FAMILY TIES SIBLING DIAGNOSTIC AND GROUP HOME | HOPE FOR YOUTH

Offers diagnostic and emergency placement for sibling groups removed from their homes or in the process of placement transition. The program can accommodate 12 children and offers a 24 hour professionally supervised environment. The program provides crisis and preventive respite services for youth and their siblings. Referrals must come from Nassau or Suffolk County Departments of Social Services as well as the Mental Health Crisis Respite Bed System.

Eligibility

Sibling groups ages 6 months to 18 years old at time of intake and ages 12 and under for single child placements. The children must be in the custody of the Department of Social Services or eligible for service through the Mental Health Crisis Respite Bed System. Access to all Emergency Housing for Homeless Persons Required a Referral by a DSS office location/hotline operator. PINS Diversion Clinical Services), and the Family Service Center (outpatient chemical dependency for youth substance abuse clinic) Referrals accepted from Department of Social Services and Department of Probation for both Nassau and Suffolk County, Family Courts, and Mental Health Crisis Respite Bed System.
