Application process

Appointments required; call to schedule. Clients must bring with them: 1. Proof of all household income and expenses for the past 30 days, this includes Food Stamp Budget Sheet. 2. Identification for all household members, including verification of children's ages. 3. Social Security numbers for all household members (1 year and older). 4. Birth certificate and social security card (or school enrollment paper if child does not have SS card) of children. 5. Picture ID for the person making the application. 6. Proof of current address.
