ASSERTIVE COMMUNITY TREATMENT (ACT) | NORTHERN RIVERS FAMILY OF SERVICES

Provides wrap around services to individuals who have been diagnosed with a severe and persistent mental illness. Assists with service planning and coordination, housing supports, money management, family education, substance abuse treatment, vocational support, mental and physical health treatment, and community referrals to offer support, rehabilitation, and treatment within the community.
