Chronic Disease Self Management Program - Niagara County Department of Health
A peer-led health education program that utilizes the Chronic Disease Self-Management Program (CDSMP) curriculum. Six weekly 2.5-hour workshops help participants learn or enhance their skills and ability to manage health and maintain an active lifestyle. Workshops are for anyone dealing with a chronic or limiting health condition, such as arthritis, high blood pressure, heart disease, chronic pain, or anxiety in order to help them take charge of their life. Offers support and information on practical ways to deal with pain and fatigue, better nutrition and exercise practices, new treatment choices, and how to talk with family and medical professionals about personal health. Helps participants to set their own goals and make a step-by-step plan to improve health and quality of life. Book provided. Caregivers are also welcome to attend workshops, which take place at various locations throughout Niagara County; call for details.
