AGING AND ADULT SERVICES PROGRAM | CATHOLIC CHARITIES FAMILY AND COMMUNITY SERVICES

* Provides individual and family support services for older adults that include: Telephone information and assistance. Geriatric wellness screening. Case management. Benefits advocacy. Caregiver training. Nutrition centers.* Offers money management for older adults.* Provides assessment and treatment for homebound older adults experiencing anxiety and/or depressive symptoms, in home counseling available, up to six sessions and maybe matched with volunteer coach to support independence and connecting with the community through the PATHS program.

Sliding scale for some services. PATHS: no fee for services.

Eligibility

Ages 60 years and older or their caregivers. PATHS/PEARLS: Experiencing anxiety and/or depression. Residing in a community setting (single home or apartment). Unable to access traditional counseling services.
