Services include Americans with Disabilities Act consultations and architectural assessment, Chronic Disease Self-Management Program, Supported Employment, Benefits Planning and Advice, Information and Referral, Peer Support both one-on-one and in groups, Individual Advocacy and Systems Advocacy, Community-Based Activities, Educational Advocacy, Family Engagement Events, Community Health Workers, NY Connects and Long Term Care Ombudsman Program.
