* Provides resources and education by community health nurses to pregnant women, postpartum women, and pediatric patients that offer: Medical monitoring. Health education. Pre-and post-natal care. Case management for high-risk pregnancies. * Offers an immunization clinic. * Provides screening for lead poison. * Offers the Pediatric Home Visit Program for children with acute/chronic health needs. * Offers the Early Intervention Program for children with disabilities or delayed development. * Offers the Infant Child Health Assessment Program/CHAP that provides screenings for physical, social and language development.
