Provides health insurance for children with services including: check ups immunizations prescription drugs eye and dental care emergency careParents choose their child's personal Primary Care Provider (PCP) from a network of physicians.Cost of coverage depends on family size and income; in some cases, coverage is free.July 2017- If a caller enrolling in Fidelis Child Health Plus and receives eligibility after the 15th of the month, they can contact the local Representative to back date the policy start date to the 1st of the following month

Fidelis Care Certified Application Counselors assist local residents access health insurance. Fidelis Care also provides local residents with quality, affordable health insurance coverage through New York State's Child Health Plus and Medicaid Managed Care programs. Fidelis also offers a Medicaid Managed Long Term Care Program called "Fidelis Care at Home". Fidelis Care at Home is for those who are 18 or older, have Medicaid and are eligible for nursing home level of care. Call for more information.
