May be done through a phone screening, child/ family assessment meeting, collaboration with the referral source and other current providers, record review, and presentation to the SPOA Committee for service planning.

Any family with a child between ages 5 to 17 years that exhibits impacting behaviors, at risk of hospitalization or placement outside of the home, involved in multiple systems, and has needs that are not met in natural environment
