Description
Provides services that can show early identification through a developmental evaluation, then review the family concerns, prioritize needs, gather resources, develop an individualized plan, coordinate services to ensure that the individual and their family receive the needed care. Also provide day habilitation for older individuals to continue life skills training and get them to reach their highest level of independence.
