of the type of assistance requested by the referral source. Usually begins with a home visit by a caseworker within three business days after receiving the referral. If the situation is considered life threatening, the home visit will be made within 24 hours.

Description
Services provided without regard to income to assist people age 18 or older who have physical or mental impairments to the extent that they cannot meet their basic needs for food, clothing, shelter or medical care; are unable to protect themselves from neglect, abuse or hazardous situations; and have no one willing or able to help them in a responsible manner. Personal Care Services Aide Program: Provides some or total assistance with personal hygiene, dressing, and feeding; nutritional and environmental support functions and health related tasks. Such services should be essential to the maintenance of the patient's health and safety within his/her own home, ordered by the attending physician based on an assessment of the patient's needs by a qualified person in accordance with a plan of care, and supervised by a registered professional nurse. Consumer Directed Personal Assistance Program: New York State recognizes that some patients who need personal assistance also have a strong desire to manage their own services. Consumer Directed Personal Assistance allows patients to screen, interview, hire, train and supervise their attendants.
